5. Supply Requests (Total) – Total supply requests for the year October 2015 through September 2016:

6. Supply for WMD‐CST – Supply requests processed for WMD‐CST for the year October 2015 through

September 2016 : 7300 requests processed – ranging from 509 to 771 per month

7. Supply for CERF‐P – Supply requests processed for CERF‐P for the year October 2015 through September

2016: 1050 requests processed – ranging from 36 to 196 per month

8. Supply for HRF ‐ Supply requests processed for HRF for the year October 2015 through September 2016:

600 requests processed – ranging from 11 to 98 per month

9. Supply for FSRT ‐ Supply requests processed for FSRT for the year October 2015 through September

2016: 450 requests processed – ranging from 9 to 72 per month

10. CERF‐P Events – Supported 89 CERF‐P events for the year October 2015 through September 2016 as follows:

a. CERF‐P Unit Support (EVAL/MTT/VG/NET): 75 events

b. ETAS training (Evacuee Tracking System): 9 events

c. Decon Trailer Assessment: 5 events

d. SME Training: 0 events

e. Event Support (conferences): 0 events

11. Equipment Maintenance Operations – CST Work Orders – total CST Maintenance requests processed for the year from October 2015 through September 2016: 2,750 maintenance requests processed

12. Equipment Maintenance Operations – CERF‐P Work Orders – total CERF‐P Maintenance requests processed for the year from October 2015 through September 2016: 625 maintenance requests processed

13. Equipment Maintenance Operations – HRF Work Orders – total HRF Maintenance requests processed for the year from October 2015 through September 2016: 500 maintenance requests processed

14. Equipment Maintenance Operations – FSRT Work Orders – total FSRT Maintenance requests processed for the year from October 2015 through September 2016: 125 maintenance requests processed

15. Work Orders (Total) – Total equipment maintenance requests for the year October 2015 through

September 2016: 4,000 work orders processed

16. Inbound Shipments‐ Total inbound shipments for the year October 2015 through September 2016:

3,900 inbound shipments – ranging from 230 to 500 per month

17. Report of Discrepancies (RODs) Processed (Inbound Discrepancies) – For the year October 2015 through

September 2016: 85 RODs (36 Team RODs; 49 Internal RODs).

18. Outbound Shipments – For the year October 2015 through September 2016: 5,550 outbound shipments

– ranging from 350 to 500 per month

19. Total Warehouse Inventory Value – current value of inventory is approximately $112,000,000.00

20. Number of Purchase Orders – October 2015 through September 2016: 6,000 were processed for 11,860 lines

21. Travel – SME’s travel approximately 120+ trips per year; SME’s traveled 123 trips from October 2015 through September 2016

22. HAZMAT Shipments – Total HAZMAT shipments for the year October 2015 through September 2016:

414 HAZMAT shipments ‐ ranging from 25 to 50 per month

23. Line items on the APL – 7,200 lines items (September 2016)

24. Line items in warehouse inventory – Typically 1600‐2000 inventory line items present 2015‐2016.

25. Processed by Inventory (line items in stock and shipped from the warehouse) – 5,525 processed for the year October 2015 through September 2016
