Attachment 15
Descriptive Data Required for Engineering Change Proposals (ECP) and Requests for
Equitable Adjustment (REA).
Engineering Change Proposals (ECPs) and Requests for Equitable Adjustments (REAs) shall be provided using the included Change Request Form. Each ECP or REA shall include a cover letter providing the details related to the requested change to include the following:
What is the nature of the change?
How will the change impact the work and schedule?
Have possible alternatives been investigated for use on the project change?
Provide Statement that changed condition has been verified with the Corps QA staff
Where the change was located and dates involved
Photographs and/or other documentation – before and after
Impacts to schedule – is a POP extension required or is work contained within original scheduled timeframe for the activity; number of days required
Summary of all quantity changes with unit pricing and extended pricing as required
Any required additional labor or equipment
Supporting invoices, billings, fees, subcontractor billings, etc.
Estimated costs with breakdown by labor (hours), subcontracts, travel, per diem, rentals, G&A, profit, etc.
