Attachment 12 – Demolition and Recycling Final Report

Contractor shall describe the original objectives of this project and activities, along with the final abatement, recycling, and disposal quantities and final site disposition. Contractor shall describe goals, methods, procedures, and personnel used for field activities for all Facilities Reduction Program abatement, demolition, and recycling/reuse activities. Additionally, there will be sections for lessons learned; proposed changes or adjustments to the Facility Reduction Program process for scoping, Contractor Quality Control, Government Quality Assurance; and ultimate project site closure. Contractor shall submit Final Report within 10 working days after completion of all field activities, unless a Government provides a different milestone the task order PWS. Contractor shall submit Final Report shall be submitted in both electronic (digital) and printed formats as directed in each Task Order.

Requirements:

1.0 Work Plans shall be submitted in electronic and hard copy formats as directed in each Task Order. The plan shall be printed on standard size (8 ½ by 11 inch) white paper, with drawings folded, if necessary, to this size or to fit within pockets of this size. Pages of 8 ½ by 11 inches or up to 11 by 17 inches are preferred. Pages shall be sequentially numbered. Drawings shall be of engineering quality in drafted form with sufficient detail to show interrelations of major features. When drawings are required, data may be combined to reduce the number of drawings. All attachments and appendices shall be identified and referenced in the text of the plan, and conveyed within the Table of Contents. The last page of the Table of Contents shall contain a list of Acronyms or Abbreviations used within the document. The plan shall be legible and suitable for reproduction, in both digital and print format.

2.0 Chapters shall be numbered sequentially. Within each chapter, each page shall be numbered sequentially, starting with the specific chapter number. Within each chapter, the paragraphs shall be numbered sequentially starting with the chapter number and using a decimal system, with each section and paragraph having a unique decimal designation. Within each chapter, any figures, tables, and charts shall be numbered sequentially starting with the chapter number. Printed Copies of the Work Plans shall be bound in a three- ring binder. Binders shall hold pages firmly while allowing easy removal, addition, or replacement of pages.

3.0 Appendices shall be lettered alphabetically. Sequence is Appendix, Tab, Enclosure.

Within each appendix, each page shall be numbered sequentially starting with the appendix letter, e.g., Appendix A-1, or Appendix A-1, Tab A, Encl 1.

4.0 A title sheet will be provided on the front of the binder and also inside the binder which includes the name of the project (site), contractor's name and address, the Corps of Engineers’ contract number, Task Order number, and project number, the date of the plan, and the title of the plan. The contractor’s name/logo shall not dominate the title sheet. The Corporate Quality Management representative shall sign the title sheet. The version of the final report (e.g., draft, rev#, final) shall be clearly identified on the title sheet.

5.0 Each page of the final report shall contain a dated footer and the Corps of Engineers’ contract/Task Order number. When revisions to the work plan are required, a revision date, revision number, Corps of Engineers’ contract/Task Order, and amendment/modification number (if applicable) shall be included in the date footer. A dated summary page listing all revised pages shall be submitted with each revision. This summary page shall be identified as such and placed directly behind the title page. All revisions to the Final Report shall be highlighted in yellow and the text bolded to match the information shown in the summary changes page.

6.0 Final Report Outline – Chapter, sections, or appendices not used for a specific project shall be marked: “Not required for this project.” However, the correct chapter, section or appendices placeholder shall be left within the document for reference.

6.1 Chapter 1 – Introduction. At a minimum the following information shall be included:

6.1.1 Project Authorization (Contract No., Delivery Order No., etc.) A copy of the actual award PWS and any Modification will be shown in Appendix A – Performance Work Statement.

NOTE: The Contractor shall also include within the PWS Appendix, a copy of any and all Asbestos Containing Materials or Other Regulated Materials sampling or analysis reports generated by the Installation/Garrison, the FRP Contractor or any sub-contractor after initial award of the project.

6.1.2 General Information deemed appropriate by the contractor, to include a short description of what was actually accomplished as part of the completed field activities for this project. (Note: this would be short description like total number of structures and a gross square footage of abatement, and gross square footage of demolition.)

6.1.3 Site(s) Location and description(s) (Installation level map)

6.1.4 Site Maps with each removed structure shown by Bldg Number

6.2 Chapter 2 – Summary of Details

6.2.1 Date Field Work Started; Date abatement started and ended for each structure; Date demolition started and ended for each structure.

6.2.2 Date Field Work Completed at each structure, to include all Site Restoration requirements for each structure.

6.2.3 Table of the structures that were physically removed during the field activities with the size of each structure.

NOTE: 6.2.1, 5.2.2, and 6.2.3 can be provided as a comprehensive table.

6.2.4 Project Statistics

6.2.4.1 Original and Final Awarded Costs of the Project, including all modification and amendments.

6.2.4.2 Total hours worked on this project.

6.2.4.3 Total hours worked involving abatement activities.

6.2.4.4 Total hours worked involving demolition activities.

NOTE: 6.2.4.3 and 6.2.4.4 may not add up to 6.2.4.2, because 6.2.4.6 includes all PM, PgM, Inspector, Safety, etc. man- hours which are typically attributed to overhead functions.

6.2.5 Materials Final Disposition

6.2.5.1 Any materials re-used by the installation (post mobilization) – quantities and types.

6.2.5.2 Any materials re-used off-site (what and whom) – quantities and types

6.2.5.3 Any materials that were Recycled: (a) on the installation, or (b) off- site –quantities and types

6.2.5.4 Buried – This would include a description of the structural footings, foundations, or basements that were left in-place as required by the PWS, and then subsequently re-buried as part of the site restoration requirements. These descriptions shall be building specific locations.

6.2.5.5 Disposal of materials both on the installation (on-post landfill) or off-site.

NOTE: This may be in the form of a table, but shall contain the below listed minimal quantities of information.

• Type of Debris materials recycled, salvaged, reused, and disposed. The table or chart shall convey the original material quantity estimated (by major category), quantity recycled, percentage recycled.

• Table 1 shown toward the end of this document is an example for this reporting (from Technical Exhibit 9 of the MATOC).

6.2.5.6 Provide a quantifiable description of cost savings for recycling and re-use versus commercial landfill disposal alternative(s). This paragraph shall also convey actual re-sale value of sold materials or derived savings through the re-use of materials, as opposed to landfill and disposal costs. The original proposal contained a value of divertible/salvageable materials

(proposal cost reduction). Did the Contractor fall short, meet, or exceed that value and what factors effected that value in this project? NOTE: This does not change any part of the proposal, award, or requirements of the project. This is strictly provided as final documentation and lessons learned on this particular project, at this particular site.

6.3 Chapter 3 - Lessons Learned

This chapter shall describe in detail any lessons learned during the project as it relates to demolition contracting, quality control, quality assurance, specific demolition activities in the field. The lessons learned shall also include a possible remedy or solution to the lesson(s). One of the Corps motto is “Continuous Improvement”. Lessons Learned are an integral part of the continuous improvement. That applies both internally and externally through our Contractors.

6.4 Appendices. The final report shall contain the following documents in the order shown as appendices:

6.4.1 Appendix A - Performance Work Statement and Modifications as Awarded (no cost information)

6.4.2 Appendix B – Daily Site Supervisor and/or Daily/Weekly/Monthly Quality Control Reports, as required by the project specific PWS.

6.4.3 Appendix C – Weight Tickets or Truck Scale Receipts supporting the figure conveyed in Chapter 2 and associated Table(s)

6.4.4 Appendix D – Copies of all sampling and test reports for asbestos and other ORM material generated by the Installation/Garrison and/or the Contractor after initial award of the project.

6.4.5 Appendix E – A copy for record of all Permits, Titles, Sanctions, Awards, Waivers, or Fines which the Contractor received as part of the abatement, demolition, and disposal activities for this project. If there were fines or sanction, the documentation shall show proof that the fine or sanction was resolved by the Contractor prior to completion of the project for final payment.

6.4.6 Appendix F – Schedule. This is the final schedule with the actual start dates and finish dates for each activity in the schedule. The Schedule should be broken out by CLIN and abatement and demolition by bldg. Site restoration does not need to be broken out by bldg. It may be scheduled by a series of buildings or blocks of buildings based on the Contractor’s actual schedule of work
