About this file
This document is an Excavation Permit form for U.S. Army Garrison, Fort Belvoir, detailing requirements for underground work. The permit involves adding conduit from manholes to generators underground, with specific procedural requirements including submitting a to-scale drawing of the excavation site, staking/flagging the excavation route every 30 feet, and obtaining approval prior to commencing work.
Key procedural details include a 10-14 working day processing time, mandatory hand digging within a 3-foot radius near utility lines, and multiple required notifications/approvals from various Fort Belvoir departments such as Business Management, Fire Prevention, Environmental, Master Planning, Provost Marshal, and Miss Utility. The permit emphasizes contractor liability for damages, requires on-site permit availability, and mandates compliance with OSHA safety laws. Contractors must call Miss Utility at least 48 hours in advance of excavation and update utility location tickets every 14 days.
