APPENDIX C
Attendance All employees are expected to arrive on time, ready to work, every day they are scheduled to work.
If an employee is absent or late for any unexpected reason, The employee must notify your supervisor and receive confirmation of receipt of the notification from your supervisor 30 minutes prior to when you are scheduled to work. If you do not do this, your time will be considered Unexcused.
The employee must also complete the Employee Leave Request Form no later than the end of the day you return to work. If the employee does not do this, the time will be considered Unexcused.
If the employee fails to report for work or to report on time for any reason or fails to call the supervisor 30 minutes prior to the shift with the reason, the Company will treat this time as Unexcused Leave. In order for the timesheet to be approved, the employee will need to mark the time as UNX (Unexcused).
Unexcused Leave is cause for disciplinary action.
As outlined below, the Company uses a points system for managing tardiness and absenteeism. Points accumulate and remain on an employee’s record for 12 months from the date of occurrence, and roll off at the first of the month following the expiration of the 12‐month period.
Points Attendance Violation
One (1) Point Arriving late to work, but within 5 minutes of your scheduled start time.
Leaving work early, but within 5 minutes of your scheduled end time.
Two (2) Points
Arriving late to work (more than 5 minutes after your scheduled start time).
Leaving work early (more than 5 minutes before your scheduled end time).
Three (3) Points
Unexcused absence with proper call‐in (within 30 minutes of start time).
Being unreachable or non‐responsive for more than 30 minutes during your scheduled working time.
Not returning from a scheduled rest period or lunch break.
Ten (10) Points Unexcused absence without proper call‐in, including no‐call/no‐show.
The following table shows disciplinary action the Company will take based on the number of points accumulated over a 12‐month period.

Twelve (12) Points A written notice that the employee has accumulated 12 or more points.
Sixteen (16) Points First written warning to the employee.
Twenty (20) Points Second written warning to the employee.
Twenty‐four (24) Points
The employee will be discharged.
In addition to accrued leave, reasons for an excused absence include:
Medical certification that the employee is unable to work.
Jury duty, military leave, medical leave, FMLA leave, subpoena, or any other leave of absence expressly authorized by the Company, the terms of company policy, or the law.
At the Site Manager’s discretion, late arrivals when employees provide evidence to substantiate a claim that they were legitimately prevented from being on time despite their best efforts.
The employee must still contact their supervisor to make notification as soon as the employee knows they will be late and complete the Leave Request Form.
Making up Lost Time Excused late arrival is not time worked. Time must be made up or marked as Leave unless otherwise authorized under the contract.
In cases of Excused late arrivals, employees may be able to make up the time if doing so is operationally feasible and will not incur additional cost to the project (for example, working during the scheduled lunch break). This may allow the employee to avoid taking Excused Leave. This must be approved in advance in writing by the supervisor or Site Manager. If the employee does not have advance approval to work during a break period, work performed during a break period will be paid but will not make up for a late arrival or absence (meaning the employee may still be required to take Leave to cover the late arrival or absence).
Excused Leave Employees must complete their timesheet with full workweek hours (40) specifying the correct type of Leave for any hours not worked. For any Leave to be Excused, the employee must submit the Employee Leave Request Form and receive approval from their supervisor.
When Leave is Excused, it must be compensated in payroll using Vacation, Sick or other available accrued Leave time. Mark the timesheet to indicate the type of Leave to apply. For any hours where there is no Leave balance to apply, even if Leave is otherwise Excused, Leave will be unpaid. In order to have the employee timesheet approved by their supervisor, mark LWOP (Leave Without Pay) unless the employee receives other instructions.
If it is deemed in the best interest of the mission, the Site Manager may designate an allowable amount of Leave Without Pay (LWOP), that is, a number of hours that employees may have as Excused but unpaid time without incurring disciplinary action. Employees will be provided a timesheet code to use for this time. This local policy will be published on site and may be discontinued at any time.
Unexcused Leave If an employee is not at work as scheduled and has not submitted the Employee Leave Request Form OR, if the employee submitted the form and has not received approval from their supervisor, the Leave is Unexcused. In order to have the supervisor approve the timesheet, the employee will need to indicate UNX for any Unexcused Leave hours.
Taking Unexcused Leave is cause for disciplinary action.
