About this file

This is a Bid Document Addendum for the County of Santa Clara, California, Health System Behavioral Health Services Department for the Independent Living Home Habitability Improvements Project (Bid Number: RFP-BHS-FY26-0513). The project seeks to award forgivable loans to qualified operators of unlicensed independent living homes serving individuals with Serious Mental Illness. Habitability improvements may include bathroom updates for ADA compliance, minor renovations such as painting and flooring, appliance repairs or replacements including water heaters and air conditioning equipment, fire system upgrades, bathroom and kitchen remodels, furniture and mattress replacement, and other improvements to expand bed capacity. The bid closing date is June 30, 2027, at 3:00 PM Pacific Time. The anticipated contract term commences upon execution of the Agreement and remains in effect through June 30, 2027, to allow for completion of County-approved habitability improvements, required inspections, and verification of compliance for loan forgiveness.

This RFP may result in multiple awards and represents a one-time funding opportunity. The County requires the operator's and home owner's willingness to grant the County first right of refusal for future vacancies, provide assurance that improvements will prevent closure and/or increase bed capacity, and certify that the home is operating legally without providing the level of care and supervision required for licensure. The solicitation includes 20 referenced documents, including a Scope of Work, Insurance Requirements, Facility Use Agreements for both owners and lessees, a Sample Service Agreement, Contracting Principles, an Inspection Checklist, Reporting and Data Collection Forms, and various appendices containing cover sheets, application checklists, evaluation criteria, proposer questionnaires, and California Public Records Act confidentiality information forms.
