About this file

This is an Application Checklist and Evaluation Criteria document for the Santa Clara County Health System Behavioral Health Services Department's Independent Living Home Habitability Improvements Project (RFP-BHS-FY26-0513). The County seeks to award forgivable loans to qualified operators of unlicensed independent living homes serving individuals with serious mental illness. Eligible improvements include ADA bathroom upgrades, renovations such as painting and flooring, appliance repairs or replacements, fire system upgrades, bathroom and kitchen remodels, furniture and mattress replacement, and bed capacity expansion. The anticipated contract term commences upon agreement execution and extends through June 30, 2027, to allow for completion of County-approved improvements, required inspections, and compliance verification for loan forgiveness. This RFP may result in multiple awards and represents a one-time funding opportunity.

The evaluation criteria consist of five sections totaling 100 points: Eligible Uses of Funds (20 points), Loan Request and Financial Readiness (15 points), Project Feasibility (20 points), Compliance and Certifications (20 points), and Benefit to BHSD Clients (25 points). Applicants must demonstrate that requested improvements fall under eligible categories with clear and reasonable cost estimates, submit loan request worksheets and budget summaries, provide proposed project timelines and implementation plans, commit to compliance with HUD, ADA, MHSA, and local codes standards, and agree to regular inspections and deed restrictions for the loan term. Additionally, applicants must show how repairs prevent displacement or expand housing, commit to becoming licensed facilities under the California Department of Social Services Community Care Licensing Division if applicable, consent to master leasing arrangements with County-contracted providers, and grant the County first right of referral for all vacancies during the loan term.
