About this file
This is a one-year program report data collection form for the Community Services and Support (CSS) General System Development Program administered by the Santa Clara County Behavioral Health Services Department for fiscal year 2024 (July 1, 2023 through June 30, 2024). The form requires comprehensive reporting on program description, goals, objectives, outcomes, clients served, annual cost per client, evaluation activities, and demographic data collection across multiple categories including age, race, ethnicity, gender identity, sexual orientation, primary language, military status, and disability status. The document mandates detailed outcome measurement using evidence-based practice standards, promising practice standards, or community and practice-based evidence standards, with supporting documentation such as survey instruments and peer-reviewed journals. Additionally, the form requires collection of data on residential status, educational status, employment status, financial support sources, health status, substance abuse issues, daily living functions, and emergency interventions.
The reporting requirements are governed by Mental Health Services Act regulations outlined in 9 CCR 3650 and 3630, which mandate evaluation of racial and ethnic gender disparities in service access, quality of care, and disproportionate representation among Native Americans and various vulnerable populations. The form requires documentation of referrals to treatment and community-based providers, including average duration of untreated mental illness and average interval between referral and treatment engagement for county mental health services. Program operators must report on group services delivered with unduplicated participant counts and attendance data, and provide detailed outcome information including indicators, percent improvement, and survey tool references. The CSS Manager Rebeca Lopez and Analytics & Reporting team member Fannie Stein are identified as key contacts for verification of expenditures and demographic data respectively, with instructions to note "Not available" for any indicators not collected during the fiscal year with explanatory narratives required.
