About this file

This is a Scope of Work document for the County of Santa Clara Behavioral Health Services Department (BHSD) outlining the Independent Living Home (ILH) Habitability Improvements Project. The project seeks applications from qualified property owners and operators of unlicensed independent living homes serving individuals with Serious Mental Illness experiencing homelessness. The project provides forgivable loans to support minor construction and rehabilitation improvements that meet United States Department of Housing and Urban Development Housing Quality Standards, including repairs to plumbing, electrical, roofing, HVAC systems, fire and life-safety features, bathroom and kitchen rehabilitation, painting, flooring, appliance replacement, and other essential upgrades. Single applications may request funding ranging from $10,000 to $500,000 per property. The anticipated contract term commences upon execution of the Agreement and extends through June 30, 2027, allowing for completion of improvements, required inspections, and verification of compliance for loan forgiveness. The County may issue funding on a rolling basis until all funds are expended.

Forgivable loans are structured with a five-year loan term carrying a 3% simple interest rate, deferred for the duration of the loan. If operators remain compliant with all loan conditions throughout the entire term, the principal and interest shall be forgiven. As a condition of funding, a deed restriction shall be recorded on the property title for the five-year loan term, requiring the property to meet County-approved habitability standards and serve eligible BHSD clients. Applicants must grant the County first right of refusal for future vacant units during the loan term and assure that improvements prevent closure or loss of capacity. Funding awards follow a priority order based on current client residency with County contracts or subsidies, affiliation with other County programs, Supplemental Security Income receipt, Medi-Cal benefits, and income levels. Awarded applicants must comply with HUD Housing Quality Standards inspections, maintain operations manuals, submit deed restrictions, provide the County first right of client referral, and participate in ongoing quarterly data reporting using County-provided templates and monthly compliance monitoring.
