STATEMENT OF WORK

Repair Floor Bldg. 265 West Bay
Contractor shall provide all labor, materials, equipment, supervision, quality control, transportation and services required to accomplish all work specified in accordance with requirements contained in the original contract specifications. Project area must have asbestos and LBP survey posted on site prior to beginning any construction activity and maintained on site until the project is completed. No work will be allowed without dust barrier containment. Extra patching will be required, especially near joints. Work requires multiple phases to accomplish and vacuum attachments on all grinding equipment. Work shall be accomplished in Bldg. 265 West. All products used shall be according to current approved line item descriptions in the existing schedule
B, which shall not be modified for this project.
1. Demolition:
a. Considering an injury due to faulty contractor equipment during a previous project, greater heed to the Government Safety Requirements and basic contract SOW documents shall be made. All precautions to prevent injury shall be taken as agreed upon at contract award of the basic IDIQ contract.
b. Work around all obstacles to remain. Protect all drains by any means possible.
Remove all coating in all areas down to concrete in the project area. The contractor shall remove all cove base, bolts, and tape, including those that may be covered by existing coating in the project area. Note: The contractor shall field verify quantities prior to task order proposal.
c. Contractor shall protect and use proper care not to injure people or damage the existing structure, equipment, landscaping, surroundings, and personal items.
Any items damaged will be the contractor’s responsibility to repair or replace.
d. The contractor shall dispose of all trash, empty containers, and removed materials, etc., used or generated in the performance of their tasks. No waste materials shall be placed in any trash receptacle on Hill AFB. The Contractor must comply with all federal, state, and local regulations regarding the transport and disposal of any/all wastes generated as a result of floor coating activities under this contract.
2. Construction:
a. Contractor shall set up 35’ tall, 21’ tall, and 12’ tall dust barriers across the width and length of the bay, all openings, and over all equipment of Bldg. 265 West while also allowing extra dust barriers for pedestrian walkways to restrooms and exit doors. Dust barriers shall completely cover the remaining bay, doors, all remaining objects, and ducts and vents in the project area. Contractor shall perform work after LBP project manager completes LBP abatement as required.
Dust barrier shall be attached to the ceiling with tape and hang like a shower curtain, either taped or weighted down at the bottom or wrapped around ducts and over vents. No work will be allowed without dust barrier containment.
b. Contractor shall hire a certified master electrician as stated in the Basic Contract to adjust electrical as necessary to perform the required task or provide generator.
c. Contractor shall have vacuum attachments on all grinding equipment and prevent spillage of dust and other fragments from the vacuum into the air and surrounding environments.
d. Contractor shall mask with plastic sheeting around perimeter of project area, drains, doors, and remaining objects.
e. Contractor shall sawcut, clean, and seal all joints and cracks, including those that may be covered up by existing coating and materials.
f. Contractor shall key into all drains and edge joints. Plug, cover, and protect by any means possible all drains.
g. Contractor shall epoxy and concrete patch all areas of damaged concrete full depth. Floor shall be smooth with no bumps or lips.
h. Contractor shall prepare the floor by diamond grinding and shot blasting.
Contractor shall also clean the entire floor by either power washing, with acceptable detergent and disposal method, or vacuum the entire floor in the project area. Contractor shall not put any wastewater or dust down any drains.
Diamond grinding shall remove all material down to concrete and provide a smooth concrete floor with no bumps or lips. Work around obstacles to remain.
i. Contractor shall install best option primer and 3/16” epoxy floor coating system sufficient to handle high pedestrian traffic and heavy loads. Floor requires aluminum oxide or best option non-skid abrasive enough to provide traction/grip but not tear up mops. Entire project area shall be smooth with no bumps or lips and the top coat color as selected by customer during the site visit.
j. Contractor shall replace all cove base removed during demolition. Color to be selected by customer during site visit.
k. Contractor shall stripe lines, single safety paint, and double safety paint for all lane demarcation, fire extinguishers, eye wash stations, and electrical cabinets per attached drawing and confirmed as requested by customer during site visit.
3. Execution:
a. Contractor must coordinate with customer, Derek Driggs (801) 586-8433, for access to the building.
b. Contractor will be required to complete work according to schedule arranged by customer and Contracting Officer not to exceed 90 calendar days from receipt of notice to proceed, unless directed otherwise by the Contracting Officer. Time adjustments will also be made due to weather delays in shipping of material. The government will give proper consideration and adjust the area to be painted based on projected user requirements and surface condition.
c. Contractor shall perform work as allowed by customer.
d. Contractor shall coordinate all work prior to starting the contract and during the performance with the project managers.
e. Contractor shall ensure that the job site is clean before leaving at the end of each workday.
f. The contractor will coordinate with the contract officer and the project managers to obtain base access badges for contractor personnel. The contractor is responsible to immediately return all access badges for personnel who no longer work for the contractor. The contractor is responsible to immediately return all access badges after completion of the contract.
g. Quality Assurance: The government will evaluate the contractor’s performance for each job.
h. Government Remedies: The contract officer shall follow the requirements of
FAR 52.246-21 Warranty of Construction for contractor’s failure to correct non-conforming construction.
