Prescott Install Elevators and Space Build Out in Buildings 162 and 164 Project 649-22-136
This project is to install the elevators that were descoped from the construction of Buildings 162 and 164, and address the operational issues that have become apparent after the building have been in operation for several years, including delivery area shortcomings, patient counseling facilities, Pharmacy Cache location problems, breakroom renovation, and additional offices in Building 164, and patient flow issues in Building 162.
Security
The C&A requirements do not apply, and a Security Accreditation Package is not required.
NAVAHCS Historic District Requirements
Building 162 and 164 are not listed on the Register of Historic Places, however they are both on the Fort Whipple Historic District. Although no changes inside the buildings need to be approved through the Arizona SHPO, all excavation work needs to be approved. All excavation work requires archeological monitoring, regardless of the depth, unless the area has been previously disturbed, which should apply to all excavation work in this project.
However, this must be reviewed by SHPO prior to the start of the excavation. If monitoring is required the certification requirements for the monitor can be found at The Secretary of the Interior Professional Qualifications Standards | U.S. Department of the Interior (doi.gov)
Smoking
Smoking is prohibited on site for contract employees.
Parking
Parking is available for contractor in Lot “S”, which is North of the construction area.
TB
TB testing will be required for contractors working on the site.
Badging
All contract employees will be required to have a security badge issued by the NAVAHCS VAPD prior to start of any work. To be issued a badge the contractor must contact the COR at least 72 hours prior to the employee arriving on site so the COR can officially request that a badge be issued to the employee. Once this has been done the contract employee must go to be Building 28 and show the VAPD an official form of ID (typically a driver’s license) and they will be issued a badge. The contractor must provide the full legal name of the employee, along with how many weeks or months that are expected to be on site and (if they are a subcontractor) what company they work for. If the employee fails to provide some official form of identification they will not be give a badge and will be sent off the site.
Construction Site Security
A 7’ (or higher) construction fence is required to be around any construction site that can be accessed by non-contractor personnel. The specific requirements for the fence can be found in Section 1.6.I of specification section 01 00 00. All interior work requires that the work areas be either inside a negative pressure enclosure or clearly marked as off limits to non-construction personnel.
Photographs
Photographs of the construction site are acceptable provided that no patients are included in the photograph. A professional photographer is not required for the project, just clear photographs with attached information describing the location of the photograph, and if necessary, the reason for the photograph.
Laydown area
The laydown area will be on the west side of Building 107, near the entrance to the “B” courtyard.
Road closures
A road closure leading to the ED is expected as part of the project. Prior to closing the road, the process of getting an ambulance to the ED ambulance entrance will need to be worked out with the COR. Because of the significance of closing an ambulance access way, a 2 week notice for the closure will be needed, and a process for allowing ambulances through after hours will need to be developed and approved. A plan for the closure, including the plan for signage and flaggers will be required.
Utility shutdowns
Utility shutdowns that will affect other buildings will need to be planned with the COR and have at least 7 days of notification prior to the shutdown.
Phasing
The Pharmacy Cache will need to be completed prior to any other work being done. This is necessary for a different project that will displace the existing Pharmacy Cache, so the pharmacy cache is the highest priority for completion. There are no additional phasing requirements.
ILSM
If the man door on the lower lever of B164 is blocked for reason for more than 4 hours at a time, an ILSM plan will be needed. Since there is some re-sloping work at the exterior apron adjacent to Room 15, the construction fence at the area should be placed as to not block the exterior door on the ground floor to eliminate the need for an ILSM for the project.
ICRA
The work in both B162 and B164 will require all IC measures listed in Class III precautions.
