About this file

This document is a Pre-Construction Checklist for a VA hospital elevator renovation project at the Hines VA Hospital in Hines, Illinois. The checklist outlines critical administrative and procedural requirements for the contractor, including submission of cost schedules, progress charts, and daily logs within 10 calendar days of receiving the Notice to Proceed. Key requirements include posting wage rates at the work site, submitting a list of subcontractors, providing daily logs detailing workforce composition, and following specific protocols for correspondence, changes, and payment requests.

The document emphasizes strict compliance with VA procedures, such as only the Contracting Officer having authority to make contract changes, mandatory submission of detailed change order proposals over $1,000, and requirements for apprenticeship certificates. The VA Project Manager and Safety Officer have specific oversight roles, with the Safety Officer empowered to halt work for serious safety violations. Contractors must also adhere to specific guidelines for subcontractor approval, labor standards, and correspondence, ensuring transparent and structured project management throughout the elevator renovation work.
