SAVAHCS TUCSON, AZ

Project 678-333 Expand Building 80 for Outpatient Diagnostics

15 May 2024

SUPPLEMENT B

A/E SERVICES - STATEMENT OF WORK

I. GENERAL INFORMATION/REQUIREMENTS

A. Background:

This SAVAHCS project is to construct additional square footage to expand and relocate outpatient diagnostics services in B80.

B. Project Scope/Objectives:

1. General: This project will construct an approximately 10,000 square foot, first floor addition to the SE corner of Building 80 to allow for expansion for diagnostic services. The expansion will address ancillary/diagnostic deficiencies by providing more space for phlebotomy, lab and imaging.

The first goal of the project is to add an MRI with the capability of interventional procedures and a new CT. Special consideration will need to be made for the electrical, structural, and architectural requirements to prepare the room for an MRI machine. Additionally, support rooms will need to be included such as changing rooms. The MRI may also be required to have a standalone chiller.

The second goal is moving existing radiology equipment such as the X-ray machines and ultrasound rooms, the phlebotomy lab, and the EKG rooms from B80 to the new addition. This will localize diagnostic services and also allow the previous space to be used for both veteran welcoming services and to expand the pharmacy (the backfill of these latter two spaces is not part of this scope). The A/E will be responsible for determining the plausibility of moving all these diagnostic services to the new space. If it all can’t fit, radiology will be prioritized, followed by the phlebotomy lab. The lab will need pneumatic tube support as well.

The occupancy type for this addition will be ambulatory health care. B80 is a mission critical building.

2. Phasing: The project’s successful completion is dependent upon a coordinated phasing which permits the construction to proceed in an efficient and orderly manner while allowing hospital staff to perform their duties with minimal disruption. Care must be taken during construction to maintain acceptable emergency egress as well as access to B80.

Ultimately, the precise sequence will be determined after a building contractor has been selected and consultations among the hospital staff, facilities personnel, contractor, and architect can be

SOW - 2

arranged. Staging for construction materials, contractor and material access and maintenance of the clean environment must all be considered.

New work shall include architectural, civil, structural, landscaping, plumbing, mechanical, electrical and all such systems and components as may be necessary to construct this addition. The remaining items listed below describe in greater detail certain unique items of scope and special utility system conditions and requirements.

3. Architectural: The addition shall take into account the continuing operation of the existing outpatient clinics within B80. Special notice should be made for the structure housing the MRI and other imaging equipment. One colorboard shall be provided at the 65% design, one at the 95% design and three at the bid set.

4. Building Utility Systems: In general, the existing utility systems will be modified for this building addition. In addition, there are several special utility system conditions and requirements which apply to this project which result in the need for more or less work depending upon the system. The following is a brief description of special utility conditions and requirements for this project:

a. Plumbing Water/Waste/Vent: All plumbing distribution piping shall be new construction.

b. Medical air, vacuum, oxygen and waste anesthesia gas piping shall be provided at selected rooms within the addition.

c. HVAC: It is envisioned to provide a new HVAC system. A new air handler with a ducted supply, return and exhaust system is required. The HVAC system shall have Direct Digital Controls (DDC) and be compatible with and be able to communicate with the hospital’s Johnson Controls Energy Management Control System (EMCS) in B80 including adding new points and graphics. Humidification shall also be provided.

d. Fire Protection Systems: Fire sprinkler and alarm systems presently exist in Bldg.80.

Design work will be required for fire protection and alarm systems as they relate to this addition. It is expected that these systems will require additions and modification and not total replacement. The Tucson VA fire alarm system is a Notifier analog addressable fire alarm reporting system with a voice evacuation alarm notification system throughout the patient areas of the hospital. The system consists of one 10 loop addressable Notifier 3030 panel in Building 57 that covers the main hospital area, one 4 loop addressable Notifier 3030 panel which covers Building 60, one 4 loop addressable Notifier 3030 panel which covers Building 80, one 3 loop addressable Notifier 3030 panel which covers Building 30, and three single loop Notifier 3030 panels which cover outlying administrative/support buildings. The system is controlled by a NOTI-FIRE-NET, network reporting system that reports the status of all devices via a fiber optics cable network and provides annunciation and control of alarm and trouble signals throughout the facility. In addition, a Network Control System (NCS) located in the Utility System Repair Operation area of Building 2 provides a graphic indication and annunciation of all troubles and alarms throughout the facility. The facility alarm system is divided into 41 individual reporting zones that are monitored 24/7 by an off premises Commercial Monitoring Company. The new fire alarm points shall tie into the existing Notifier system and graphics upgraded. All new strobes

SOW - 3

shall be synchronized with the existing B80 strobes. The contractor shall hire a certified Notifier contractor to tie into as well as modify the existing fire alarm system.

e. Electrical: All branch circuit panels, conduit, wire, wiring devices, and fixtures are to be new. The types and amount of power required for the addition are to be evaluated and new electrical outlets, lighting and panels shall be provided. The existing emergency generator shall be evaluated as well. Sinks shall be battery operated with manual override. Lighting shall include energy efficient LED fixtures and LED exit lights. Lightning protection shall be provided throughout the roof to all rooftop equipment. Controlled outlets are also required.

f. Commissioning using a third-party commissioning agent, hired by the A/E, will be required as part of this project.

5. Special Systems & Equipment: The addition will include all special systems and equipment typical of a state of the art medical clinics such as monitoring, communications, etc. Although some systems or portions of systems will be provided by and even installed by the VA outside of the general construction contract, the A/E will be required to provide a complete design based on consideration of all required systems, equipment and furniture. Plans will be required which show all such items and contractual requirements related thereto (i.e. furnished & installed by contractor or VA). An open ladder cable tray shall be installed. The specifics for each system are as follows:

Hill-Rom Navicare Nurse Call System. Code blue and code green capability. It is envisioned to expand the existing system on the first floor to serve the new addition. Hill-Rom shall be hired to design this system.

Public Address System. The A-E will expand the existing Public Address system on the first floor.

P-Tube. The P-Tube system shall be expanded over to the new addition. SwissLog shall be hired to design this system.

Omnicells. Provide data and power drops for future Omnicells.

The A/E and contractor shall take all efforts to protect for reuse all nurse call and Public Address system homerun data cabling pathways.

Access Control. Access control (card readers) shall be provided.

Security. Security cameras shall be provided along with cabling and conduit. Setback for parking shall also be addressed.

Telephone. Provide a complete sytem throughout the addition to include cabling and conduit, etc.

Cable trays shall be included in the design.

C. A/E Design Scope, General

SOW - 4

The A/E shall utilize VA Form 10-6298 in preparing their proposal. The A/E shall utilize the form several times over in order to show the costs associated with each design submission (15%, 35%, etc.). The A/E shall develop and provide a complete set of drawings and specifications for bidding and construction contract purposes which meet the previously stated Project Scope/Objectives. The A/E shall also provide limited construction period services as described in this and other sections of the A/E contract. The A/E shall continue the design process by meeting with the VA design team. The purpose of these meetings is to obtain information for the design development submission. The remaining portions of this document outline in more detail the specific requirements of the A/E scope of work for this project.

D. Design Submissions The following design submissions will be required:

Preliminary Design submission – 15% Schematic Design submission – 35% Design Development submission – 65% Construction Documents submission – 95% Backcheck Design submission – 100% Final Design submission

Refer to section three, “Design Submissions - Minimum Requirements” and the Program Guide, PG-18-15, Volume C A/E Submission Instructions for Minor and NRM Construction Program located on the VA Technical Information Library (TIL) (SOW Attachment 1) AE Submission Instructions for more specific information on the requirements for each of these submissions.

Design submissions are to be delivered to the Contracting Officer no later than the dates listed under Section III, Paragraph H of this contract.

E. Design Submission Reviews:

Each submission (excluding the Final Design submission) will be followed by a VA review period of 21 calendar days. After the review, a submission review meeting at the VAMC will be held with the A/E and VA design team to discuss the results of the review, to resolve issues and to review the requirements and goals of the following submission.

Design submissions will be reviewed to verify adherence to all contract requirements. Design review comments shall not relieve the contractor from compliance with the terms and conditions of this contract. Design submissions will be required to be re-submitted in full if the basic requirements of the design submission are not met or if the submitted material contains numerous and or serious errors and omissions. Time required to re-submit such submissions shall not be grounds for extending other submission deadlines. The A/E shall resolve and answer all VA design review comments and questions in writing no later than 14 calendar days after the submission review meeting, indicating the response to, or action being taken as a result of the review comment and shall incorporate any such actions in the following design submission.

F. Reproduction of Submission material and Bid Sets The A/E shall reproduce all required design submission materials. These are considered to be a part of the cost of design. The A/E will be reimbursed for these costs. At the completion of design, the A/E shall be required to print up to five (5) construction document bid sets on a

SOW - 5

reimbursable basis (to be supported by invoice). Note that all bid administration activities will be performed by the VA Contracting office.

G. Other Design Meetings and Design Review Trips In addition to the scheduled formal design submission review meetings, it is expected that the A/E will need to hold other design meetings with the VA Design Team and make other investigative site visits to coordinate and resolve various design issues and to gather information needed for design. It shall be the responsibility of the A/E to request and conduct such meetings and investigative site visits as necessary to meet the requirements of this contract and to provide a quality design and final set of construction documents. For design site investigations, call ahead to the Contranting Officer’s Representative (COR) to obtain access to occupied, locked or concealed spaces. Allow adequate lead-time, where appropriate, to coordinate access to normally occupied functional spaces.

H. A/E Personnel Changes The A/E shall exclusively utilize the key personnel named in the proposal to perform the identified functions and services. In the event that any personnel named in the proposal are unable to perform their duties, the A/E shall promptly submit to the Contracting Officer in writing, the name and qualifications of a proposed replacement with equal or superior qualifications. No substitution shall be made without prior approval of the Contracting Officer.

I. Use of VA As-Built Record Drawings/Site Investigation The COR will assist the A/E in locating and assembling all as-built record drawing information that may be helpful to the A/E in the design of this project. There is not one single set of as-built drawings which clearly depicts the current conditions of Bldg. 80 as a whole. Although the record drawings should be accurate and be helpful in design, the record drawings cannot be assumed to be 100% accurate and the design must not be based on this information alone. It is expected that all existing conditions information which is important to this design will be verified in the field through site investigation work as needed throughout design.

J. Construction Budget and Bid Alternates

The A/E shall design this project such that the cost of construction will not exceed the construction budget amount of $13,545,000 and will be responsible to evaluate and determine to what degree this budget can accomplish the scope and priorities stated for this project. This is the amount of funds that will be available to make an award to a general contractor for construction in the spring of 2027. The construction contract will most likely be awarded to a Service Disabled Veteran-Owned Small Business (SDVOSB) through a best value procurement process.

A/E cost estimates for the project shall be representative of the cost to make an award to a general contractor for construction and should therefore include such items as overhead and profit of general and sub-contractors. The A/E shall be required to design the project with at least ten percent of the estimated cost/scope of the project identified as deductive bid alternates to provide some flexibility at the time of the construction procurement.

SOW - 6

II. SPECIAL & TECHNICAL REQUIREMENTS

A. Design Criteria The A/E shall utilize the VA Planning, Design, and Construction Criteria listed below to the extent applicable for the scope of this project. Any deviation from this criteria shall be noted with an appropriate explanation at the time of design review submittals. With the exception of PG-18-4, Standard Details, all criteria listed below are available to view and download at the VA’s Technical Information Library (TIL) on the internet at http://www.va.gov/facmgt/standard/. The COR will provide assistance in obtaining access to and making use of this information as needed. The A/E should also review the A/E Quality Alerts and Design Alerts listed at this web site as an added measure of quality control during design.

Hard copies of the VA Standard Details may be obtained from VA Standards Service. The A/E is responsible to review and correctly utilize any codes or standards incorporated in these documents by reference. Standard Codes such as the Uniform Building and Plumbing codes, and NFPA codes shall be utilized as applicable for the design of this project. B. Drawings (CAD)

All drawings shall be accomplished using an automated graphics system using AutoCAD 2022 or a newer version and shall be plotted on 42” X 30” size drawing sheets. Utilize VA layering standards. Assure that all information shown on drawings is adequately cross-referenced, clear as to intent, and indicative of owner responsibilities as opposed to Contractor responsibilities. Do not designate references to certain trades typical of subcontractors (refer to "The Contractor" or "The SAVAHCS"). Edit legends to specifically include all required items and to not include extraneous information. Assure that legends and abbreviation charts specifically state range of applicability. Provide individual legends for individual sheets where it is reasonable to do so.

VA Standard Details shall be used as a reference in preparing drawing details. Any deviations from VA Standard Details shall be noted to the COR. The A/E shall be responsible to edit all details, whether VA or A/E standard details, to the specific conditions and requirements of this project. Many notes on VA Standard Details are instructional notes to the designer concerning the use of and editing of the detail and should not appear as detail notes on the construction documents.

C. Specifications The project specifications shall be prepared by the A/E from the VA Master Specifications. The A/E shall identify all VA master specification sections applicable to the project, and then edit and tailor the specs to fit the scope of work. If additional specification sections are needed that are not listed in the VA Master Specs, the A/E is responsible to write and edit an appropriate CSI format specification section. Assure that items not applicable are not included. Write additional text where needed. When contractor submittals are required in portions of a specification other than the "Submittals" paragraph, place a cross-reference in the "Submittals" paragraph. Remove extraneous information from throughout specification sections, especially in the "Submittals" paragraph. Do not over specify. Assure that product specifications are not proprietary. Include a “basis of design” for specified equipment and materials.

D. Construction Cost Estimates

Estimates shall be completed using a consolidated database; meaning, estimates that are completed within separate disciplines shall utilize the same format and a single summary shall be provided. Magnitude estimates are not acceptable. Line items shall show quantities and unit prices and break out labor and material separately. Special line items for equipment and subcontracts shall be included to reflect accurate overall estimated bid pricing. Labor rates shall reflect minimum wages required to be paid under the Davis-Bacon Act for the local geographic area. Current Davis-Bacon minimum wage rates are available from the Contracting Officer. The A/E shall use the 34 division system for estimating to the extent applicable to the project. Any bidding or "design update" contingency shall be identified separately in the line items under mark-up. Construction contingency (for changes during construction) should not be included in

SOW - 8

the estimate. The bottom line figures shall be escalated as necessary, by the A/E, to reflect expected conditions/costs at the mid-point of construction (estimated to be fall of 2028).

E. Submittal Log The A/E shall develop a list of all submittals required by the specifications during construction.

The list shall be grouped by specification section and identify each individual item or groups of items within each specification section. Provide paragraph references for each item. Provide footnotes that identify any chronology or special conditions related to the stated item. Any submittals that are required in more than one section, but only constitute one item, shall be listed with cross-reference. Use the development of the submittal log as a method of quality control for the specifications to assure that submittal requirements are logical and that the products specified do not conflict with information presented on the drawings.

F. Narratives Written narratives shall be used as part of each design submission in addition to drawings, specifications, estimates, and other required items. The narrative should be used to elaborate on and provide an explanation of key design decisions, alternatives, and issues and in general to communicate all information relevant to the design submission which may not be evident to the VA Design team by review of other submission materials alone. The narrative should also summarize key information, input and decisions provided to the A/E by the VA Design Team which may have significantly influenced the course and outcome of the design. During the early stages of design, the narrative should especially be used to document in summary form the key issues, objectives, obstacles and alternatives of the project and a logical explanation of why one design choice was made over other alternatives. Any specific construction phasing requirements necessary to maintain Medical Center Operation and to adequately execute construction of the project shall also be addressed as part of the narrative.

G. Project Constructability and Construction Schedule Throughout the design process, the A/E shall consider the constructability of the project as it relates to certain design choices, construction schedule and estimated construction costs. Issues of constructability and how it affects the design of the project should be addressed in design submission narratives.

The A/E shall also be required to establish a preliminary construction schedule indicating the entire length of time estimated for the completion of the construction contract. This schedule shall include pre-construction activities (such as submittals and mobilization), specific construction activities/events, special phasing requirements and final activation of the new space by the VA users. The schedule shall be developed to a level of detail such that the duration of construction activities is measured in weeks. Although a complete and detailed network analysis using the Critical Path Method (CPM) is not required, key relationships between certain construction activities should be indicated.

SOW - 9
