About this file
This is a Pre-Award Contractor Evaluation Form used by federal agencies to assess bidder/offeror eligibility and safety compliance prior to contract award.
The form requires contractors to submit occupational safety and health (OSHA) performance data across four calendar years (2023-2026), including total man hours worked at jobsites and offices, the number of cases involving days away from work or restricted activity, the days away restricted or transferred (DART) rate calculated as the number of cases multiplied by 200,000 divided by total man hours, and the number of serious, willful, or repeat OSHA violations incurred within the past three years. Contractors must provide their six-digit North American Industrial Classification System (NAICS) code for the acquisition, identify who administers their Safety and Health Program, and supply their Experience Modification Rate (EMR) for workers' compensation insurance over the past three years. The form specifies that an EMR greater than 1.0 or four serious violations, one repeat violation, or one willful violation disqualifies the contractor from award. Contractors must attach completed OSHA 300 and 300a Forms and provide explanations for any violations. The form is completed and submitted with the contractor's proposal.
