About this file
This is a technical assessment report documenting limited indoor air quality and surface mold sampling conducted at the C.W. (Bill) Young Bay Pines VA Medical Center in Bay Pines, Florida on December 18, 2024. The assessment was performed by Terracon Consultants, Inc. (Report No. H4247213) in accordance with a proposal dated July 1, 2024, and a subsequent change order dated December 4, 2024. The work was conducted for Above Group, Inc., acting as the project manager, with services directed toward establishing baseline/background mold conditions in Building 100 designated areas scheduled for renovation to support installation of a new PET/CT imaging system.
The assessment included visual inspections, indoor air quality (IAQ) parameter measurements (CO2, CO, temperature, and relative humidity), three surface swab samples analyzed via direct microscopic examination, and four non-cultured air samples (two outdoor baseline samples and two indoor samples) analyzed for total non-viable bioaerosols. Key findings indicate mold growth confirmed in two surface samples (CS-1 at Room 2C-120 door threshold with 1,200 spores/cm² and CS-2 at Room 2C-121 windowsill with 1,300 spores/cm²), with Penicillium/Aspergillus types as the dominant genera. Interior air samples showed total mold spore counts of 100 spores/m³, not elevated compared to outdoor averages of 995 spores/m³. All IAQ measurements (CO2: 359–412 ppm, CO: 0.0 ppm, temperature: 72.3–74.4°F, relative humidity: 52.7–54.3%) were within acceptable ASHRAE standards. Recommendations include thorough cleaning of affected windowsills and floors using mild detergent solutions, with particular attention to doorway transitions and wall-to-floor joints to prevent water migration and microbial growth.

Terracon Report No. H4247213
Dear Ms. Knight:
Terracon Consultants, Inc. (Terracon) is pleased to present the results of the limited indoor air
& surface mold sampling, performed on December 18, 2024 in the designated areas of the
Bay Pines VA Medical Center located in Bay Pines, FL. This assessment was conducted in general accordance with Terracon Proposal Order PH4247213 Dated July 1, 2024 and subsequent change order, dated December 4, 2024. Our services were requested to perform background/baseline mold sampling in areas to be renovated as part of the new PET/CT installation project.
PROJECT DESCRIPTION
Project Background
It is our understanding that the designated areas of Building 100 are scheduled for renovations as part of upgrades to support the new PET/CT system. It was requested that
Terracon perform baseline/background surface and airborne mold sampling. Air and surface samples were obtained in random areas and in areas where conditions could support microbial growth, based on visual observations.
Scope of Services
Terracon Consultants, Inc. (Terracon) representative Mr. Karl Kologiski conducted the air and surface mold sampling on December 18, 2024. Copies of applicable licenses and certifications are included in Attachment C.
Indoor air quality (IAQ) parameters that were also measured in designated indoor as well as outdoor locations included carbon monoxide (CO), carbon dioxide (CO2), temperature, and relative humidity (RH).

FIELD ACTIVITIES
Visual Assessment
A visual assessment was performed in designated areas. The indoor environment was observed for visual evidence of water damage, suspect mold growth, or other hygiene concerns that could affect indoor air quality. Destructive sampling/testing and observations of the HVAC system were not within the scope of work for this project.
General IAQ Measurements
The CO2, temperature and RH measurements were obtained from the designated areas using a
QTrak™ (Model 9565) Indoor Air Quality instrument manufactured by TSI. The instrument is calibrated by a third party annually. Measurements were obtained from the designated indoor location and the outdoor environment, for comparison purposes.
Surface Sampling
Three swab samples were collected using laboratory-supplied swabs rolled over any suspect mold-impacted area. The surface swab samples were submitted under chain of custody (COC) to Eurofins for identification and quantification of bioaerosols (i.e., mold spores, mold hyphal structures, etc.) via light microscopy. Eurofins is accredited by the AIHA® Laboratory
Accreditation Programs, LLC under the Environmental Microbiology Laboratory Accreditation
Program.
Samples taken from noted surfaces were analyzed by direct microscopic examination. The primary purpose of a direct microscopic examination of a sample taken from a surface is to determine whether or not mold is growing on the surface sampled, and if so, what kinds of mold are present. This type of analysis may identify marker genera that may be indicative of indoor growth as opposed to simply settled spores. The presence of biological materials on a particular surface is not a direct indication of what may be in the air.
Spore Trap Air Sampling
Terracon collected four non-cultured air samples (including two outdoors) for analysis for total non-viable bioaerosols. Samples were collected using a tripod to elevate the sample approximately four feet from ground level. The outdoor samples were collected to establish a baseline for air quality.
The samples were collected utilizing a Buck BioAire™ pump calibrated at 15 liters of air per minute prior to sample collection. Calibration was performed using a sample cassette in conjunction with a manufacturer-supplied calibration device. Each sample was collected over a span of 10 minutes for a total volume of 150 liters of air/sample.
The samples were submitted under chain of custody (COC) to Eurofins for identification and quantification of bioaerosols (i.e., mold spores, mold structures).

EVALUATION CRITERIA
Visual Assessment
Since many building materials can provide a source of food for mold spores, water is the limiting factor in minimizing the potential for mold growth. Therefore, visible fungal/mold spore stains, excessive dirt, and water-damaged building materials should not be observed. When these conditions are observed, it indicates a need to conduct cleaning or remediation activities to minimize the potential for mold-related issues.
General IAQ Measurements
Carbon Dioxide
The American Society of Heating, Refrigerating, and Air-Conditioning Engineers, Inc. (ASHRAE)
Standard 62.1-2022, Ventilation for Acceptable Indoor Air Quality, gives specific design criteria for determining the ventilation rates for different types of facilities. The standard calls for outdoor air supply requirements based on the usage of the space, the occupant density, and the square footage of the space. Generally stated, ASHRAE notes that CO2 concentration for offices no greater than 700 ppm above the outdoor air concentration equates to a breathing zone outdoor airflow rate of 15 cfm per person.
OSHA regulates occupational exposure to CO2 under 29 Code of Federal Regulations (CFR)
1910.1000, Table Z-1 – Limits for Air Contaminants. The permissible exposure limit (PEL) for
CO2 is 5,000 parts per million (ppm) as an 8-hour TWA.
The ACGIH® has established threshold limit values (TLV®) for CO2 of 5,000 ppm as an 8-hr
TWA and 30,000 ppm as a short-term exposure limit (STEL). TLVs® are guidelines and are not enforced by OSHA.
Carbon Monoxide
The EPA has established a CO standard for outdoor air. Currently, the National Ambient Air
Quality Standard (NAAQS) is 9 ppm, as an 8-hour average. Many indoor air quality professionals consider the NAAQS standard to be applicable as a conservative, but realistic guideline when applied to the indoor environment.
Temperature and Relative Humidity
Indoor air temperature and relative humidity are physical conditions important to the perception of comfort. ASHRAE Standard 55-2023, Thermal Environmental Conditions for
Human Occupancy recommends indoor temperatures during the cooling season (i.e., summer) between 73- and 79-degrees Fahrenheit (oF).During the heating season (i.e., winter), recommended temperatures can range between 68 and 75 oF. For all seasons, the recommended RH range is between 30 and 60%.
Facilities | Environmental | Geotechnical | Materials 4
ASHRAE Standard 55-2023, recommends that relative humidity in occupied spaces be controlled to less than 60% to reduce the likelihood of conditions that can lead to condensation and microbial growth.
Surface Sampling
At present, no mandatory regulations or standards have been established for the maximum allowable concentration of mold spores.
Spore Trap Sampling
At present, no mandatory regulations or standards have been established for the maximum allowable concentration of bio-aerosols such as mold spores. Over the past years, industrial hygienists, and researchers from a number of governmental and non-governmental agencies have collectively gathered a significant body of data from air, dust and surface samples during both investigative studies and remediation projects. The recommended criteria for evaluating airborne mold concentrations that have emerged from peer-reviewed publications include the following:
◼ The mold spore concentrations in indoor air should be quantitatively lower than, but qualitatively similar to, that of outdoor air.
◼ The presence of one or more mold genera at significant levels indoors but not outdoors is evidence of indoor amplification (i.e., biological growth occurring in the indoor environment).

Carbon Dioxide (CO2)
The average outdoor CO2 concentration measured was 335 ppm. Therefore, based on ASHRAE
Standard 62.1-2013, indoor locations with CO2 concentrations greater than 1,035 ppm (335 ppm + 700 ppm) would be areas suspected of insufficient outdoor air delivery.
• The CO2 concentration measured inside the designated areas ranged from 359 ppm to 412 ppm.
The CO2 results were below the above noted interior CO2 concentration of 1,035 ppm based on
ASHRAE Standard 62.1-2013. A summary of the CO2 measurements is presented in Table 2.0.
Carbon Monoxide (CO)
Refer to Table 2.0 for measured CO levels and locations collected during the assessment.
Measured concentrations were below referenced threshold levels.
Temperature and Relative Humidity
The average outdoor temperature was 72.9 ° Fahrenheit (F). Designated area temperature readings were 72.3 and 74.4°F. The average outdoor RH was 74.8%. Designated areas RH levels were 52.7% and 54.3%.
Based on the data collected, the relative humidity readings and temperature levels recorded from the noted areas were within ASHRAE recommended levels. A summary of IAQ measurements is presented in Table 2.0.

The three surface swab samples were collected from areas of visible suspect staining / debris on surfaces in the designated areas. The purpose of a direct microscopic examination of a sample taken from a material is to document the level of total mold present on the material sampled, and if so, what kinds of molds are present. This type of analysis may identify marker genera that may be indicative of indoor mold growth. The presence of biological materials on a particular surface is not a direct indication of what may be in the air.
Based on the visual observations, size of the suspect microbial growth area and the reported quantity of Penicillium/Aspergillus type spores per square centimeter, mold growth was confirmed in samples CS-1 and CS-2. The other remaining settled spores were reported at De minimis levels.
De minimis levels per industry consensus are not considered to be a concern as related to
Indoor Air Quality (IAQ) as the levels present are not considered significant and are not considered evidence of indoor amplification. The mold spores categorized as de minimis settle out on surfaces from the indoor air, and are introduced into the building via outdoor air infiltration, occupant traffic, from interior plant matter, etc.
Background debris reported in the samples results often constitute pollen, dirt, cellulose – synthetic fibers, etc.
At present, no mandatory regulations or standards have been established for the maximum allowable concentration of mold spores on surfaces. Swab samples provide semi-quantitative results for total mold contamination. Samples are analyzed by direct examination which identifies mold spores and structures but does not differentiate between viable and nonviable mold spores.

• Total indoor airborne mold spore counts were not elevated when compared to the average of the outdoor airborne mold spore concentrations.
• The bolded indoor bioaerosol mold spore concentrations noted in Green could be interpreted as de minimis.
The total bioaerosol values documented in this report are empirical values. To date, no data support a medically based threshold limit or dose-response relationship for exposure to mold aerosols or mold aeroallergens. No state or federal exposure limits have been established for mold aerosols. The bioaerosol laboratory report is presented in Appendix B.

CONCLUSIONS & RECOMMENDATIONS
Conclusions Recommendations
Mold growth was confirmed along with visible debris, and dust on the windowsill of Room 2C- 121 (Storage).
It is recommended that a thorough cleaning be performed of the windowsills, utilizing a mild detergent solution.
It is also recommended that the windowsills in all rooms be cleaned as part of routine housekeeping activities.
Mold growth was confirmed along with visible debris & staining on carpeted / tile floor transitions (door threshold) in Room 2C-120.
Dirt and debris was observed on the tiled floors in various locations of the designated areas.
It is recommended that when cleaning / moping the tiled floor areas, water accumulation be removed and the floor be dried as soon as possible to reduce the risk of water migrating in the adjacent carpeted areas and lower sections of drywall walls.
It is recommended that a thorough cleaning be performed of the floor utilizing a mild detergent solution. Areas of staining and debris building up at doorways and wall to floor joint should be given special consideration.

Spore types listed without a count or data entry were not detected during the course of the analysis for the respective sample, indicating a raw count of <1 spore.
† The spores of Aspergillus and Penicillium (and others such as Acremonium, Paecilomyces) are small and round with very few distinguishing characteristics. They cannot be differentiated by non-viable sampling methods. Also, some species with very small spores are easily missed, and may be undercounted.
††Background debris indicates the amount of non-biological particulate matter present on the trace (dust in the air) and the resulting visibility for the analyst. It is rated from 1+ (low) to 4+ (high). Counts from areas with 4+ background debris should be regarded as minimal counts and may be higher than reported. It is important to account for samples volumes when evaluating dust levels.
The analytical sensitivity is the spores/m^3 divided by the raw count, expressed in spores/m^3, per spore and per sample.
For more information regarding analytical sensitivity, please contact QA by calling the laboratory.
