About this file

This is a Limited Pre-Renovation Asbestos and Lead Paint Survey Report for the C.W. (Bill) Young, Bay Pines VA Medical Center in Bay Pines, Florida, prepared by Terracon Consultants, Inc. for Above Group Inc. The survey was conducted on December 17-18, 2024, and June 12, 2025, to identify asbestos-containing materials (ACM) and lead-based paint in designated Building 100 areas scheduled for renovation to install new PET/CT equipment. The survey covered interior areas including rooms 2C118, 2C107, 2C107A, 2C112D, 2C129, SC120, 2C121, 2C124, 2C125, and various hallways, with floor finishes of vinyl tile, faux wood plank, carpet, and carpet tile; wall finishes of painted drywall, vinyl cove base, ceramic tile, and plastic wall panels; and suspended ceiling systems.

The asbestos survey identified 29 homogeneous areas with 79 bulk samples collected and analyzed. Results detected asbestos in multiple materials classified as Non-Friable Category I (NESHAP/OSHA classification), including 3-5% chrysotile in mixed yellow/black mastics under vinyl floor tiles and carpet (approximately 1,414 square feet total), 2-10% chrysotile in white paper-wrapped thermal system insulation (TSI) on mechanical equipment (80 linear feet total), 2% chrysotile in tan duct mastic (20 linear feet), and 20 wood interior doors assumed to contain asbestos. The lead paint survey of eight samples showed no detectable lead concentrations above laboratory reporting limits. All identified asbestos-containing materials require removal by a Florida-licensed asbestos abatement contractor as OSHA Class II work activities prior to renovation, with compliance required under EPA NESHAP regulations (40 CFR 61, Subpart M) and Florida Department of Environmental Protection requirements, including third-party air monitoring during abatement. The survey was limited to accessible areas, and additional suspect materials may exist in concealed locations.

1.0 INTRODUCTION

Terracon Consultants, Inc. (Terracon) conducted a limited pre-renovation asbestos survey, and limited lead paint survey of the designated Building 100 areas of the C.W. (Bill) Young, Bay Pines VA Medical

Center in Bay Pines, Florida. Surveys and sampling were limited to suspect materials located within interior areas that are part of the PET/CT installation in the designated areas.

The survey was conducted from December 17-18, 2024 and June 12, 2025, by Terracon representative

Mr. Karl Kologiski, an Asbestos Hazard Emergency Response Act (AHERA)-accredited asbestos inspector in general accordance with Terracon Proposal PH4247213, dated July 1, 2024. Although reasonable effort was made to survey all accessible suspect materials within the designated interior and roof locations, additional suspect but un-sampled materials could be located in walls, in voids, or in other inaccessible or concealed areas. Applicable licenses and certifications are presented in Appendix D.

Homogeneous areas of suspected asbestos-containing materials (ACM) and suspect lead paint were visually identified, surveyed, and documented. Asbestos sampling was performed in general accordance with the United States (U.S.) Environmental Protection Agency (EPA) 40 Code of Federal Regulations

(CFR) Part 763 Subpart E 763, known as AHERA. Suspect lead paint samples were obtained from representative surfaces potentially containing lead coatings.

1.1 Project Objective

Terracon’s services were performed to accommodate planned renovation activities and to satisfy requirements of the EPA regulation 40 CFR 61, Subpart M, National Emission Standards for Hazardous

Air Pollutants (NESHAP). NESHAP regulations prohibit the release of asbestos fibers to the atmosphere during renovation or demolition activities. The asbestos NESHAP requires that potentially regulated ACM

(RACM) be identified, classified, and quantified prior to planned disturbances or demolition activities.

Lead paint testing was performed to comply with applicable sections of the U.S. Occupational Safety and

Health Administration (OSHA) Lead Standard (29 CFR 1926.62) for disturbance of paint/coatings with lead that will be encountered during renovation or demolition activities.

1.2 Reliance

This report is for the exclusive use of Above Group, Inc. (Above Group) for the project being discussed.

Reliance by any other party on this report is prohibited without written authorization of Terracon and

Above Group. Reliance on this report by Above Group and all authorized parties will be subject to the terms, conditions, and limitations stated in the proposal, this report, and the existing agreement between Above Group and Terracon. 2.0 BUILDING DESCRIPTION

The designated interior areas of Building 100 included in the survey consisted of the mechanical system and the following rooms: 2C118, 2C107 & 2C107A, 2C112D, 2C129, SC120, 2C121, 2C124, 2C125 ,Hallways C2-47, C2-49, C2-50 C2-52 and mechanical system mezzanine. Interior floor finishes of the Building 100 areas in the subject areas included vinyl floor tile, faux wood plank, carpet, and carpet tile. Wall finishes included painted drywall - joint compound, vinyl cove base, ceramic tile, and plastic wall panel. Ceilings consisted of a suspended ceiling grid and ceiling tiles.

3.0 ASBESTOS FIELD ACTIVITIES

The asbestos portion of the survey was conducted on December 17-18, 2024 and June 12, 2025, by

Terracon representative Mr. Karl Kologiski an AHERA-accredited asbestos inspector. A copy of the inspector certificates and applicable licenses are attached as Appendix F. The survey was conducted in general accordance with the sample collection protocols established in EPA 40 CFR Part 763 Subpart E

763.86, AHERA. A summary of survey activities is provided below.

3.1 Visual Assessment

Survey activities were initiated with limited visual observations of the accessible, designated areas to identify homogeneous areas of suspect ACM. A homogeneous area (HA) consists of building materials that appear similar throughout in terms of color and texture with consideration given to the date of application. The limited assessment was conducted in readily accessible areas of the buildings.

3.2 Physical Assessment

A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material which can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials.

3.3 Sample Collection

Based on results of the visual observations, bulk samples of suspect ACM were collected in general accordance with AHERA sampling protocols. Samples of suspect materials were collected in randomly selected locations in each homogeneous area. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker.

The selection of sample locations and frequency of sampling were based on Terracon’s observations and the assumption that like materials in the same area are homogeneous in content. A total of seventy-nine (79) bulk samples were collected by Terracon from twenty-nine (29) homogeneous areas of suspect ACM.

Facilities | Environmental | Geotechnical | Materials 3

3.4 Sample Analysis

Bulk samples were submitted under secure chain of custody protocols to Scientific Analytical Institute

(SAI) of Greensboro North Carolina for analysis by polarized light microscopy with dispersion staining techniques per EPA methodology 600/R-93/116. The percentage of asbestos, where applicable was determined by microscopic visual estimation. SAI is accredited under the National Voluntary

Laboratory Accreditation Program (NVLAP) Accreditation No. 200664-0. Asbestos laboratory data is presented in Appendix A. Representative asbestos sample photographs are presented in Appendix C.

Asbestos sample locations are presented in Appendix E.

4.0 LEADPAINT FIELD ACTIVITES

4.1 Visual Observations

The lead paint survey was limited to readily observable and accessible surfaces that may be disturbed during the planned renovations. Terracon cannot guarantee that all lead paint coated surfaces were identified during our survey as some lead paint may be hidden from sight or in inaccessible locations, or the homogeneous construction areas identified may not be truly homogeneous. This limited lead paint survey was not performed in accordance with U.S. Department of Housing and Urban

Development (HUD) Guidelines for the Evaluation and Control of Lead-Based Paint (LBP)

Hazards in Housing or applicable to EPA 40 CFR 745 requirements. It should be understood that this limited lead paint survey was not considered to be comprehensive in nature, and the results are not intended to be used to determine all lead hazards, develop lead abatement plans, or prepare detailed cost estimates for lead abatement.

4.2 Visual Assessment

A visual assessment of each unique combination, defined as a color, component, and substrate, of suspect lead paint was conducted in addition to an assessment of the paint condition.

4.3 Sample Collection

A total of eight (8) paint chip samples of suspect lead paint were collected. Paint chip samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker.

Laboratory results are presented in Appendix B. Representative lead paint sample photographs are presented in Appendix C.

4.4 Sample Analysis

Paint chip samples were submitted under chain of custody to Scientific Analytical Institute (SAI) for analysis by flame atomic absorption spectrometry per EPA SW-846/3050B/7000B. SAI is accredited under the Environmental Lead Laboratory Accreditation Program (ELLAP) (Accreditation No. 173190).

5.0 ASBESTOS REGULATORY OVERVIEW

The asbestos NESHAP (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions and asbestos waste disposal practices. The NESHAP regulation also requires the identification and classification of existing ACM according to friability prior to demolition or renovation activity.

Under NESHAP, asbestos-containing building materials are classified as:

1. Friable ACM, or

2. Category I Non-Friable ACM, or

3. Category II Non-Friable ACM.

Friable materials are those that, when dry, may be crumbled, pulverized, or reduced to powder by hand pressure. Category I Non-Friable ACM includes packings, gaskets, resilient floor coverings and asphalt roofing products containing more than 1% asbestos. Category II Non-Friable ACM are any materials other than Category I Non-Friable ACM that contain more than 1% asbestos.

Under NESHAP, Regulated Asbestos-Containing Materials (RACM are classified as:

1. Friable ACM.

2. Category I Non-Friable ACM that is in poor condition and/or has become friable.

3. Category I Non-Friable ACM that will or has been subjected to sanding, grinding, cutting, or abrading.

4. Category II Non-Friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act upon the material in the course of renovation or demolition operations.

In the State of Florida, asbestos activities are regulated by the Florida Department of Environmental

Protection (FDEP). Some counties have developed an enforcement division to carry out the responsibilities of the FDEP and have developed environmental and asbestos ordinances in which additional compliance is required. Since the facility is located in Pinellas County Florida, the NESHAP regulations are under the authority of the Pinellas County Air Quality Division (727-464-4425).

Per the NESHAP, RACM must be removed prior to renovation or demolition activities that will disturb the materials. Removal of any ACM or RACM must be conducted by a State of Florida-licensed asbestos abatement contractor, in compliance with Florida Statute (F.S) 469.001-014. In addition, third party air monitoring should be performed during and following the abatement.

The OSHA Asbestos standard for construction (29 CFR 1926.1101) regulates employee workplace exposure to asbestos. The OSHA standard has (2) Permissible Exposure Limits (PEL) that require employee exposure to airborne asbestos fibers be maintained below 0.1 asbestos fibers per cubic centimeter of air (0.1 f/cc) for an 8-hour Time-Weighted Average (8-Hr. TWA) and 1.0 f/cc for a 30-minute TWA (Excursion Limit). The OSHA standard classifies construction and maintenance activities which could disturb ACM and specifies work practices and precautions which employers must follow when engaging in each class of regulated work.

6.0 LEAD-CONTAINING PAINT REGULATORY OVERVIEW

Lead is regulated by the EPA, HUD, Florida Department of Environmental Protection (FDEP) and OSHA.

The EPA and FDEP regulate lead use, removal, and disposal, and OSHA regulates lead exposure to workers.

The EPA defines Lead-Based Paint (LBP) as any paint, varnish, stain, or other applied coating that contains lead equal to or greater than 1.0 mg/cm2, 5,000 mg/kg, or 0.5% by dry weight as determined by laboratory analysis.

For the purpose of the OSHA lead standard, lead-containing materials includes any detectable concentrations of metallic lead, all inorganic lead compounds, and organic lead soaps. A synopsis of the OSHA regulations (29 CFR 1926.62) and the applicability are as follows:

The OSHA Lead Standard for Construction (29 CFR 1926.62) applies to all construction work where an employee may be occupationally exposed to lead. All work related to construction, alteration, or repair

(including painting and decorating) is included. The Lead-in-Construction standard applies to any detectable concentration of lead in paint, as even small concentrations of lead can result in unacceptable employee exposures depending upon on the method of disturbance-removal and other workplace conditions.

Under this standard, construction includes, but is not limited to, the following:

◼ Demolition or salvage of structures where lead or materials containing lead are present.

◼ Removal or encapsulation of materials containing lead.

◼ New construction, alteration, repair, or renovation of structures, substrates, or portions containing lead, or materials containing lead.

◼ Installation of products containing lead

◼ Lead contamination/emergency clean-up.

◼ Transportation, disposal, storage, or containment of lead or materials containing lead on the site or location at which construction activities are performed.

◼ Maintenance operations associated with construction activities described above.

Laboratory results are presented in Appendix A. Representative asbestos sample photographs are presented in Appendix C. Asbestos sample locations are presented in Appendix E. Estimated quantities are based on a cursory field evaluation, and actual quantities may vary significantly, especially if asbestos containing materials are present in hidden and/or inaccessible areas not evaluated as part of this survey. It should be noted that the quantity provided is an estimate only and should not be used for bidding or notification purposes. It should be noted that suspect materials other than those identified may exist. Should suspect materials other than those which were identified during this survey be uncovered prior to or during the renovation process, those materials should be assumed asbestos-containing until sampling and analysis can confirm or refute their asbestos content.

7.2 Recommendations

Non-Friable Cat I Mixed Yellow/Black Mastics In its current state, the noted mastics can be considered a Non-Friable, Category I ACM. As such, this material should be removed by a Florida licensed asbestos abatement contractor as an OSHA Class II work activity. In addition, The State of Florida requires any concrete waste (CMU, Slab, etc.) to be free of any ACM material (paint, mastics, etc.) prior to disposal or recycling.

Non-Friable Cat I, Tan Duct Mastic and White TSI Mastic

In its current state, the noted mastics can be considered a Non-Friable, Category I ACM.

Assumed ACM, Interior Door and Frame Assemblies

Due to the on-going operations in the designated areas of the PET/CT scan areas, the doors and frames could not be tested for asbestos at the time of the survey. The metal door frames were labeled as fire-rated door frames, however there were no markings on the wood doors. The door and frame assemblies are assumed to be asbestos containing and are to be treated as such, until the designated areas are vacated, at which time, the doors and frames can be drilled so that the interior contents can be sampled for asbestos content.

Terracon recommends that if any materials not sampled in this survey are uncovered during planned work that these suspect materials should be considered asbestos containing until sampled and proven otherwise. It is recommended that any materials identified as containing asbestos be removed prior to renovation or demolition activities by a Florida-Licensed Asbestos Abatement Contractor. We also recommend direct contact with the Pinellas County Air Quality Division (727-464-4425) for confirmation and clarifications of their asbestos program requirements.
