Questions Yes or No
Does the contractor have a badge or pass identification system that controls access and movement control that must be worn or displayed on the outer garment at all times?
Are badges easily recognizable to facilitate quick and positive identification?
Contractors will wear assigned badge/pass as briefed by the escort?
Is there a control system that provides rigid accountability procedures for handling of lost, damaged, forgotten, and no longer required badges?
EXPLAIN:
Does the contractor understand and will comply with all entry control check point requirements, and that they are subject to being searched prior to being granted entry.
The contractor will provide a copy of employee passports for ascertaining citizenship?
Has an official favorable background check been conducted on all contracted personnel?
Will the contractor comply with the local standard operating procedures for ship and or shore based operations?
Will the contractor comply with all OPSEC and anti-terrorism requirements as briefed by the escort?
Do employees understand that they must stay with the assigned escort at all times?
Do employees understand not to discuss activities taking place at the jobsite or disclose priority information to third parties?
Does the contractor understand that they must contact and update the cognizant Contract Administration Officer of any foreign national data changes?
Does the contractor understand that all subcontractors must conform to the provisions of the primary contractor’s ACP?
Does the contractor understand that in order to receive approval for foreign national employees, the contractor shall include in their ACP the following employee data: full name, place of birth, citizenship, number of years employed by the contractor, and position?
Does the contractor understand that the local ship or shore facility commanding officer will make individual determinations of desirability of access for foreign nationals?

Questions Yes or No Does the contractor understand that until written authorization from the commanding officer is received, foreign nationals will be denied access to the ship or shore facility?
Does the contactor understand that this ACP only applies to current job orders awarded under this agreement?
Does the contractor understand that they must comply with the approved ACP, and that noncompliance may result in termination of the contract if it is not in the best interest of the United States Government?
Does the contractor understand that the United States Government reserves the right to cancel the contract when determined to be in the Government’s best interest?
Does the contractor understand that the use of foreign nationals without an approved ACP or when a previously authorization has been cancelled, will be considered a violation of the security regulations, and upon confirmation by the CAO of such violation, the contract, agreement or job order issued under this agreement may be terminated for default?
Does the contractor understand that they are responsible for the actions by all subcontractors, technical representatives and other persons granted access to U.S. Navy vessels, and adjacent areas and work sites?
