About this file

This is a Past Performance Questionnaire form used by the Architect of the Capitol (AOC) to solicit feedback from previous clients of contractors bidding on federal contracts. The questionnaire is submitted by contractors to references who have previously hired them and must be returned directly to the AOC Contracting Officer Moyah Wilson (moyah.wilson@aoc.gov) and Andrew Eichhorn (andrew.eichhorn@aoc.gov) by the proposal submission deadline of Wednesday, May 20th, 2026.

The questionnaire evaluates contractor performance across 16 categories using a four-tier rating system: Outstanding (O), Satisfactory (S), Marginal (M), and Unsatisfactory (U). Evaluation areas include delivery and schedule performance, quality control and work quality, pricing reasonableness, compliance with labor and safety standards, management and coordination capabilities, corporate integrity, and customer satisfaction. Respondents must rate overall performance and provide supporting information, particularly for Outstanding or Unsatisfactory ratings. The form also requests information on corrective actions taken, contract modifications, and adverse actions such as cure notices, show cause letters, or terminations. References are asked whether they would award another contract to the contractor and to provide any additional comments. This questionnaire applies to Solicitation #AOCSSB27R0002 for Senate Food and Related Services.
