About this file
This is a Past Performance Questionnaire template for the Architect of the Capitol (AOC) related to solicitation AOCSSB26R0009 for Russell Food and Dirksen Ground & Hart Food and Related Services. The questionnaire is to be completed by previous clients or users of a contractor bidding on the Senate Food and Related Services contract and submitted directly to AOC Contracting Officer Moyah Wilson (moyah.wilson@aoc.gov) and Andrew Eichhorn (andrew.eichhorn@aoc.gov) by the revised proposal submission deadline of Friday, August 14, 2026. Responses are considered source selection sensitive and protected under Federal Acquisition Regulation 42.1503.
The questionnaire requests evaluation of contractor performance across 16 rating areas using a four-point scale (Outstanding, Satisfactory, Marginal, or Unsatisfactory). Evaluation categories include meeting delivery/completion schedules, quality control, work quality, cost reasonableness, labor and safety standards compliance, corrective actions taken, contract modifications, relationship with government personnel, subcontractor coordination, corporate management and integrity, customer satisfaction, and overall performance. Respondents must provide supporting information for outstanding or unsatisfactory ratings and explain any negative performance issues, cure notices, suspensions, or terminations. The questionnaire also asks whether the respondent would award another contract to the contractor and invites additional comments to support the overall rating assessment.
