About this file

This is a Past Performance Questionnaire template for RFP No. FA5641-26-R-0001 issued by the 764th Enterprise Sourcing Squadron for the Germany Air Mail Terminal (GAMT) IV mail handling and distribution requirement for United States Air Forces in Europe (USAFE).

The questionnaire is distributed by offerors to past clients and references who have direct knowledge of the contractor's performance history. Respondents must complete the form electronically and submit it directly to the Procuring Contracting Officer (PCO) Ms. Noemi Toth-Phillips at noemi.toth-phillips@us.af.mil, with courtesy copies to Contract Specialists Ms. Barbara Wafzig at barbara.wafzig.de@us.af.mil and Ms. Ina Hemmer at ina.hemmer.de@us.af.mil. The questionnaire is classified as source selection sensitive information under RFO 2.101 and 3.104 and must not be returned to the offeror. The template collects general contractor information including name, address, contact details, project title, contract number, kilograms of mail processed annually, and contract performance dates, along with respondent information. The performance evaluation section uses a three-point scale (S=Satisfactory, U=Unsatisfactory, N=Neutral) to assess eleven criteria: management capabilities, personnel hiring and retention, quality control plan adherence, deficiency correction, problem identification, responsiveness to contract changes, provision of secured facilities, overall performance, contract award recommendation, schedule compliance, and customs regulation compliance. Respondents must provide narrative explanations for any unsatisfactory ratings and additional remarks as appropriate.

1. The 764th Enterprise Sourcing Squadron is currently conducting a source selection for the United States Air Forces in Europe (USAFE) mail handling and distribution requirement, specifically the Germany Air Mail Terminal (GAMT) IV. One of the considerations in proposal evaluation is the verification of each offeror’s past performance. This evaluation assesses the offeror’s ability to successfully perform work of similar scope and magnitude to the current requirement. To ensure an accurate assessment, the Government relies on information provided by references with first-hand knowledge of the offeror’s performance history.

2. Specific areas of interest regarding the offeror’s past performance are summarized in the electronic past performance questionnaire e-mailed to you by the offeror.

3. While this questionnaire was distributed to you by the offeror/contractor, it is essential that you complete the form electronically and return it directly to the US Government Procuring Contracting Officer (PCO) Ms. Noemi Toth-Phillips at noemi.toth-phillips@us.af.mil. A courtesy copy shall be sent to the Contract Specialists (CS), Ms. Barbara Wafzig at barbara.wafzig.de@us.af.mil and Ms. Ina Hemmer at ina.hemmer.de@us.af.mil.

4. Please DO NOT return or e-mail the completed questionnaires to the offeror or contractor.

Once filled out, this document contains source selection sensitive information and must be handled in accordance with RFO 2.101 and 3.104. Should you have any concerns, please contact the PCO or the CS at the email addresses mentioned above.

5. Your assistance in this evaluation is greatly appreciated. A prompt response is essential to ensuring a successful and timely completion of this source selection process.
