About this file

This is Appendix C to a Performance Work Statement (PWS) for FY25 B1 Fort Cavazos Doors Maintenance & Services, establishing the Performance Requirements Summary that defines Government surveillance methods and remedies for contractor performance failures.

The appendix outlines performance objectives across 16 key service areas with corresponding standards, acceptable quality levels (AQLs), surveillance methods, and remedies for non-conformance. Critical requirements include: providing key personnel (CMM, Lead Maintenance Mechanic/M&S Supervisor, and part-time L1 SHO) at 100% staffing with deductions equal to contract costs for unfilled positions; submitting BaseOps Monthly Reports and Safety Reports with no more than one late submission per performance period (5% CLIN deduction penalty); maintaining 100% current licenses and certifications (zero deviation); completing 95% of preventative maintenance on schedule; completing 95% of corrective maintenance within specified timeframes per Table H.2.1 (with 0.5% invoice reduction per business day of delay); and preparing 95% of service order documentation within required timelines. Additional requirements mandate 99% compliance with safety requirements, 100% completion of annual safety training (AT Level I, OPSEC, iWATCH, and Cyber Awareness), 100% contractor quality control performance, and 100% rework acceptance without further defects. Inspection occurs via 100% inspection for most requirements and random sampling for safety and training items. Remedies range from invoice reductions (5% to 25% of affected CLINs) to negative CPARS narratives, employee work restrictions, and potential contract default termination for recurring issues.

C. Performance Requirements Summary

C.1. General The rights of the Government and remedies described in this section are in addition to all other rights and remedies set forth in this contract. Specifically, the Government reserves its right under the Inspection of Services and Termination for Default clauses. Any deductions pursuant to the Performance Requirements Summary (PRS) shall reflect the reduced value of service performed under the contract. The Contractor shall not be relieved of full performance of the services hereunder and may be terminated for default based upon inadequate performance of service even if a deduction was previously taken for inadequate performance. The Government may modify the type and frequency of inspection of items.

C.1.1. Purpose The Performance Requirements Summary is designed to:

C.1.1.1.

List the contract requirements considered most critical to acceptable contract performance.

(Column 1).

C.1.1.2.

List the contract paragraph number(s) which describe the requirements (Column 2).

C.1.1.3.

Define the standard of performance for each required service specified (Column 3).

C.1.1.4.

Show the maximum allowable degree of deviation or Acceptable Quality Levels (AQLs) from perfect performance that will be allowed by the Government before contract performance is considered unsatisfactory (Column 4).’

C.1.1.5.

Explain the quality assurance methods the Government will use to evaluate the Contractor’s performance in meeting the contract requirements (Column 5).

C.1.1.6.

Show the remedy available to the Contractor or Government to bring performance within the standard defined in Column 4 (Column 6).

C.1.2. Nonconformance the government will document all Contractor non-conformances.

C.1.3. Performance Requirements Summary The Performance Requirements Summary lists those services/tasks for which the Government will make deductions from payment due the Contractor when services/tasks are found to be unsatisfactory during inspections made using the checklists. These deductions are in addition to reduced invoicing described in Section 7 of the PWS and the pricing workbook.
