Such health care coverage will be first effective December 1, 2017. The fringe benefit contribution collected in November 2017 was directed to the Fringe
Benefit Group to collect for partial December premiums with any remainder to be directed to the qualified defined contribution plan.
An employee is eligible for fringe benefit contributions towards the health care coverage as of the first day of the month following the employee's first day of employment.
The Company anticipates that continued legal changes and guidance provided for under the Patient Protection and Affordable Care Act of 2010 may continue to have a material impact on health care coverage. Therefore, it is agreed that the Company may change insurers, vendors or plan design of any components of the health care coverage, which includes coverage for medical, prescription, employee assistance program ("EAP"), dental and vision coverage for employees and covered dependents during the life of this Agreement. If an employee opts-out of the health care coverage due to an approved waiver pursuant to company policy, the total fringe benefit contribution made on his/her or behalf will be directed to the qualified defined contribution plan on behalf of the employee. The employee may opt-out of all of the health care coverage, or the employee may opt-out of the medical/prescription/EAP, dental and/or vision as component parts of the health care coverage.
14.2 Each employee will be eligible to participate at his or her own full cost in other elective benefits available through the Fringe Benefit Group, The Contractors Plan.

f) The fringe benefit contribution for additional years may be increased to match the rate of actual increase in costs for each level of participation in the health care coverage. This increase, if any, will be communicated to the employees and the union once benefit renewals have been finalized each year.
g) The Family fringe benefit contribution was calculated such that the employee could receive family level health care coverage with any small balance remaining to be contributed to the qualified defined contribution plan. If the employee elects a coverage level other than family, or waives the medical/dental and/or vision, then any remaining fringe left over after paying for the health care coverage level elected (if any), will be contributed to the qualified defined contribution plan.
h) The fringe benefit contribution made on behalf of each employee will equal the fringe benefit rate times the number of hours worked (or for which an employee is entitled to paid time-off), up to a maximum of 40 hours per week (except as otherwise required by law).
