SUMMARY OF CHANGES (REVISED)

SECTION SF 30 - BLOCK 14 CONTINUATION PAGE

The following have been added by full text:

AMENDMENT 04

1. Update Solicitation Sections: 00100, 00801, 00804, 00805, and 00806.

2. Update Solicitation Attachments: Attachment B - Coefficient Table; Attachment E – JOC Statement of Work;

Attachment F - Seed Project #BDTXHT Scope of Work (Rev 01); Attachment J – Seed Project SF1442; and provide new Attachment F.3 – Seed Project CAD drawing of CIA fence line.

3. Extend the required response date and time from 2 May 2019 at 2:00 PM to no later than 30 May 2019 at 2:00 PM (local time).

4. Provide Government response to the following pre-proposal inquiries (PPIs):

PPI 019: RFP Section 00100 page 6 of 65 provides a list of items to be included with the Price Proposal. As with any project the means and methods utilized to complete the seed project may influence the proposed price but also provide added value to the Government. The current Price Proposal Inclusions does not allow for a brief seed project technical narrative, which could prove highly beneficial in analyzing proposed prices. Will the Government add a one-page seed project technical narrative to the Price Proposal inclusions?

GOVT RESPONSE: Technical narratives will not be considered as this acquisition is lowest price technically acceptable (LPTA).

PPI 020: During the site visit the Government ET said that the existing picnic tables and associated concrete slabs/foundations would be removed by the contractor. Is the contractor required to demo and dispose of the existing picnic tables and associated concrete slabs? If so will grade need to be re-established and grass seeded?

GOVT RESPONSE: The picnic tables and associated concrete will not be removed as part of this project.

PPI 021: During the site visit the Government ET said that the Government would remove the existing Jersey Barriers located on Pier 7. Will the Government remove the existing jersey barriers currently on pier 7 or will this be the responsibility of the contractor?

GOVT RESPONSE: The Government will move the barriers as needed.

PPI 022: Due to the proximity of the seed project to the nearby Puget Sound, are there any specific environmental controls that will be required during construction?

GOVT RESPONSE: See SOW Revision (Rev.) 01, Section VII, B.

PPI 023: Due to the proximity of the seed project to the nearby Puget Sound and a portion of the project occurring on Pier 7, will Longshoreman’s Insurance be required?

GOVT RESPONSE: It is the Contractor's responsibility to determine the applicability of Longshoreman's Insurance. See Section 00804, 6.

PPI 024: Please provide the incumbents name & coefficient if it is publicly available.

GOVT RESPONSE: This information is not publicly available. Coefficients are company specific.

PPI 025: Attachment-F; I. SOW; Para. 2; Pg. 1 of 3; Since it's the intent for the seed project to match existing decorative metal fencing, is it possible to get the name of the manufacturer of this specific product?

GOVT RESPONSE: No. See SOW Rev. 01, Section IV, B.

PPI 026: Attachment-F; VI. Scope; Item-B; Page 3 of 3; Item b; States, "Install approximately 700 LF decorative fence to match ferry terminal fence (tie fence to ferry terminal fence)" Is there and existing masonry column at the tie-in location?

GOVT RESPONSE: No.

PPI 027: Attachment-E; 3. DESCRIPTION; Para. 3; Page 2 of 42; States, "For task orders where the Government provides no drawings, the Contractor shall provide sketches to clearly show an understanding of what work is included in the proposal." Is this requirement applicable to the Seed project proposal submission? or is this information only required on future Task Orders after Award? Are we to provide any narratives to further describe our approach? and is so to what extent?

GOVT RESPONSE: A CAD Drawing for the Seed Project is provided with this amendment; see Attachment F.3. Technical narratives will not be considered.

PPI 028: Attachment F- Seed Project; Section I. Statement of Work; Item C; Page 3 of 3: Per the instruction for the site visit it states, "offerors will NOT be allowed to bring cameras to the site visit or take pictures". Is it possible for the Government to take and provide photos from and during the site visit and provided to all offerors?

GOVT RESPONSE: Due to project location photos are not available. A CAD Drawing for the Seed Project is provided with this Amendment; see Attachment F.3.

PPI 029: Attachment F- Seed Project; Section I. Statement of Work; references a "monolithic concrete footing".

Section VI. Scope, Item C; references "concrete footing (exposed beam above grade)" Are we to assume that there is a monolithic footing extending below grade with an exposed beam/portion extending above grade? or is it intended to just to bear on existing grade?

GOVT RESPONSE: See SOW Rev. 01, Section VI.

PPI 030: Will NAVFAC provide photos of the current site, lighted signs that need to be raised, decorative black fence, and block columns?

GOVT RESPONSE: Due to project location photos are not available.

PPI 031: Per Attachment - E; 4.1 PRE-PRICED AND NON-PRE-PRICED COEFFICIENTS; Page 3 of 42, States, "The offeror’s Pre-Priced and Non-Pre-Priced coefficients shall account for a wide variety of risks of doing business, including but not limited to:

~ Compliance with environmental laws ~ Costs associated with utility scopes, facility protection".

In the event where there is work within the Bremerton Hospital environment, there will be a requirement for ICRA.

Is this cost to be included within our coefficient? or applied to our Multiplier #03? Or would they be considered direct costs to the task order?

GOVT RESPONSE: The Multiplier for work at the Bremerton Hospital has been removed from this solicitation with this amendment. The Government will not award Hospital work under this contract. See Amendment.

PPI 032: Once the JOC is awarded, will it be acceptable for the prime contractor to include escort costs for project management oversight in task order bid proposals for all jobs located in the Limited Area and Restricted Waterfront areas? Or, shall the JOC solicitation proposal account for these type costs in the coefficient or multiplier?

GOVT RESPONSE: No, that is the purpose of the multipliers.

PPI 033: Solicitation Page 12, Section 00100, Paragraph 11, Factor 3, Past Performance: If utilizing a Past Experience project in which the offeror performed as a subcontractor, is the Past Performance PPQ to be completed by the owner or the General Contractor that the offeror worked directly under?

GOVT RESPONSE: In this case, the prime/general contractor shall complete the PPQ form.

PPI 034: Multiplier Work Percentages: Per Attachment - E; 4.3 MULTIPLIERS; Page 5 of 42, There are (4) defined multipliers based on specific locations and/or conditions, where considerations for increased exposure to cost, travel or access. What is the percentage of work has historically been performed in each classification, and/or is anticipated under this contract?

GOVT RESPONSE: For the timeframe of 11/15/2015-2/15/2018, each multiplier was approx. < 5% of the total awarded projects. However, note that the pricing structure for this contract differs from the previous JOC contract. Project locations also vary.

PPI 035: Five Percent Outlying Facilities: Per Attachment - E; 1. GENERAL INFORMATION; Page 1 of 42 Indicates that the Government estimates approximately ninety-five percent (95%) of the work will occur at Naval Base Kitsap, Naval Magazine Indian Island, Manchester Fuel Depot, Naval Station Everett, Naval Air Station Whidbey Island, and Naval Hospital Bremerton, and the remaining five (5%) will occur at other outlying facilities.

Will special considerations be allowed within our direct work costs to recover remote location work as defined in the (5%) outlying facilities? or are the remote outlying location logistics be required as a portion of our coefficient?

And if so what percentage of work is anticipated for those locations?

GOVT RESPONSE: See Amendment 02.

PPI 036: Work Load Per Installation: Per Attachment - E; 1. GENERAL INFORMATION; Page 1 of 42, States the Government estimates approximately ninety-five percent (95%) of the work will occur at Naval Base Kitsap, Naval Magazine Indian Island, Manchester Fuel Depot, Naval Station Everett, Naval Air Station Whidbey Island, and Naval Hospital Bremerton. For the benefit of analyzing potential impacts in the development of the various multipliers, could you please provide anticipated percentage of work or task orders for each installation locations.

GOVT RESPONSE: Future work locations cannot be anticipated. As noted above in PPI 31 response, hospital projects will no longer be awarded under this contract. The following information lists the number of projects awarded in specified locations during the timeframe 11/15/2015-2/15/2018: 1 @ Everett, 1 @ NASWI, 79 @ NBK Bangor, 19 @ NBK Bremerton, 24 @ NBK Keyport, 15 @ NBK Man., 5 @ NMII, 21 @ NUWC, 15 @ PSNS, 52 @ SWFPAC. NOTE: these numbers include projects inside and outside of the locations referenced by multipliers. See PPI 34 response.

PPI 037: Anticipated Annual Task Orders: How many task orders can we anticipated on a yearly basis? and what was the average dollar amount range for those task orders that were awarded under the previous JOC contract?

GOVT RESPONSE: We cannot anticipate the number of future projects. For the timeframe of 11/15/2015-2/15/2018: 241 task orders were awarded on the JOC. Award amounts ranged from approximately $8,000 to $997,000. The average award amount was approximately $197,000.

PPI 038: The designated location of the new fence line is at the top of the existing rock sea wall. It is anticipated that any digging or drilling activities will encounter the subsurface sea wall rock. Is a spread footing acceptable instead of drilling to 4-foot depth for post placement to avoid the rocks?

GOVT RESPONSE: No.

PPI 039: Attachment F, Section IV. Product Specifications, states to match existing black coated fence material.

The existing fencing finish has a G90 galvanized coating over the steel, followed by a zinc phosphate coat, then an epoxy powder coat and finally a top finish coat consisting of a polyester color coat. Is it the intention of the Government to match the existing 4-layer coating system as described above?

GOVT RESPONSE: See SOW Rev. 01, Section IV, B.

PPI 040: Will the government be providing a designated location on an installation where most task orders will be performed, for the JOC contractor to establish a field office and storage/laydown area?

GOVT RESPONSE: No.

PPI 041: Will the government be providing a copy of the site visit sign-in sheet?

GOVT RESPONSE: The site visit sign-in sheet was provided as an attached document to previously released Amendment 02.

PPI 042: Attachment-E; Section 4.3 MULTIPLIERS, 2nd Paragraph States, “Multiplier one will be for work in the Controlled Industrial Area (CIA) at NBK Bremerton” are we to assume that only the area outside the CIA would have no multiplier? And are there other areas or installations or facilities where no multiplier is to be consider?

GOVT RESPONSE: Please review location information provided with the information on the multipliers.

PPI 043: Attachment-F; VI. Scope; Item-C; Page 3 of 3; Item C; makes the reference "with RST from ferry terminal to pier 7." Please define the reference to RST? And is it to extend from the existing ferry terminal fence or to the ferry terminal?

GOVT RESPONSE: See SOW Rev 01 VI. Scope, C.

PPI 044: Attachment F- Seed Project; I. Statement of Work; Para.2; Page 1 of 3; states The fence installed should be black powder coated, similar to existing perimeter fence, with square tube metal post vice brick columns with a monolithic concrete footing. What Does the phrase "square tube metal post vice brick columns" reference? and are there any as-builts available for the existing perimeter fence that provides detailed information to determine the construction requirements?

GOVT RESPONSE: Brick columns will not be constructed. "Square tube metal post" refers to a fence post. As-builts for the existing perimeter fence are not available.

7. All other terms and conditions remain unchanged. Offerors are reminded to acknowledge receipt of this amendment when submitting proposals in accordance with the RFP instructions.

The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. In addition, if the Contracting Officer determines that the number of proposals that would otherwise be in the competitive range exceeds the number at which an efficient competition can be conducted, the Contracting Officer may limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals.

3. The LPTA process is selected as appropriate for this acquisition because the best value is expected to result from selection of a technically acceptable proposal with the lowest evaluated price. In order to permit efficient competition, the following methodology will be utilized. Proposals will initially be screened for price and placed in order of price (lowest price to highest price). The number of proposals to be evaluated for technical acceptability may be limited to the lowest priced offer at the discretion of the Contracting Officer. If the number of proposals to be evaluated is limited, the technical proposal shall be provided to the evaluator(s) without any identification of price. If the lowest priced proposal is not found to be technically acceptable then the next lowest priced proposal shall be evaluated. If that proposal is also not found to be technically acceptable, then the process described will be conducted again as many times as necessary, until such time as the Government identifies a technically acceptable proposal. If the Contracting Officer later determines that discussions are necessary, the procedures in FAR 15.306 shall be followed. Accordingly, under this methodology, the technical factors and subfactors of some proposals may not be evaluated by the Navy. If discussions are deemed necessary by the Contracting Officer, all proposals will be evaluated (both technical and price) for the purpose of establishing competitive range. At no time during the technical evaluation will the SSEB be made aware of the offerors’ pricing, nor their particular price ranking.

4. An overall non-price factors rating must be at least “ACCEPTABLE” in order to be eligible for award.

An “UNACCEPTABLE” rating in any factor results in the overall non-price factors proposal being rated “UNACCEPTABLE” unless corrected through discussions. An “UNACCEPTABLE” rating in any subfactor results in the factor being rated “UNACCEPTABLE” and the overall non-price factors being rated “UNACCEPTABLE” unless corrected through discussions. An overall non-price factors rating of “UNACCEPTABLE” makes a proposal ineligible for award. If an offeror receives a “UNACCEPTABLE” rating in any non-price factor, no additional proposal evaluation will be performed. Accordingly, under this methodology, some of the technical factors of some of the evaluated proposals may not be evaluated by the Navy.

11. EVALUATION FACTORS FOR AWARD

1. The solicitation requires the evaluation of price and the following non-price factors:

Factor 1 - Technical Approach Factor 2 – Experience Factor 3 - Past Performance Factor 4 – Safety

Price

*The distinction between experience and past performance is experience pertains to the types of work and volume of work completed by a contractor that are comparable to the types of work covered by this requirement, in terms of size, scope, and complexity. Past performance pertains to both the relevance of recent efforts and how well a contractor has performed on the contracts.

2. Basis of Evaluation and Submittal Requirements for Each Factor.

(a) Price:

For the purposes of establishing price for this solicitation, the offeror shall provide a proposed percentage for each coefficient ELIN description and multipliers one, two, and three, and four on the “Coefficients and Multipliers” tab of Attachment B, Coefficient Table and Seed Project Proposal. Failure to propose for each coefficient and multiplier will result in the offeror being ineligible for award. The proposed coefficients will become part of the contract upon award and will remain the same for the base year and all options.

On the “Seed Project” tab of Attachment B, Coefficient Table and Seed Project Proposal, the offeror is to enter in their proposed costs for the seed project. The statement of work for the seed project will be provided as Attachment F. The Total Evaluated Price will be the offeror’s total proposed Pre-Priced and Non-Pre-Priced costs of the seed project with the applicable coefficients, city cost index weighted average, multiplier, and sales taxes factored in. Any item descriptions, means item numbers, material, labor, equipment, and other unit costs entered for Pre-Priced items should come directly from R.S. Means.

While filling out both tabs of Attachment B, the offeror shall ONLY fill in grey cells and shall not alter any cells that are not grey. This form contains pre-established mathematical formulas for the express purpose of reducing offerors’ calculation errors and assisting in evaluating the offeror’s price. Revising the formulas, or adding, deleting, or revising any other portion of the form not specified above may result in an unclear or ambiguous price, thereby making the offer ineligible for award without discussions.

Coefficient – A coefficient includes any and all costs (generally indirect costs) above and beyond RS Means bare costs with the exception of specialized costs unique to a given Task Order project (e.g.

specialized training, USL&H insurance, specific environmental permit, etc.). Specialized costs may be negotiated with each Task Order.

Please review section four, Coefficients and Multipliers, and section five, Pre-Priced and Non-Pre-Priced Work of Attachment E, N44255-18-R-5005 Statement of Work for a more detailed description of what should be included in the Pre-Priced and Non-Pre-Priced Coefficients as well as the multipliers for this contract.

(1) Solicitation Submittal Requirements-The offeror’s price proposal shall be separate from the technical proposal. Complete and submit the following:

(i) Cover letter in accordance with FAR 52.215-1(c)(2), including DUNS number

(ii) SF 1442, Solicitation, Offer, and Award Blocks 14 through 20c

(iii) Attachment B, Coefficient Table and Seed Project Proposal

(iv) Letter from the offeror’s surety. The letter should verify the offeror has a minimum of $1,500,000 bonding capacity per project and $20,000,000 aggregate.

(v) Bid Bond in the amount of 20% of the Seed Project.

(vi) Ensure the offeror’s Representations and Certifications, including the supplemental certifications included in Section 00600, are current and posted in SAM.

(2) Basis of Evaluation: The Government will evaluate price based on the Total Evaluated Price of the seed project, as calculated in Attachment B, Coefficient table and Seed Project Proposal. Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:

(i) Comparison of proposed prices received in response to the RFP.

(ii) Comparison of proposed prices with the IGE.

(iii) Comparison of proposed prices with available historical information.

(iv) Comparison of market survey results.

Evaluation of the price proposal will determine the reasonableness, and completeness of the offeror’s proposal.

Evaluation will determine the offeror’s comprehension of the requirements of the RFP as well as to assess the degree to which the proposed price accurately reflects proposed performance.

The Bid Bond will be evaluated for accuracy and completeness in accordance with FAR 28.101.

The letter from the bonding company will be evaluated to assess the risk of the offeror listed in Block 15A of the SF1442 being able to provide sufficient bonding for both individual and multiple task orders during the life of this contract.

Representations and Certifications will be reviewed in SAM to ensure they are complete.

(b) Technical Factors:

Note 1: “Offeror” refers to the entity submitting the proposal, as determined by the name, address, Entity Identifier Number, and Commercial and Government Entity (CAGE) code that appears in Block 14 of the SF 1442 for construction solicitations or Block 15 of the SF 33 for service solicitations. If the offeror's proposal presents the experience, past performance, or safety data of any entity with an Entity Identifier Number or CAGE code that differs from that in the SF 1442 OR SF 33, the information will not be considered unless the offeror establishes a basis for imputing the other entity's data to the offeror. The Government will not assume or independently investigate the existence and nature of a relationship between the offeror and any other entity that has a different Entity Identifier Number and CAGE code. The offeror must clearly explain why another entity's experience, past performance, or safety record is relevant to or indicative of the offeror's ability to perform this contract. A mere statement describing a corporate affiliation is insufficient; the offeror must also establish how the connection or nexus between it and the other entity relates to the offeror's qualifications. The explanation must be included in the Construction Experience Project Data Sheet (Attachment C) required under Factor Two, Experience. An offeror's failure to adequately justify a basis for crediting it with another entity's experience, past performance, or safety data could result in a deficiency, thereby making the offeror ineligible for award without discussions.

Note 2: While the Government may elect to consider data obtained from other sources, the burden of providing detailed, current, accurate, and complete experience, past performance, management, technical, management, safety, and small business utilization information rests with the offeror.

Note 3: The non-price proposal shall be separate from the price proposal. See Section 00100 for additional instructions, format, and submission quantities required for the proposal.

Note 4: The page limitation for each factor/subfactor is provided in the submission requirements for each factor found later in this section. Page limitations will be strictly enforced. Any pages submitted in excess of the page limitations will not be reviewed and any data or material they contain will not be considered. Specific submission items which are excluded from the page count are listed in each factor later in this section.

Note 5: Projects submitted for Factor Two, Experience, shall be the same projects submitted for Factor Three, Past Performance.

Note 6: Failure to meet solicitation submittal requirements may result in an unacceptable rating.

(1) Factor 1, Technical Approach:

(a) Solicitation Submittal Requirements:

The composition and management of the firms proposed as the team for this contract will be evaluated in this factor.

The offeror shall submit the following information:

(1) Provide a narrative describing the proposed primary construction firms for this contract and the rationale for proposing this arrangement. Provide the role, responsibilities, and contractual relationships between the various firms (see FAR Subpart 9.6). The narrative shall also include a simple organizational chart that clearly identifies the lines of authority between the entities. If the experience of an entity is being claimed in Factor Two, that entity must be named in the above narrative and organizational chart.

The technical approach narrative shall be limited to one double-sided page (or two single-sided pages) including the organizational chart. The information requested in item two below is not included in this page limitation.

(2) In addition to the narrative, the offeror shall submit a signed copy of a joint venture agreement, partnership agreement, teaming agreement, approved mentor protégé agreement (MPA), or letter of commitment for each member of the offeror’s team identified above (e.g., joint venture member, partner, team member, subcontractor, parent company, subsidiary, or other affiliated company, etc.).

(b) Basis of Evaluation:

The assessment of the offeror’s technical approach will be used as a means to evaluate the organizational structure and teaming relationships proposed by the offeror. This factor will be rated on an Acceptable or Unacceptable basis.

(2) Factor 2, Experience:

(1) Solicitation Submittal Requirements:

The offeror shall submit the following information:

(i) Construction Experience:

Submit up to five construction projects for the offeror that best demonstrates your experience on relevant projects.

Relevant projects are those that are similar in size, scope, and complexity. For purposes of this evaluation, similar in size, scope, and complexity is further defined as:

Size: Similar in size are projects between $2,000 to $1,500,000. One project shall be over $500,000.

Scope: Similar in scope are general construction projects involving replacement, repair, demolition, or alterations of existing facilities and equipment, such as electrical, mechanical (to include HVAC equipment and controls), fire protection equipment (to include alarm panels and pumps), painting, facility renovation, equipment repair and/or alteration, and other general construction activities. New construction would be limited, and of a straightforward, uncomplicated nature. Work may take place in secure industrial areas. or hospital environments.

One project shall be facility renovation, remodel, or repair.

Complexity: Similar in complexity is considered as managing projects with limited site access and lay-down area, compliance with environmental regulations, and work in an industrial environment.

Projects submitted for the offeror shall be completed, or substantially complete, within the past five years of the date of issuance of this RFP. The term “substantially complete” shall mean a construction project with construction of the facility/project more than 80% complete.

Offerors failing to provide relevant projects within the specified time frame demonstrating experience in the above may be rated unacceptable.

A project is defined as a construction project performed under a single task order or contract. For multiple award and indefinite delivery/indefinite quantity type contracts, the contract as a whole shall not be submitted as a project;

rather offerors shall submit the work performed under a task order as a project.

The attached Construction Experience Project Data Sheet (Attachment C) is MANDATORY and SHALL be used to submit project information. Except as specifically requested, the Government will not consider information submitted in addition to this form. Individual blocks on this form may be expanded; however, total length for each project data sheet shall not exceed one double-sided page (or two single-sided pages).

For all submitted projects, the description of the project shall clearly describe the scope of work performed and the relevancy to the project requirements of this RFP (i.e.: unique features, area, construction methods).

If the offeror is a Joint Venture (JV) or Mentor Protégé (MP), relevant project experience should be submitted for projects completed by the Joint Venture/Mentor Protégé entity. If the Joint Venture/Mentor Protégé does not have shared experience, projects shall be submitted for each Joint Venture/Mentor Protégé partner. Offerors are still limited to a total of five projects combined. Joint Venture/Mentor Protégé offers must include at least two relevant projects for each partner. If the Joint Venture/Mentor Protégé includes more than two firms, the team's submission shall include at least one relevant project for each firm. Offeror may receive an unacceptable rating if the minimum number of projects for each firm is not submitted.

If an offeror is utilizing experience information of affiliates/subsidiaries/parent companies (name is not exactly as stated on the SF1442), the proposal shall clearly demonstrate that the affiliate/subsidiary/parent firm will have meaningful involvement in the performance of the contract

(2) Basis of Evaluation:

The basis of evaluation will include the offeror’s demonstrated experience and depth of experience in performing relevant construction defined in the solicitation submittal requirements. The assessment of the offeror’s relevant experience will be used as a means of evaluating the capability of the offeror to successfully meet the requirements of the RFP. The Government will only review five projects for construction. Any projects submitted in excess of the five for Construction Experience will not be considered.

(c) Past Performance

(3) Factor 3, Past Performance:

(a) Solicitation Submittal Requirements:

If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted with the proposal for each project included in Factor Two, construction experience. If there is not a completed CPARS evaluation, then submit Past Performance Questionnaires (Attachment D) for each project included in Factor Two for Construction Experience. The offeror should provide completed Past Performance Questionnaires (PPQ) in the proposal. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror shall complete and submit with the proposal the first page of the PPQ, which will provide contract and client information for the respective project(s). The Government may make reasonable attempts to contact the client noted for that project(s) to obtain the PPQ information. However, offerors should follow-up with clients/references to help ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Amanda Bonus, amanda.bonus@navy.mil.

Offerors may provide any information on problems encountered and the corrective actions taken on projects submitted under Factor Two – Experience. Offerors may also address any adverse past performance issues.

Explanations shall not exceed two (2) double-sided pages (or four (4) single-sided pages) in total.

The Government reserves the right to contact references for verification or additional information. The Government’s inability to contact any of the offeror’s references or the references unwillingness to provide the information requested may affect the Government’s evaluation of this factor.

Performance award or additional information submitted will not be considered.

(b) Basis of Evaluation:

This evaluation will consider how well the offeror’s team performed on relevant projects submitted under Factor Two – Experience and may also consider past performance on other projects currently documented in known sources.

The Government will consider the recency and relevance of the information, the source of the information, context of the data, and general trends in the Contractor’s performance. This evaluation is separate and distinct from the Contracting Officer’s responsibility determination. The assessment of the offeror’s team’s past performance will be used as a means of evaluating the offeror’s team’s probability to successfully meet the requirements of the RFP.

Offerors lacking relevant past performance history will receive a NEUTRAL Confidence rating.

(4) Factor 4, Safety

(a) Submittal Requirements:

The offeror shall submit a narrative that fully describes the safety management system that they will use to oversee the safety compliance and performance of self-performed and sub-contractor performed work and the offeror’s method for qualifying, evaluating, selecting all tiers of sub-contractors. The narrative shall include:

The offeror shall submit the following information:

(1) Experience Modification Rate (EMR); include five previous calendar years’ worth of data [2018, 2017, 2016, 2015, 2014]. This rate compares company’s annual losses in insurance claims against its policy premiums. The offeror may submit an insurance company-provided equivalent rate if no EMR exists. If the offeror has no EMR or premium, for any year, affirmatively state so and explain why. Any extenuating circumstances that affect the EMR and negative trends should be addressed as part of the factor submission.

(2) OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) and Total Recordable…
