NRL Visitor COVID-19 Vaccination Procedures
Purpose: To communicate procedures regarding NRL Visitor COVID-19 Vaccination requirements.
Background: The relevant government guidance regarding Visitor COVID-19 vaccination requirements has been thoroughly discussed and the Commanding Officer has approved the following procedures.
Official Onsite Visitors:
1. Official onsite visitors will complete DD Form 3150, “Contractor and Visitor Certification of Vaccination;
and maintain a current completed DD Form 3150 and show it to authorized DoD personnel upon request. Failure to complete and show the DD Form 3150 to NRL Visitor Control at the time of the access request may result in denial of access. Visitor Control will provide blank DD Form 3150s to prospective visitors upon request.
2. Official visitors who are not fully vaccinated against COVID-19, or who decline to volunteer their COVID vaccination status, must show an electronic or paper copy of negative results from an FDA-authorized or approved COVID-19 test administered no earlier than 72 hours prior to their visit.
3. NRL will NOT provide COVID-19 testing to visitors.
4. Official Visitors are non-DOD individuals seeking access, one time or recurring, in association with performance of official DOD business (e.g., to attend a meeting) but do not have credentialed recurring access (CRA).
Personnel to whom the COVID vaccination requirements may not apply.
1. Personnel who will NOT enter into any buildings on the facility during the course of their visit will NOT be required to complete DD Form 3150 (e.g., delivery or taxi personnel).
2. Personnel who may need to access a building for any purpose (e.g., groundskeeper accessing bathroom) will follow the same requirements as Official Onsite Visitors.
Implementation and effective date: December 16, 2021
SUBJECT: Mandatory Certification of COVID-19 Vaccination Status for Visitors
Effective December 16, 2021, all visitors entering the Naval Research Laboratory (NRL) facilities will be required to present a completed DD Form 3150 "VISITOR CERTIFICATION OF VACCINATION."
Upon arrival at an NRL facility, visitors must present a completed and signed DD form 3150 indicating their current vaccination status:
• Fully Vaccinated Visitors are considered "fully vaccinated" two weeks after completing the second dose of a two-dose COVID-19 vaccine (e.g., Pfizer-BioNTECH or Moderna) or two weeks after receiving single dose of a one-dose vaccine ( e.g., Johnson & Johnson/Janssen).
• Not Yet Fully Vaccinated Visitors who received their first dose of Moderna or Pfizer and the second is scheduled, or visitors who received their final dose less than two weeks prior to the visit.
• Not Vaccinated Visitors who have not received any COVID-19 vaccinations
• Decline to Respond
Any visitor who indicates they are Not Yet Fully Vaccinated, Not Vaccinated, or Declines to Respond will, in addition to presenting the DD 3150, be required to provide a valid negative COVID-19 test result dated no more than 72 hours prior to the visit or they will be denied entry.
