Instructions **NOTE: This tab contains instructions for completing Tables ES-1 and ES-2** Instructions for the ES-1 Offeror Summary Table
Row 3 Fill in the Offeror's Name. "Offeror" is defined as the Prime Contractor with its CAGE code identified in Block 15A on Standard Form (SF)-33, Solicitation, Offer, and Award.
Column A List all entities who will be involved with performance or management of the requirements of this solicitation. The term “entity” is used to mean any or all of the following: Prime, JV, JV Team Member, any subcontractors, corporate parent, division, subsidiary and/or affliliate. Add rows as necessary.
Column B List all Places of Performance, being careful to include accurate physical locations.
Note: Place of Performance for this solicitation pertains to the physical location of the entity that is deploying personnel or responsible for the work; not the Government site where the work is being performed. If work is being performed at a contractor site, then that would also pertain to Place of Performance.
Column C Provide the CAGE Codes/DUNS numbers associated with the entity that is proposing to perform the work for this solicitation.
Note: Past performance of any entity with multiple CAGE codes/DUNS will be considered for the same physical location if they have distinct roles and responsibilities or the current and previous CAGE codes were created as a result of a corporate acquisition, reorganization merger, or other administrative reason and refer to the same division in the same physical location for where the work is being performed or from where the work is being directed in accordance with the proposed roles and responsibilities. If multiple CAGE codes/DUNS for the same physical location will be involved with performance or management of the requirements of this solicitation, ensure they are identified on separate lines in Table ES-1 (Offeror Summary).
Column D Provide a brief Work Description and/or Program Role and Responsibility for each CAGE Code/DUNs identified.
For Example: Cage U18A6: As Prime Contractor, responsible for providing (or managing subcontractors that provide O- and I-level aircraft maintenance (scheduled and unscheduled), aircraft launch and recovery operations, aircraft detachment support, engine and propeller overhaul and repair, on-site component repair and supply support for T-34/44/6 program at NAS Pensacola, FL, NAS Whiting Field, FL, and NAS Corpus Christi, TX.
Note: Provide a definition of the legal relationship between the entities if it is other than a Prime/subcontractor relationship.
Column E Provide the percentage of total proposed price (minus Parts & Material CLIN costs) being careful to include all entities so that the total proposed price adds up to 100% of the contract price. If there are current and previous CAGE codes/DUNS as a result of a corporate acquisition, reorganization, merger, or other administrative reason, that refer to the same division in the same physical location for where the work is being performed or from where the work is being directed place N/A in this column and ensure the proposed price/work for this effort is covered under the current CAGE codes/DUNs.
