Use/relationship:
Airframe Maintenance History Summary Report provides a detailed description of all maintenance performed on inducted aircraft at a repair facility. The information in the report will be retained as a permanent part of the maintenance records of the individual aircraft.
This Data Item Description (DID) contains format and content preparation instructions for the data product generated by the specific and discrete task requirement as delineated in the contract and described in the Performance Work Statement (PWS).
This DID is for One-Time use for solicitation N00019-10-R-0039.
Requirements:
1. Format. Report to be in accordance with Section V of the SDLM Spec (Attachment 4) for PMI 1, PMI 2, PMI 3, PMI 4, Phase Un-Scheduled maintenance, and modifications. The format may be of the contractors choosing, but must be mutually agreed upon by the government and the contractor.
2. Content. The report shall include:
1. Aircraft Type/Model/Series
2. Aircraft Bureau Number
3. PMI Number
4. Total flight hours and landings since new and last rework
5. Number of extension and total months extended
6. Operating months during last service period and total since new
7. Date aircraft was inducted into rework
8. Date rework was completed.
9. Number of days aircraft rework was in process.
10. Technical Directives Incorporated/complied with by number and title.
11. Reporting Activity (Contractor performing rework)
12. Contractor point of contact, name and telephone number for report.
13. Inspection Results and Corrective Action Details:
(a) Task Identification (ID) Number 1) Assigned Tasks - This is a unique reference for each inspection requirement and is located in the far left-hand column of the Structural-Zonal and System-Component Requirement listing located in Section II of this specification.
2) Other Defects - For defects discovered outside of the assigned inspections in section II of this specification, the contractor shall assign task numbers sequentially as follows: C001, C002, C003, etc. These tasks and other required information shall be listed following the “Assigned Task” discussed above.
(b) Part Number/Item Nomenclature - This is the part number and nomenclature of the component or part as listed in the IPC.
(c) Defect Class Code - Codes to identify category of defect(s) found as defined in paragraph 2-2.4 of this specification and as listed in the heading on the Maintenance History Summary Report form.
(d) Description of Defect - Provide complete information on defect(s) found. Defect information is to include but not limited to the following:
1) Type of Defect - crack, corrosion, deformation, wear, etc.
2) Defect Location – describes location of the defect.
3) Detail Description - provide sufficient detail to accurately describe defects such as:
4) Cracks - provide length, depth, location.
5) Corrosion - provide type, depth, category (severity), extent (see Table II and III, pages 5-18 and 5-19 of this specification for assistance in developing verbal description)
6) Wear - depth and extent
(e) Description of Corrective Action - Provide a thorough description of corrective action accomplished to include listing the document number(s) and page number(s) from which corrective action was taken.
(f) Inspection Man-hours - Provide actual manhours required to accomplish inspection.
(g) Corrective Action Man-hours - Provide actual manhours required to accomplish corrective action.
(h) DF - Defects found. This field is used when more than one defect is found under one task. Sequentially number each defect with the same task number.

(14) Total Manhours:
(a) Defect - Total manhours required to correct defects while aircraft is undergoing PMI-1, PMI-2, PMI-3 or PMI-4.
(b) Inspection - Total manhours required to perform inspection tasks while aircraft is undergoing PMI-1, PMI-2, PMI-3 or PMI-4.
(c) Total - Sum of manhours in paragraphs (14)(a) and (14)(b) above.
(15) Date: Date aircraft MHS Report was completed.
(16) Page: Sequential number assigned to a specific page.
(17) Remarks: Pertinent maintenance information that does not fit any of the MHS requirement categories above is to be recorded in this paragraph. Examples are provided below:
(a) Special Work Request - Describe work accomplished from squadron work request accompanying the aircraft for induction into PMI-1, PMI-2, PMI-3 or PMI-4.
(b) Interval Change Recommendations to Inspection Tasks Listed in the PMI-1 through PMI-4 Specification - Interval(s) of Inspection Task(s) may need to be changed due to defects discovered or other pertinent factors. Request contact or provide Task ID number; recommended interval and technical justification.
(c) Add or Delete Inspection Task(s) - Damage discovered to item(s) peripheral to a listed inspection task item or discovery of unrecorded modification(s) (terminating actions may be justification for adding inspection requirements).. Request contractor provide specific recommendation(s) and technical justification.
(d) Aging Aircraft - Provide details of inspections performed and any defects found, accomplished, terminating modifications, etc.
(e) Incoming Discrepancies - Provide details of incoming discrepancy defects and any corrective action performed.
(f) Components that are Replaced or Overhauled - Provide details on defect, if applicable, and part numbers or serial numbers.
(g) Other Directives Accomplished - Please provide detailed descriptions as well as findings for any directives that are accomplished during PMI-1, PMI-2, PMI-3 or PMI-4; i.e., inspected in accordance with Revision 2 Option I Phase III.
(h) Miscellaneous - Record any other pertinent PMI-1 through PMI-4 maintenance information related to C-9B aircraft.
(18) Results of all Non-Destructive Inspections (NDIs) accomplished.
(19) A listing of all Noted But Not Corrected (NBNC) discrepancies.
(20) All corrosion found during inspections and the resulting actions taken.
