About this file

This document contains a past performance questionnaire template for offerors responding to solicitation N00024-20-R-4408, issued by the Naval Sea Systems Command for multiple award contracts to provide sustainment services for Littoral Combat Ships on the East Coast. The solicitation seeks proposals for drydocking selected restricted availabilities, selected restricted availabilities, continuous maintenance availabilities, unscheduled maintenance, and emergent and OCONUS maintenance under three MAC constructs. MAC III is anticipated as a small business set-aside for preventative/planned maintenance availabilities, facility maintenance, and corrosion control CONUS requirements. All MAC-IDIQ contracts will have a five year ordering period. Offerors must submit the past performance questionnaire with supporting documentation by the proposal due date to be evaluated. The questionnaire collects information on up to five relevant past performance examples, including contract details, customer references, cost and schedule performance, technical success, and a customer satisfaction assessment.
