About this file
This is a Division of Work Listing Worksheet template for prime and subcontractors to indicate their areas of responsibility across seven main Statement of Work (SOW) sections for the Human Health and Performance Contract 2 (HHPC2) at NASA Johnson Space Center. The worksheet breaks down the work into detailed subsections: Contract Management and Administration (15 areas), Medical/Health/Laboratory Services (3 areas), Labs & Facilities (2 areas), Operations and Research Capabilities (32 areas covering topics like behavioral health, radiation monitoring, biomedical engineering, and various laboratory disciplines), Life Cycle Development (6 areas), Directorate Services (9 areas), and External Collaborations (2 areas).
The template requires offerors to identify all subcontractors by name, address, size classification (small business, woman-owned, etc.), CAGE code, and subcontract value. Companies must mark which SOW sections each entity will support by placing an "X" in the corresponding boxes, ensuring all SOW sections have either prime or subcontractor coverage. This document is designed to provide NASA with a clear understanding of work distribution among the prime contractor and its subcontractors.

Instructions:
List prime contractor(s) and all subcontractors down the left. Place an x in each box associated with each SOW section for which each company will be performing the work.
The Offeror shall provide a summary listing (by name and address) of all subcontractors (regardless of dollar value) that have been identified throughout the Offeror’s proposal, the subcontract value associated with each entity, and the SOW sections that each subcontractor proposes to support. This listing shall also include the subcontractors size classification (i.e., Small business (SB), Woman Owned Small Business (WOSB), etc.) and cage code. Fill in so all SOW sections have a Prime and/or subcontractor associated with accomplishing that requirement.
