About this file

This is a Past Performance Questionnaire (PPQ) template for the Carrier Engineering Maintenance Assist Team (CEMAT) solicitation (N00024-26-R-4114) issued by Naval Sea Systems Command. The questionnaire is completed by customer references to evaluate contractor past performance and must be submitted directly to the Government by August 10, 2026, rather than provided by the offeror. References are identified by the offeror in Section A, which captures contract details including contract number, project title, period of performance, customer point of contact information, contract type, and description of supplies or services provided.

The questionnaire evaluates contractors across six performance criteria using a five-point adjectival rating scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory). The evaluation categories are: (1) Quality—ability to complete tasks conforming to customer requirements and workmanship standards; (2) Schedule Performance—ability to develop accurate schedules and adapt to requirement changes; (3) Cost Performance—ability to forecast and control costs; (4) Management—ability to overcome technical challenges using sound engineering principles and develop adequate technical reports; (5) Small Business Subcontracting—compliance with subcontracting plan goals for small, disadvantaged, women-owned, HUBZone, veteran-owned, and service-disabled veteran-owned businesses; and (6) Regulatory Compliance—adherence to applicable laws and regulations including financial, environmental, safety, labor, and reporting requirements. Evaluators must provide substantiating comments for each rating and submit completed questionnaires to Mr. Tyler Pacak at Naval Sea Systems Command.
