About this file

This document is a Bid Cover Sheet for Invitation to Bid (ITB) ITBS-63500-00013659 issued by DAS Procurement Services for the Clackamas Complex Building 15 Painting project in Oregon. The document serves as a formal certification and acknowledgment form that bidders must complete when submitting a proposal, with the version dated February 20, 2019. While specific project details are not fully elaborated in this attachment, the document indicates it is related to a painting project for Building 15 at the Clackamas Complex.

The bid cover sheet requires bidders to certify multiple compliance aspects, including non-discrimination practices, tax law adherence, absence of conflicts of interest, and confirmation of not being on excluded parties lists. Notable requirements include having a written policy preventing sexual harassment if the contract anticipated price is $150,000 or more, not having undisclosed debt to the state, and preparing the bid independently without collusion. Bidders must also verify they are not on government sanctions lists such as the Specially Designated Nationals list maintained by the U.S. Treasury and the System for Award Management exclusion lists.
