About this file
This is a Present and Past Performance Questionnaire (PPQ) template for the Total Maintenance Contract for DoDEA facilities in Okinawa, Japan (Solicitation HE1254-26-R-E003). The questionnaire is designed to collect and evaluate contractor past performance information from the offeror's references and customers.
The document contains two primary sections. The first section, completed by the offeror, captures contractor information including firm name, CAGE code, UEI, point of contact details, and work performed classification (prime, subcontractor, joint venture, or other). It also requires identification of the reference customer, contract details, period of performance, final contract value, contract type, and a brief description of the effort with relevance to solicitation requirements. The second section, completed by the reference customer, evaluates the contractor across six performance categories using a standardized rating scale: Poor/Unsatisfactory (P/U), Satisfactory (S), Good (G), Very Good (VG), Excellent (E), and Neutral/N/A (N). The evaluation criteria address compliance with delivery schedules and performance milestones, business practices and ethics, conformance to contract requirements and workmanship standards, overall contract compliance, customer commitment and satisfaction, and overall performance. The questionnaire also includes a binary question regarding willingness to purchase services from the contractor again and space for additional comments. Completed questionnaires must be returned via email to the Department of Defense Education Activity, Contract Specialist Emani Gray, at emani.gray@dodea.edu.
