About this file
This document is an instructional guideline for hazardous material reporting procedures for construction projects at Tinker Air Force Base. The contractor and Quality Assurance Inspector must submit hazardous material Safety Data Sheets (SDS) or Material Safety Data Sheets (MSDS) directly to Tinker AFB environmental personnel—Tammy Detrixhe (tammy.detrixhe@us.af.mil) and Kandice Sanders (kandice.sanders@us.af.mil)—via email for verification and entry into the base's hazardous material database.
Per Tinker AFB specification 00 72 00 (Environmental Requirements for Construction on Tinker Air Force Base), contractors must complete and submit an AF3952 form along with an AF3000 form to the Contracting Officer/QA Inspector for review by environmental hazardous material management personnel. Additionally, contractors must add a line item to their AF66 submittal register documenting the hazardous materials being utilized on the project and resubmit the updated AF66 with a new AF3000 to the QA Inspector if the hazardous materials were not included on the original submittal.

Hazardous Material Reporting for Construction Projects on Tinker AFB
The contractor/QA Inspector shall contact and send all hazardous material SDS/MSDS information directly to the Tinker AFB environmental personnel (Tammy Detrixhe -tammy.detrixhe@us.af.mil and Kandice Sanders -kandice.sanders@us.af.mil) in an email, so that they can verify and confirm that it’s been entered into Tinker AFB’s hazardous material database.
Also, according to the Tinker AFB specification 00 72 00 - ENVIRONMENTAL REQUIREMENTS FOR CONSTRUCTION ON TINKER AIR FORCE BASE, on pg. 14 section 1.4 HAZARDOUS MATERIAL REPORTING, the construction contractor is to fill out an AF3952 form and submit it, along with an AF3000 directly to the Contracting Officer/QA Inspector to be reviewed by the appropriate environmental hazardous material management personnel and processed accordingly. The contractor will also need to add a line item to their AF66 (submittal register) pertaining to the hazardous materials being utilized on the project, and then resubmit the AF66 with a new AF3000 to the QA Inspector to be processed, if it is not already on the original AF66.
