About this file
This is a Certification Regarding Debarment, Suspension and Other Responsibility form required for prime contractors submitting proposals to the State of Florida for RFP 26-24-CW, a re-advertisement solicitation for the elevation of flood prone homes. The project involves eleven single-family structures requiring elevation work, with respondents permitted to propose any number of structures from one to all eleven that they can complete within the required timeline. No preference is given to respondents bidding on all properties.
The certification form requires prospective participants to verify that they and their principals are not presently debarred, suspended, proposed for debarment, or excluded from covered transactions by any Federal department or agency. Contractors must affirm they have not been convicted of or had civil judgements rendered against them for fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property within the three-year period preceding the proposal. Additionally, prospective participants must certify they are not currently indicted or criminally or civilly charged with any such offenses and have not had public transactions terminated for cause or default within the preceding three years. Any prospective participant unable to certify to these statements must attach a written explanation to their proposal submission.
