**When filled in this document is Source Selection Sensitive IAW FAR 2.101 and FAR 3.104**

The 412th Contracting Squadron (AFTC/PZIOA), Edwards AFB, CA is conducting a source selection for the Base Supply Services contract. Please complete and return this questionnaire within 5 workdays. Handwritten responses are sufficient. Do not send a copy of the completed questionnaire to the contractor being assessed. You may be contacted by our office to review the results of the questionnaire. The questionnaire information will be used in the evaluation of the Past Performance Factor contained in the Instructions to Offerors of the solicitation. Completed questionnaires will become part of the official source selection records and protected in accordance with FAR 3.104 and 2.101. Performance meets/met contractual requirements with many exceeded to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with no more than a few minor problems for which corrective actions taken by the contractor were highly effective.
