About this file
This is a Past Performance Rating Form template used as part of RFP FA9101-26-R-B011 for the FARM III contract. The form is source selection sensitive information and serves as a standardized evaluation tool for assessing contractor past performance on relevant projects.
The form is completed in two sections. Offerors provide project identification information including contractor name, contract number, order number (if applicable), project title, and total period of performance. Raters—typically government customer representatives or agency officials—complete the reference information section with their name, title, agency/customer affiliation, phone number, and email address. The rater evaluates six specific performance criteria, each with five possible adjectival ratings: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory. The six evaluation criteria are: (1) Quality, (2) Schedule, (3) Cost Control, (4) Management, (5) Small Business Subcontracting, and (6) Regulatory. Each criterion includes detailed definitions for every rating level. Raters must select one rating per criterion by checking the corresponding box. For any Marginal or Unsatisfactory ratings, raters must provide additional comments substantiating the rating. If a criterion cannot be rated, raters must note "N/A" in the comments section. The completed form is returned to the offeror for inclusion in their proposal submission to the government in accordance with the RFP instructions.

RATING Adjectival Rating
Definition
Exceptional Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective.
Very Good Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the Contractor were effective.
Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear, or were, satisfactory.
Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor's proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the Contractor's corrective actions appear or were ineffective.
