RFI #4 6 Feb 2017

1. Question: If bid bond has already been submitted based on the 7 Feb 17 due date, is a new bond required due to the revised due date?

Answer: A revised Bid Bond is not necessary.

2. Question: I noticed the SOW and the RFI mention an existing Daiken system. Do you have any information on this unit or associated ductwork? Is this unit in our area of work?

Answer:

3. Question: What will the dry and wet respirator rooms be used for? Is there any machinery in there?

Answer:

4. Question: Page 43 of the Solicitation - item b. –“ Preliminary sketches/drawings, plant or equipment layouts, and catalog cuts/brochures for Comprehensive interior design (CIDs), SOW” Please expand the description of “catalog cuts”. Are not catalog cuts part of the submission process which would take place after the IFC drawings are approved?

Answer: No, catalog cuts are a part of 10% submittal for proposed products to be included in design.

5. Question: Patriot Comm requests to have the due date for the proposals extended a minimum of two weeks. Several of our subcontractors including our design firm suffered personal and business damage during the last two rounds of tornedos. Some have not in some cases had electricity and internet for long periods of time. This has greatly hampered their ability to respond to the RFP.

Answer: Amendment forthcoming. Adjustment to the due date will be included.

6. Question: In the Answers to RFIs released on 20 January 2017, the answer to #59 references “attached AutoCAD files.” There were no AutoCAD files released for Building 140. Please advise.

Answer: The as-builts referred to are the two files named: “YKAA-FP0001 Bldg 140 Main Floor Plan.dwg and YKAA-FP0002 Bldg 140 Mezzanine.dwg”. These files should contain what you need.

7. Question: In building 149 the drawings do not indicate a lighting fixture type to be used. Would the government like U-SMC to provide LED lighting fixtures for these new spaces as an added value to the government or use the type T5 fluorescent fixture as noted in the SOW for Building 140?

Answer: Refer to SOW paragraph 2.B.3 sentence reads “,emergency wall lights, 2’ X 4’ 3-lamp T8 interior lights on motion sensor surface mounted and ceiling grid mounted,” and RFI #1 posted on FedBizOpps As-Builts Drawings sheet E-103 for existing lights/schedule, and Base Facility Standards Part 6B Interior Lighting.

8. Question: What is the function of the Dry Respirator Cleaning and the Wet Respirator Cleaning? GOVT employees to clean respirators. Are there any special electrical requirements for these spaces?

Answer: No

9. Question: Reference Project UHHZ160082 (Repair/Upgrade Existing Locker Room in Bldg 149), are we to connect the existing emergency showers to sanitary sewer?

Answer: No.

10. Question: Please confirm that Volume I as I understand it consists of:

Answer: Volume I, Completed RFP will consist of the completed and signed RFP with a cover letter delineating any exceptions taken to the RFP terms and conditions with accompanying rationale. However, offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined unacceptable and therefore not considered eligible for award. Offerors shall ensure that all clauses and provisions that require “fill-in” information are appropriately completed, including the proposed prices associated with the contract line items in the Schedule of the RFP.

a. Question: Cover Letter outlining any objections to the contract language. - Answer: YES

b. Question: Completed and Signed SF1442 Answer: YES

c. Question: Matrix (Attachment 37) Answer: YES

d. Question: TCIR/DART Rates:

Answer: YES

e. Question: Tax Liability Form (Attachment 19) Answer: YES

11. Question: When you refer to the Schedule of the RFP what are you referring to?

Answer: This is an error. You will not be submitted proposed prices in the Schedule of the RFP. Per L-900 you prices will be submitted on the Atch #16.

12. Question: Does "Completed RFP" refer to the Proposal in its entirety?

Answer: Completed RFP is referring to a completed/signed back of SF1442. Also note that amendments need to acknowledged. Also the cover letter outlining any objections to the contract language is applicable.

13. Question: Please clarify that Representations and Certifications will be required at "Post Award" 52.219-08 and not included in the Proposal. (Page 20 of 50).

Answer: That clause pertains to Post Award action. Your SAM registration should include Representation and Certifications and will be verified upon receipt of proposal.

14. Question: A wipe test is required for each sampling location. Sampling locations are where new attachments will be made to existing structural, mechanical and electrical systems. The analysis of each wipe test is expensive. Can you specify the expected number of sampling locations?

Answer: “Existing structural, mechanical and electrical systems” is broad and could produce many sampling locations. As directed by SOW paragraph 4.G. Environmental Considerations.

15. Question: According to our Environmental Engineers, hexavalent chromium cannot be measured by a wipe test, and is measured by an air sample. Do you want the hexavalent chromium measured by an air sample?

Answer: “Measurement” of hexavalent chromium not in SOW. GOVT recognizes existence of “heavy metal dust” per Environmental Office report. Wipe sample identifies if “heavy metal dust” is present on existing surface and Bioenvironmental Office will determine cleanliness per SOW paragraph 4.G. Environmental Considerations.

16. Question: In order to determine what level of abatement and remediation should take place during the demo, we need to have an idea of what criteria (e.g. parts per square feet) you are using to compare the metals. Lead has a widely accepted industry standard, however, the other metals do not have an industry standard and criteria can vary widely. Can you provide those criteria?

Answer: As directed by SOW paragraph 4.G. Environmental Considerations, this will have to be coordinated with Robins Bioenvironmental Office during design phase.

17. Question: During the site visit we were told that a Hazardous Materials Report exists. If so, does this report contain an Asbestos Survey that we would have access to? Will we be required to do an Asbestos Survey prior to demo?

Answer: No asbestos report exists for Bldg 140 which was constructed when ACM allowed, i.e. existing 9” x 9” tile and mastic in existing Engraving Shop should assumed to have ACM and be removed per SOW paragraph 2.B.3.

18. Question: Provide a table of contents (TOC) showing Division 1 specifications needed and a listing of the other Divisions (just Division, not each required section within the Division), as well as the edited Division 1 sections required for the project?

Answer: "TOC of all Division 1 specs and all UFGS divisions to be utilized on each Design-Build project"

a. Question: And one more clarification, just to make sure we are talking about the same thing for Division vs Section. To me, Division is just the very broad category of work. For instance Division 08 - Openings. That doesn't define whether the openings are hollow metal doors, or roll-up garage doors, or aluminum curtainwall. To get into what type of products would be used in a project, I would show Sections within the Division.

Showing Sections gives some useful information about what is going into the project and becomes the basis for building a submittal schedule or register, so I'm used to showing needed sections right at the start of a project. But, if you are not interested in that for the proposal, I can certainly leave the sections out. I'm just wanting to understand what you would like to see.

Answer: Your understanding is correct.

b. Question: My understanding of the SOW wording is that we are to show Divisions and Sections for Division 1, but just Divisions for the rest. That is correct?

Answer: Request Specification Divisions and individual Sections within each Division for both Design-Build projects.

19. Question: Refer to Attachment 4 – Robins AFB Division 1 Specs and Forms, Index of Specifications. In this Index of Specifications template is states after division 1 for the designer to use UFGS specs from this point down. However, once we get down to Division 26 Electrical there are several Division 16 specifications listed. These do not match up with the standard UFGS spec designations. Are there pre-edited RAFB electrical specifications that we are to choose from? Please clarify what we are utilize here, UFGS or some RAFB standard electrical specs??

Answer: Per SOW, utilize UFGS specifications for everything other than Division 1.

20. Question: Refer to L-900 Instructions to Offerors of the Solicitation, Part D., Sub-Factor 1 – Completeness of the Demonstration Projects. For projects UHHZ 160049 and 160082 it tells us to provide a 10% design and to see CEMACC SOW paragraph 1.4.1. When reviewing CEMACC SOW paragraph 1.4.1.3 for Specifications it tells us to “provide an outlined list of proposed UFGS individual Divisions to be used” as part of the 10% submittal. Further down under 1.4.3.3 under 30% design requirements it tells us that the Division 01 sections provided by RAFB are not required at this stage. But when you refer to the project specific SOW, under Part 2.B.1 it tells us to provide the specification table of contents to include 10% edited Robins Division 1 specifications, which is in conflict with what CEMACC SOW Part 1.4.1 instructs us to do. Please clarify if the Robins Division 1 specs are to be included in this 10% submission and if so is it just in Table of Content form.

Answer: Provide Table of Contents only that shows all Specification Divisions and individual Sections within each Division for both Design-Build projects. DO NOT EDIT Robins DIVISION 1 SPECS.

21. Question: Referring to the response to RFI #1 regarding LEED requirements, the response stated, “Consideration to meet Certified 30-37 points for Industrial projects…”

a. Please provide a LEED scorecard for potential credits required/desired. Answer: Refer to Base Facility Standards for LEED Industrial projects.

b. Please confirm LEED certification or registration is NOT required.

Answer: Correct

c. Please confirm no LEED documentation will be required to be submitted.

Answer: Refer to Base Facility Standards for LEED Industrial projects.

22. Question: Referring to the response to RFI #4, regarding access to the mezzanine above CMM Lab for inspection during site visit, participants were instructed on where they could and could not go, and access was not provided to the mezzanine by the Government. We require knowing what type of unit and the specs of the unit in order to complete 10% design.

Answer: Existing unit to be replaced on the mezzanine is “Trane Climate Changer”.

23. Question: It was observed during the site visit that Building 140, there is a substantial amount of existing equipment and office furniture in the existing CMM Lab and existing Engraving shop. Will this equipment be relocated by the owner prior to construction?

Answer: If existing governmnet equipment is within spaces to be renovated at Bldg 140 and Bldg 149 projects, it will be moved, stored and reinstalled by the government.

24. Question: It was observed during the site visit that Building 149, there is a substantial amount of existing equipment and office furniture in the work area. Will this equipment be relocated by the owner prior to construction?

Answer: See response above.

25. Question: Regarding existing equipment, to be relocated, as referenced in RFI #13, who is responsible for relocation?

Answer: See response to question above. Disregard response to question #13 in RFI #2 posted on fedbizopps 20 Jan 2017.

26. Question: Regarding RFI #13, and the existing pop-up ceiling – please see photo. At the existing piece of equipment, the ceiling profile varies in height to accommodate the piece of equipment. Our question is if the new CMM Lab ceiling needs to match the existing ceiling as shown in the picture below?

Answer: new 2’X2’ acoustical ceiling tile height to be 9’-6” above finish floor.

27. Question: Will separate Superintendent, Quality Control, and Safety personnel be required at each demonstration project site, or will the personnel be able to share roles across the three projects?

Answer: Per CEMACC SOW paragraph 1.28 Quality Control, Quality Control responsibilities, program and records are required.

28. Question: What are the government’s site specific personnel requirements for supervision, safety, and quality control at demonstration project UHHZ 160082 Repair/Upgrade Locker Room Bldg 149?

Answer: : Per CEMACC SOW paragraph 1.28 Quality Control, Quality Control responsibilities, program and records are required.

29. Question: What are the government’s site specific personnel requirements for supervision, safety, and quality control at demonstration project UHHZ 150376 Construct/Install C-17 Fabric Shelter at N-27?

Answer: : Per CEMACC SOW paragraph 1.28 Quality Control, Quality Control responsibilities, program and records are required.

30. Question: What are the government’s site specific personnel requirements for supervision, safety, and quality control at demonstration project UHHZ 160049 Repair/Renovate Interior Room 17 CMM Lab Building 140?

Answer: : Per CEMACC SOW paragraph 1.28 Quality Control, Quality Control responsibilities, program and records are required.

31. Question: Please provide Hazardous Material Survey for Building 140 and Building 149. If one is not available, will the D/B contractor be required to hire and have one performed prior to design/construction?

Answer: Refer to SOW paragraph 4.G Environmental Considerations for UHHZ 160049. There is no hazmat in Bldg 149.

32. Question: Due to existing number of unanswered RFIs and forthcoming, will the response date be postponed?

Answer: Amendment 0001 posted due date to 21 Feb 17 4:00 PM EST.

33. Question: A submittal register cannot be provided at the 10% design phase, because the specifications have not been edited at the 10% design phase. If a submittal register is generated from SpecsIntact at this stage it would be 100+ pages. Since you have capped the requirements of the Tab A factor to 60 pages, we cannot provide a submittal register 1.) because it alone would exceed the page count and 2.) because the specs are not edited at this phase in design and the submittal requirements would not be accurate whatsoever. Please remove the requirement of a submittal register – it is apparent that Robins AFB does not require a submittal register to be submitted until the 100% Design Submission per 1.4.5.4 in Attachment #1 CEMACC SOW.

Answer: Refer to individual demonstration project SOW paragraph 2 for requirements.

34. Question: At Repair/Upgrade Existing Locker Room in Bldg 149, SOW references a pad mounted Daiken unit is to remain. Please provide a photo of the existing unit and/or the unit serial number for further investigation.

Answer: Photo to be provided.

35. Question: RFI Response 71 states that no design is required for UHHZ 150376. Who is responsible for designing the electrical load requirements, lighting requirements, panel requirements, feed requirements, utility pit tie-in requirements, etc.?.

Answer: The Government contract will be with the successful offeror to provide a complete and usable facility.

36. Question: Is a site plan, haul route, etc. required for the UHHZ 150376 project?

Answer: Refer to SOW Paragraph 2.B.111.

Civil

37. Question: Will there be an as-built, updated survey of the existing addition at Bldg 149 for completion of 100% Final Design Package provided by the Owner, or is it to be included in the Civil design fee?

Answer: Refer to fedbizopps RFI #1 posting for only information available. If offeror requires additional information, it will at their costs.

Architectural

38. Question: Are construction contract administration services to be provided by the A/E on the design-build team, and priced into the proposal (i.e.: shop dwg review, OAC meetings, periodic evaluations of the work, etc)? The Statement of Work indicates 100% of the design fee is to be paid at Corrected Final Drawings.

Answer: Refer to individual demonstration projects SOW for AE services required and payments which are made based on submittal percentages.

Structural

39. Question: The fabric shelter does not have a 10% design requirement. Is anything required to be submitted on this building at the 10% stage?

Answer: Refer to fedbizopps attachment #16 Pricing Sheet construction costs requirements.

40. Question: Is the fabric shelter a design-build task order? Are no A/E design team services or design cost component to be included in the proposal?

Answer: Refer to fedbizopps attachment #16 Pricing Sheet and individual SOW.

Mechanical:

41. Question: Is information on heat output for existing and new equipment in Building 140 available?

Answer: If both printers are operating at the same time, the anticipated equipment/printer heat load is 46,000 BTU/hr.

42. Question: Are equipment specifics including equipment type, make, and model number available for all equipment in the existing CMM Lab and proposed new Room available (all of Building 140)?

Answer: Equipment is GOVT furnished GOVT installed.

43. Question: Please provide building Dimensions for Building 140. Existing Condition AutoCAD files of Bldg 140 main floor plan and mezzanine floor plan are only files available.

Answer: No guarantees to accuracy of existing AutoCAD files

44. Question: Please provide size and location of Mezzanine for Building 140. See response above.Scope for Building 149 calls for Humidification and Dehumidification... Is Humidification truly required? The RFI call for wall mounted units which do not have an option for humidification Circuiting, just dehumidification.
Answer: Refer to SOW for requirements.

45. Question: Existing radiant heaters in Building 149 are shown to be Electric Infrared Heaters on existing drawings, but the scope of work refers to existing Gas Fired Infrared Heaters. Please clarify heat source.

Answer: Existing infrared heaters are electric per as-builts.

46. Question: RFI responses state that compressed air lines in Building 140 are to be tied into existing. Please provide location and deliver pressure of existing lines.

Answer: Existing lines adjacent to CMM Lab mezzanine.

47. Question: Are the new HVAC Systems for Building 140 assumed to condition the restrooms as well? Are the existing exhaust systems for the restrooms to remain or be replaced?

Answer: Per SOW, no restroom work.

48. Question: Attachment 4, Index of Specifications lists Division 16 specifications under the Division 26 section for electrical work. This seems to conflict with Attachment 34, paragraph 1.1.3, which says use UFGS for all Technical Sections. UFGS no longer uses Division 16 for electrical technical sections. Will the designer use UFGS Division 26 for electrical technical sections, or is there some other source for Division 16 technical sections?

Answer: No AE design required from GOVT standpoint.

49. Question: Attachment 36, UHHZ 160049 Statement of Work, paragraph 4, F, 1,a states: Look at loading of main transformer. If not sufficient for the new total load, replace it and the main switchboard. Can the government provide a 13-month demand history for the transformer and more information about the characteristics of the switchboard and the service conductors for this building. Can the government provide more information about the electrical requirements of any additional equipment to be installed in the lab expansion? Without this information, it is not possible to make a determination regarding their sufficiency. For the purposes of this proposal, would it be acceptable to consider it to be sufficient, and evaluate it after award? Disregard requirement concerning “look at loading of main transformer”.

Answer: Per SOW paragraph 2.B.3., new transformer will be provided.

50. Question: Attachment 36, UHHZ 160049 Statement of Work, paragraph 2, B, 1 requires that 10% concept design package shows connection location to existing power source inside Bldg 140 paragraph 2,B,3 references a bus duct for connection of the HVAC unit. Is it the government's intent that the new 120/208V transformer also be fed from a new busway switch?

Answer: Refer to SOW paragraph 2.B.3., for new transformer.

51. Question: RFI's_2_20-Jan-2017 indicates that two HVAC units will be required for the building. Should the second HVAC unit also be fed from a new busway switch?

Answer: Bldg 149 project will have two new units not Bldg 140 project.

52. Question: Solicitation; Page 4 – Bond Requirements, paragraph A states…”Each offeror shall submit a bid bond (Standard Form 24) in the amount of $2,800,000. This amount was established by taking 20% of the estimated value each year of contract. Total amount to be obligated against this contract is $70 million, therefore estimated yearly amount is $14 Million; twenty (20%) percent is $2,800,000.”

a. Please clarify whether the intent of this statement is to require a $2,800,000 bid bond with a penal sum of $560,000, or a $14,000,000 bid bond with a penal sum of $2,800,000?

Answer: Bid bond in amount of $2.8M with corresponding penal sum.

b. If the former, does the bid bond expire with the award of the ‘seed projects’ or 180 calendar days?

Answer: Payment and performance bonds will be required at the task order level for projects exceeding $30,000. Additional bid bonds will not typically be required at the task order level.

53. Question: Can photographs be provided of the interior of this space?

Answer: Yes, photos of both Bldg 140 and Bldg 149 will be posted on fedbizopps.

54. Question: What is the intent for the award of the 3 seed projects? Each project individually to 3 distinct Awardees or could multiple projects/all projects be awarded to one Awardee?

Answer: The government may elect to award these projects as minimum guarantee projects if it is determined in the best interest of the Government.

57. Question: We have asked a question regarding the submittal register multiple times and have yet received any additional information or a response to our questions.

There is a conflict in the Individual Demonstration Project Scope of Work with the CEMACC SOW. In the CEMACC SOW, the Submittal Register is not required for submission until the 100% Design, because it cannot be generated until the specifications are edited – it is physically impossible. Since the RFP requires the use SpecsInTact to generate the specs, it is also used to create the Submittal Register. If the Submittal Register is not required to be submitted until 100% Design per base standards in the CEMACC scope of work section 1.4.1, because at that point the specs are finalized and it can be generated, why is the individual scope of work for demonstration projects requiring one?

Answer: The GOVT wants to see the offerors understand project requirements.

58. Question: A submittal register cannot be provided at the 10% design phase, because the specifications have not been edited at the 10% design phase. If a submittal register is generated from SpecsIntact at this stage it would be EXTREMELY long because there would be auto populated items that do not pertain to the project due to the fact that the specifications have not been edited at 10% Design Phase. For example, the unedited specification 04 20 00 Masonry requires the following submittals:

Answer: Utilizing Division 04 Masonry: Populate Form 66 to indicate masonry required on both projects with submittals for CMU, Anchors, Ties, and Bar Positioners, and Joint Reinforcement. At the 10% submittal level, this is extent needed. Also, Robins Division 1 specs has section 01300 Submittals and Contractor-Furnished Items which will be utilized versus UFGS section 01 33 00.

a. Question: As you can see, our demonstration projects do not require Salvaged Brick, Portable Panels, Clay or Shale Brick, etc. However, ALL of these would be on a submittal register generated from SpecsinTact at this phase – multiply this across twenty or thirty specification sections, and the submittal register would be extremely long and not accurate whatsoever.

Answer: See response above for extent of information needed on Form 66 at 10% submittal level.

b. Question: Furthermore, the page count has been amended to 80 pages for the total submission. The submittal registers alone could be that long. How are we supposed to submit submittal registers and stay under the 80 page count?

Answer: Form 66 should require approx. two pages plus or minus.

59. Question: Please refer to the Statement of Work for UHHZ 160306 Civil Engineering Multiple Award Construction Contract (CEMACC) Robins Air Force Base, Georgia, Section 01 11 00 – 18, paragraph 1.4.5 100% Design Submission, subparagraph 1.4.5.3, outlines the Submittal Register is to be included with the 100% specification submission. This is the Base Standard from the Office of the Base Civil Engineer. Please explain how a submittal schedule can be provided at the 10% phase (as required by the CEMACC IDIQ Statement of Work for Demonstration Projects) when the Base Standard is for it to be provided at the 100% Design Phase.

Answer: See response above.

60. Question: CEMACC RFIs 2_20-Jan 2017, Response, #64. Was answered that the expected new CMM Lab in Bldg 140 had a heat load for equipment/printer is 46,0000BTU/hr. What is the anticipated heat load for the existing CMM lab once the printer equipment is relocated?

Answer: Approximately 25,000btu/hr.

61. Question: Is the existing unit for the existing CMM Lab to remain, or is it to be removed and replaced by the new unit provided for the new CMM Lab?

Answer: Existing HVAC unit located on mezzanine will be demoed and replace with new unit to handle existing CMM Lab and new space.

64. Question: RFI 3 for FBO Post 27 Jan 2017 Q52 and Q56 have a response that the SOW makes no reference to a life cycle cost analysis. However, the response to Q66 explains why a life cycle cost analysis is desired on projects. We have no intent to provide this analysis because there are no options to evaluate. The buildings are existing and the basic system types are defined by the government already. Please confirm as stated in Q52 and Q56 a Life Cycle Cost analysis is not applicable to these specific demonstration projects.

Answer: LC cost analysis reference is in SOW paragraph 4.E.8 for UHHZ 160082 Bldg 149 and will be required.

65. Question: RFI 3 for FBO Post 27 Jan 2017, Q79 regarding Bldg 149 implies that all supply air delivered to the wet and dry respirator rooms, the locker room and the storage room cannot be recirculated. The make-up air for this would require a dedicated 100% outdoor air unit. It is a significant cost increase for the HVAC on this building. Can we get clarity on this?

Answer: The preference would be ducted return with outside air unit for both respirator rooms, locker room and storage.

66. Question: RFI 3 for FBO Post 27 Jan 2017, Q100 regarding the Fabric Shelter asked “RFI#2 Response 71 states that no design is required for UHHZ 150376. a. Answer - Per CEMACC SOW paragraph 1.28 Quality Control, Quality Control responsibilities, program, and records are required.

b. The response does NOT answer the question – 1.28 Quality Control, nor the included clause reference to FAR 52.246-12, list the PERSONNEL requirements for on-site Quality Control, Site Superintendence, and Site Safety Health Officer (SSHO).

i. What are the government requirements for on-site superintendence for each of the projects – i.e. can on person share the role of QC Manager, SSHO, and Superintendent?

Answer: Each demonstration project will require one superintendent at each project and one QC person for ALL three demonstration projects, but QC person will be required to be onsite at one of the three projects. No reference found for SSHO in SOW.

ii. In regards to the three separate demonstration projects can the on-site management staff share the role or is separate on-site management staff required for EACH demonstration project site?

Answer: See response above.

68. Question: RFI 3 for FBO Post 27 Jan 2017, Q10 asks Question - The project is a set-aside for SDVOSB. Are all SDVOSB’s required to be Certified & Verified with the CVE in order to bid and be eligible for award on this project?

a. Answer - Offerors self-certify to their small business status.

b. Typically, most projects require a CVE certification for SDVOSB’s to be awarded contracts as required and outlined by the VA https://www.va.gov/osdbu/verification/. Yes, small business can self-certify status, but the question is whether a CVE certification is required for contract award (which is the norm) and not just a self-certification (which is rare).

Answer: Offerors will self-certify. The Department of Defense (DOD) recognizes a DD form DD214 as self-certifying. Reference FAR 19.307.

69. Question: The Site Visit Presentation states to ensure offeror submits completed Tax Liability Form (RFP Attachment #19). In which section should this be included?

Answer: Include this in your Volume I.

70. Question: Are the bid bonds to be sent in separately from the proposal?

Answer: You can submit with your Proposal OR send in by itself. However, if sending by itself it must be received NLT the proposal due date.

71. Question: In the Solicitation, Sub factor 2 ‐ Program Management Plan, Item d, key quality control personnel are to be outlined (with no overlapping duties and responsibilities). This question has been asked by many and the answer has been “Per CEMACC SOW paragraph 1.28 Quality Control, Quality Control responsibilities, program, and records are required.” We know they are required, the question is, can the General Contractor's Project Superintendent also serve as Site Safety and Health Officer and/or Quality Control, or does it need to be a dedicated person with no other responsibilities? Please clarify.

Answer: Each demonstration project will require one superintendent at each project and one QC person for ALL three demonstration projects, but QC person will be required to be onsite at one of the three projects. No reference found for SSHO in SOW.

73. Question: One of the Tension Fabric Shelter vendors asked if the timeline for performance on the Construct/Install C17 Fabric Shelter project can be extended to 150 days? They feel the 90 day performance period is not enough to allow for design, fabricate, deliver and install a custom project of this size. Please advise if this timeline can be extended to 150 days?

Answer: YES - Amendment forthcoming that will revise from 90 to 150 days.

74. Question: Construct/Install C17 Fabric Shelter. In the SOW where the electrical work is described, there is some language that is confusing. According to section 2.B.II, we are to provide a 30A breaker in an existing switchboard that is adjacent to the utility pit to feed a new lighting panel located at the shelter. Per section 4.F.1.a, a new underground feeder for the new panel will need to be provided via directional bore from the pit to the shelter, but it also indicates to make “electrical connections” in the pit. Since there is no way to provide power by tapping into the conductors in the pit, my understanding of the electrical service is that it will come from the existing switchboard to the pit, and from the pit through new underground conduit to the new lighting panel at the shelter and that the “connection” refers to the new conduit and feeder. Please clarify if there is a power source in the utility pits that we tap into, or will it be required for the contractor to pull new feeds for the shelter, and if we need to pull new feeds, where do we pull from? Or, Will the switchboard for lighting be installed by the government and the contractor connects to that power source?

Answer: Successful offeror’s electrical sub will pull new conductors from existing “MDP” panel adjacent to existing transformer (refer to drawing N27 Site Plan and reference to new panel “MDP” for location of “MDP” panel and transformer) thru existing electrical pit at N-27 with connection to new lighting panel inside/adjacent to new fabric shelter at N-27. Directional boring will be from existing electrical pit at N-27 to new lighting panel inside new fabric shelter. Approximate distance of new conductors is 150’-0” +/-.

INSTRUCTIONS TO CONTRACTORS

1. Prepare three copies of AF Form 3064, in accordance with these instructions, and submit to the contracting officer at the time stated in the contract or as established by the contracting officer.

2. Blocks 1, 2, and 3. Enter the contract number, starting date, and completion date as shown on the contract.

3. Block 8. Enter the title of the project as shown on the contract.

4. Block 9. Enter name and address of your firm. The three copies submitted to the contracting officer must be dated and signed by the contractor or an authorized representative of the firm.

5. Lines 1 through 12. Major elements of work (whether unit price or lump sum contract) such as excavation, concrete work, carpentry, engineering, installation of communication electronic facilities, etc., will be shown in Column B in logical sequence and in sufficient detail to identify the element. If the contract calls for more than 11 work elements, additional copies of the AF Form 3064 may be used as a continuation for the work elements.

6. The percentage of each listed work element to the complete job will be shown opposite each element in Column C. As an example, if 3 work elements are shown: excavation, concrete work, and backfill, and it is estimated that excavation is 50% of the total job, concrete 30%, and backfill 20%, these percentages will be shown opposite each element. The total must equal 100%.

7. Columns D through Q will be used to show planned periodic progress, as required by the contracting officer, during the period of the contract. Only the top half of the space under these columns will be used by contractors. Using the example cited above and assuming that the contract period for performance is 8 weeks, and it is planned to accomplish 10% of the excavation work the 1st week and 20% each week until excavation is complete, 10 would be shown in the upper half of Line 1 under Column D, 20 under Column E, and 20 under Column F for a total of 50%. If it is planned to accomplish the concrete work during the 4th, 5th, and 6th weeks in equal amounts, 10 would be entered under Columns G, H, and I on Line 2. Assuming the backfill will be accomplished during the 7th and 8th weeks, 10 would be entered under Columns J and K on Line 3. If the contract extends beyond 14 weeks, additional copies of AF Form 3064 may be used as a continuation of performance record.

8. Total amount of work planned at intervals as shown in Columns D through Q will be totaled and entered in the top half of the space under the appropriate column of Line 12. Again, using the example cited above, the totals under Columns D through K would read 10, 20, 20, 10, 10, 10, 10, 10 for a total of 100%. These totals will be used to plot an overlay graph using Lines 4 through 11, beginning with Column D and extending to the right the number of columns (weeks) authorized by the contract for performance time.

9. In the event the contract is modified by the contracting officer under terms of the contract, changing the progress of work as originally scheduled, adding or deleting work or changing the original completion date, a revised progress schedule will be prepared and submitted to the contracting officer for approval. In preparing the revised schedule, the amount of work completed will be considered, together with the changed new completion date set forth in the contract modification. Considering these factors, the work under the contract will be rescheduled over the new total performance time in the same manner that the original schedule was prepared. The new completion date will be entered in Block 3. "First Revised Schedule," "Second Revised Schedule," etc., will be entered to the left of the form title. Time of submission of the revised schedule will be determined by the contracting officer.

10. At such intervals as may be established by the contracting officer, the contractor will submit to the contracting officer AF Form 3065, "Contract Progress Report," indicating the percentage of work accomplished for each work element during the established reporting period. Accurate reports are of the utmost importance to the contractor and to the Government, since the percentage of completion, or progress, thus reported is used in administration of the contract and may be used in connection with approval of partial payments under the contract. Care should be taken to plan the work in such manner that it can be accomplished as stated in this schedule. If, for any reason, it becomes known that any part of the progress under this contract will be delayed, this fact should be reported to the contracting officer immediately.
