| 3 | PWS | 12.3.3 | Root Cause Analysis (RCA) is the process of discovering the root causes of problems in order to identify appropriate solutions. RCA assumes that it is much more effective to systematically prevent and solve for underlying issues rather than just treating the evident problem or symptom. Root cause analysis can be performed with a collection of principles, techniques, and methodologies that can all be leveraged to identify the root causes of an event or trend. Looking beyond superficial cause and effect, RCA can show where processes or systems failed or caused an issue in the first place. Unless obvious external issues are the cause this may require an indepth review of trend analysis of events leading up to, at the point of an event and the time that followed to properly diagnose the true Root Cause. PWS 12.3.3 (g.) requires a one sentence summary of the failure's root cause. Can you please clarify this requirement is for a short summary of the work performed and the technician's suspected Root Cause for the failure and not a complete RCA? | Root Cause is in Section 12.4.3 i. |

It is our assumption that the DOD customer does not have any experience or knowledge in the area and shall not have that duty placed upon them. | The PWS was updated to state: a. | The contractor shall manipulate the switchgear and they will not be held liable for any downstream damage assuming correct procedures are followed. The contractor may require the site to fill out a release of liability form for any downstream equipment damage. Regardless of who manipulates the switchgear, there is risk of damage to critical equipment downstream. There is no increase in risk regarding who performs the switchgear manipulation, however, it is a risk that we as the government must assume. |
