SECTION 0101
GENERAL REQUIREMENTS
1. GENERAL:
1.1. Commencement, Prosecution and Completion of Work: The Contractor shall be required to commence work under this contract within ten (10) calendar days after the date of receipt of Notice to Proceed. Work to be performed under this Contract will be ordered by the execution of Delivery Orders by the Contracting Officer. Delivery Orders issued are subject to the terms of the Contract.
1.2 Applicable Codes: All work under this contract shall be accomplished in accordance with the latest published version of the following as applicable:

1.3 Contracting Authority: All references made to the Contracting Officer shall include and be applicable to the Contracting Officer’s Representative (COR) to the limit of his/her authorization as given in writing by the Contracting Officer. Authorization to make changes to the Contract is always retained by the Contracting Officer. Unless otherwise specified, all requests from the contractor and all approvals by the Contracting Officer shall be in writing.
1.4 Permits: The Contractor shall obtain a fully executed dig permit from the Directorate of Public Works prior to performing any excavation work on this project. Excavation is defined for purposes of this contract to be any disturbance of the soil to a depth exceeding 6”. The contractor is responsible for obtaining the signatures from all of the responsible officials identified on the permit. The contractor shall provide a drawing for markup to accompany the permit as it is routed for signatures. Information from utilities personnel showing more accurate placement or unmarked utilities shall be identified on the accompanying site drawing and a copy shall be kept with the excavation crew(s) at all times. The contractor shall provide a copy of the approved work schedule to the identified signing official and shall coordinate all on site utilities marking with the appropriate official.
The contractor shall obtain approval from the Ft. Wainwright Fire Department prior to
31 Mar 2015 performing any flame cutting or welding operations, or closing any road. Contractor shall also obtain confined space entry permits from the Ft. Wainwright Safety Office in accordance with OSHA standards and 29 CFR 1910. Other permits may be required; if other permit requirements are identified or a question arises on a device or procedure, contact the Contracting Officer.
2. CONTRACT DRAWINGS, MAPS AND SPECIFICATIONS:
2.1 General: Standard Details, on 8 1/2” x 11”, follow the Specifications and form a part of the Contract. Additional details may be included in individual Task Orders to clarify specific site conditions. The Contractor shall check all drawings and specifications furnished to him immediately upon their receipt. The Contractor shall promptly notify the Contracting Officer of any discrepancies.
2.2 Verification of dimensions: The Contractor shall be responsible for the coordination and proper relation of his work to the site conditions and shall familiarize himself with all details of the work and working conditions. The Contractor shall verify all dimensions in the field, and will advise the Contracting Officer in writing of any discrepancy before performing any work.
2.2 Existing Conditions and Required Accessories: Because of the small scale and general nature of the drawings, it is not possible to indicate all offsets, fittings, change of elevations and accessories which may be required. The contractor shall investigate conditions affecting all his work and shall arrange such work accordingly, furnishing such fittings, and accessories as may be required to meet such conditions. Supports, anchors, and restraints shall be selected to withstand all static and dynamic conditions of loading to which they may be subjected.
3. MATERIAL AND EQUIPMENT INSTALLATION: All material and equipment shall be installed in accordance with the manufacturer’s recommended instructions. Prior to installation, the contractor shall notify the Contracting Officer, in writing, of any discrepancies between the contract drawings and specifications and the manufacturers recommendations.
4. SUPERVISION: All work to be performed under this contract shall be coordinated by the Contractor or his responsible field supervisor to assure proper performance of the contract. The general contractor shall be responsible for the overall supervision of work and shall insure a complete and operable facility. The absence of the Government Inspector at the site shall in no way relieve the Contractor from his responsibility of performing all work herein in compliance with all codes and quality standards. Detailed requirements of the Contractor’s Quality Control Program are given in Section 0107. All work shall be performed or supervised by journeymen of the applicable trades. Any work performed in deviation from these specifications or the contract drawings shall be immediately corrected by the Contractor regardless of the stage of completion.
5. COORDINATION OF WORK: It shall be the Contractor’s responsibility to conduct all phases of his work in connection with this contract in such a manner as to assure a minimum of interference with Government activities in the area. Provisions shall be made by the Contractor and approved by the Contracting Officer to avoid interference’s with the free passage into and
31 Mar 2015 out of adjoining and adjacent areas not involved in the contracted work. Contractor shall work in phases, limit areas of work, adjust times of work, or provide alternate access routes if necessary to allow for traffic flow in critical areas or heavy use times. Contractor shall notify the Contracting Officer, in writing, two (2) working days in advance of any work that will cause a complete blockage of an area.
6. PLANNED UTILITY OUTAGES AND BLOCKAGES: All utility outages and blockages shall be of minimum duration. Outages and closures shall be scheduled with the Contracting Officers Representative as far in advance as possible, in no case less than seven (7) calendar days before the interruption and a follow-up notification made 24 hours before the interruption. The contractor shall provide notification to all occupants in buildings that will be directly affected with utility outages, blockages, or access and parking disruptions caused by his work. Notification shall be done at seven (7) days and 24 hours prior to the outage. The notification shall tell when, for how long and the type of outage/disruption that will occur.
Occupants shall be notified by personal contact if possible and by posting the notice on all entrance doors to the building. In addition to the occupants, notices shall be provided to the Contracting Officer. Notices shall be removed after completion of the outage/disruption.
7. EXISTING STRUCTURES, UTILITIES AND EQUIPMENT: All existing structures, utilities and equipment shall be protected from damage. Structures, utilities or equipment damaged by the Contractor shall be repaired or replaced to match the preexisting condition.
8. STORAGE AREA: Project material shall be stored by the Contractor in suitable areas designated by the Contracting Officer. The Government will not assume any responsibility for the security or protection of any materials or equipment stored at the site by the Contractor. The Contractor shall be required to fence storage areas containing dangerous or hazardous materials with 6-foot chain link fence, if he chooses to use the storage area on post. The Contractor will be allowed to fence the entire storage area if he chooses. The fence shall be removed and the area cleaned and repaired to the pre-existing condition prior to completion of contract. The Contractor shall isolate all flammable and toxic materials from other materials being stored and secure these materials within the fenced area.
9. UTILITIES NOT SHOWN: If the Contractor encounters, within the construction limits of the entire project, utilities not shown on the plans and not visible as to the date of this contract, and if the Contractor determines that these utilities will interfere with construction operations, the Contractor shall immediately notify the Contracting Officer as to the necessity for removal or relocation. Verbal notification shall be confirmed in writing.
10. FIRE PREVENTION: Cutting or welding will be permitted only in areas that are or have been made fire safe. Where possible, all combustibles shall be located at least 35 feet horizontally from the work site. Where such location is impracticable, combustibles shall be protected with flameproof covers or otherwise shielded with metal guards or curtains. Edges of covers shall be tight to prevent sparks from going under them. This precaution is also important at overlaps where several covers are used to protect a large pile. Other fire prevention precautions shall be in accordance with the latest National Fire Codes and local regulations. The contractor shall have a type “A” fire extinguisher at the site of all cutting or welding operations.
31 Mar 2015 A permit shall be obtained as detailed in paragraph 1.4 of this section.
11. TESTING: Testing shall be the responsibility of the Contractor and shall be performed by an independent and state certified testing facility. Test reports shall be submitted as detailed in Section 0106.
12. LAYOUT OF THE WORK: The Contractor shall be responsible for laying out his work as detailed in Section 0642 Field Engineering.
13. SITE CLEANLINESS: The contractor shall maintain a clean and hazard free work site.
Cleanup shall be daily or more often as necessary to maintain a clean and hazard free work site.
If at any time during the progress of work, the Contracting Officer determines that the Contractor is failing to comply with the clean up requirements. The Contracting Officer may direct the contractor to take such measures, as he deems necessary to constitute corrective action.
14. TOILET FACILITIES: Unless otherwise approved by the Contracting Officer, the Contractor shall provide chemical toilets, placed at locations approved by the Contracting Officer, for use by the Contractor’s personnel. The Contractor shall maintain and keep such toilets in a clean and serviced condition.
15. PRICING OF CHANGES: In accordance with the Contract Clause DOD FAR SUP 52.236-7000, entitled, “Modification Proposals - Price Breakdown”, the Contracting Officer will consider the Contractor’s proposals complete if labor is broken down by major categories of work. Pricing for man-hours shall only include a rate as they appear on the Contractor’s certified payroll, including superintendents and quality control personnel. Labor burden shall be broken out separately. Material shall be priced by unit price for each item to be added or deleted.
Equipment rental shall be listed as either Contractor owned or third party owned and shall reflect a commercial rental rate for each item. Overhead and profit will be applied to the total price of the added or deleted work.
16. PREPARATION OF PROJECT SCHEDULES AND REPORTS: The reports required by the clause FAR 52.236-15 entitled, “Schedules for Construction Contracts”, shall be accomplished on and in accordance with instructions pertaining to USARAK Form 377. Original and three (3) copies, and shall be submitted for the Contracting Officer’s review and approval thirty (30) calendar days prior to construction. A Contractor shall not begin construction until the USARAK Form 377 has been approved by the Contracting Officer. Contractor’s failure to meet the timeliness requirements of this paragraph, may at the discretion of the Contracting Officer, result in the disapproval of the Contractor’s progress schedule, and denial of authorization to begin construction.
17. DAVIS-BACON ACT: Owners, partners and officers of a corporation, partnership or sole proprietorship who perform work predominantly with their hands or with tools and equipment while on the work site, will be considered mechanics and laborers for the purpose of the requirements of the Davis-Bacon Act and other related acts. This provision will apply to the prime Contractor and subcontractors at any tier.

18. UTILITIES: Utility services required on the job site for accomplishment of the work will be furnished at no cost to the Contractor; however, the Government will make no corrections or alterations to existing utility facilities for the accomplishment of the work and any changes required by the Contractor will be at his expense and at no additional cost to the Government.
Utilities for office and/or storage buildings or areas will not be furnished free of charge.
19. ARTIFACTS: Any and all items of Archeological and Cultural Materials which may be discovered in the course of demolition, excavation, or other construction operations, shall remain the property of the Government. Upon discovery of any artifacts, the Contractor shall immediately notify the Contracting Officer, who shall direct the Contractor as to the course of action to be taken. Unauthorized collection and removal of Archeological and Cultural Materials is a felony and subject to prosecution.
20. ACCESS ROSTERS: In no case will the Government allow access to the installation to individuals not on the roster. Therefore the Contractor shall submit a roster indicating the name of each employee that requires access to the installation. The roster shall be prepared on the Contractor’s letterhead citing the appropriate contract number(s). The roster will be submitted to the Contracting Officer who will in turn provide it to the Provost Marshal. Individuals on the roster will be issued either a temporary vehicle pass or a vehicle sticker. In no case will the Government allow access to individuals not on the roster; therefore, the Contractor shall be required to update the roster as necessary.

SECTION 0106
SUBMITTALS
PART 1 GENERAL
1.01 SCOPE OF WORK
A. This section specifies the requirements for providing the following submittals to the Government:
1. Material submittals identified in the contract documents.
2. Project control submittals identified in the contract documents.
3. Equipment operation and maintenance manual submittals identified in the contract documents.
4. Contractor work schedule.
5. Record Drawings.
6. Test Results.

1.03 MATERIAL SUBMITTALS
A. The Contractor shall provide all material submittals, including physical samples, to the Contracting Officer for approval for use on the items of work identified in the Contract.
Submittals shall not be required for each Task Order unless materials other than those already submitted and approved are proposed for the Task Order. Submittals for materials approval shall include, as applicable, standard catalog cuts, product descriptions, testing agency certifications, highlighted specification sheets, design calculations, brochures, test results and certificates of
31 Mar 2015 compliance to fit the submittal requirements specified. Submittals shall provide sufficient information to establish that the material or item(s) meets all requirements of these technical conditions. Physical samples shall bear the contract number, submittal number, and specification section. All physical samples shall become the property of the Government unless stated otherwise.
B. The Contractor shall provide all material submittals for one entire specification division at one time. The Contractor is encouraged to provide all material submittals at one time.
1.04 PROJECT CONTROL SUBMITTALS
A. The Contractor shall provide three (3) copies of all project control submittals to the Contracting Officer.
B. Project control submittals include all field test results, quality control reports and other submittals recording field inspections and compliance with the Contract Drawings and Specifications.
1.05 EQUIPMENT MANUAL SUBMITTALS: Not Used.
1.06 CONTRACTOR WORK SCHEDULES:
A. Within 14 days of execution of each Task Order (and three working days prior to the beginning of work on each Task Order) the Contractor shall present for approval, in writing, to the Contracting Officer, his proposed schedule of work. During the course of the work, if it becomes necessary to update or modify the work schedule, notification shall be made immediately to the Contracting Officer and a revised work schedule shall be submitted within 2 days of the notification to the Contracting Officer for approval. Notification for planned disruption of utilities shall be provided to the Contracting Officer at least seven (7) calendar days prior to their occurrence.
1.07 RECORD DRAWINGS:
A. Record drawings shall be submitted to the Contracting Officer in accordance with SECTION
0108, RECORD DRAWINGS.
1.08 TEST RESULTS:
A. Results from tests shall be submitted to the Contracting Officer within the time period indicated in each Section of the Technical Specification, but not more than 14 calendar days from commencement of the test unless otherwise specifically approved in writing from the Contracting Officer.

1.09 CONTRACTING OFFICER REVIEW
A. The Contracting Officer requires 30 calendar days to review submittals and re-submittals.
This requirement shall be reflected in the submittal control record.

PART 3 EXECUTION
3.01 SUBMITTAL CONTROL RECORD
A. Preparation
1. Prepare the Submittal Register, ENG form 4288-R in accordance with the instructions provided at the preconstruction conference. The completed forms are the submittal control record.
2. Make three (3) copies of the submittal control record and place them in the material submittal notebooks.
3.02 MATERIAL SUBMITTAL NOTEBOOK
A. Preparation
1. Prepare three (3) sets of 3-ring notebooks for the Government, sized for 8-1/2 by 11 inch sheets, with each set large enough to contain a submittal control record and a set of material submittals.
2. Attach one label to the outside of each notebook. Each label shall contain the following information:
“Material Submittal Notebook” (line 1) Contract Number (line 2) Contract Title (line 3) Contractor’s Name (line 4)
3. Insert tabbed and labeled section dividers into each notebook. The number of dividers in each notebook shall equal the number of technical divisions in the contract technical specifications (see Index, Technical Specifications), plus two. For each notebook, label the dividers as follows:
First divider: “Submittal Control Record”
Last divider: “Drawings”
31 Mar 2015 Intermediate dividers: Label each divider with the word “Division”, followed by a division number, for each division included in the technical specifications.
For example, if the contract technical specifications consisted of Division 1, Division 2, then the notebook divider sequence and labeling would be as follows:

A. Preparation Prepare the TRANSMITTAL of Material Submittal forms ENG form 4025 DPW VERSION (identified at bottom of page 2) in accordance with the instructions provided at the preconstruction conference. The completed forms are the material approval submittal records.
3.04 MATERIAL APPROVAL SUBMITTAL PACKAGES
A. Preparation
1. Prepare each material submittal.
2. Attach to each material approval submittal record the material submittal(s) identified on the record. Each material approval submittal record with its attached material submittals is a material approval submittal package.
B. Submittal
1. Submit four copies of each material approval submittal package to the Contracting Officer.
C. Re-submittal

1. The Contracting Officer will return one copy of each submittal package with the submittals marked as approved or disapproved. Disapproved submittals will be marked with the reason for disapproval noted on the material approval submittal record.
2. The Contractor shall retain all approved material submittals contained within the disapproved packages, and resubmit four copies of each corrected material approval submittal package to the Contracting Officer.
3. If the Contractor desires to make a substitution for a product for which he has already submitted a material approval submittal package, then he shall submit four copies of a substitute material approval submittal package.
D. Approved Submittals
1. The Contracting Officer will return one copy of each approved material approval submittal (re-submittal) package to the Contractor with the material approval submittal record signed and marked ‘approved”.
3.05 PROJECT CONTROL SUBMITTALS

1. Submit two copies of each project control submittal to the Contracting Officer.
C. Additional Submittals
1. The Contractor shall submit two copies of each additional project control submittal required by the Contracting Officer when a project control submittal indicates that the work is not in accordance with the contract documents.

SECTION 0107
CONTRACTOR QUALITY CONTROL PROGRAM
1.1 SCOPE OF WORK
1.1.1 The Contractor shall provide and maintain an effective quality control program that complies with the Contract Clause, Inspection of Construction FAR 52.246-12, SPECIAL CLAUSES of the contract, and the requirements of the TECHNICAL SPECIFICATIONS.
1.2 SUBMISSION AND APPROVAL OF THE CONTRACTOR’S QUALITY CONTROL
PLAN (CQC)
1.2.1 A quality control plan shall be furnished to the Contracting Officer for approval in three copies within ten calendar days after receipt of the notice to proceed. This plan shall include procedures, instructions, and reports to be used to provide adequate coverage for all quality control activities required by this contract, and to ensure quality construction in accordance with the plans and specifications. In addition, this plan shall include the following:
1. Names, qualifications, and organization of personnel to be used for quality control.
2. Specific responsibilities and authority of quality control personnel.
3. Test program including names and qualifications of all independent testing laboratories to be employed and the specific tests that each will perform. Other tests to be performed and the Contractor Quality Control personnel who will be responsible for each test will be listed. The test program will also include the method for prior approval of test procedures and equipment, and the methods used to calibrate the testing equipment and otherwise ensure the reliability of information obtained from the tests.
4. Responsibilities and procedures for coordination and advance notice to the Government for all inspections and tests required to be witnessed by the Government.
5. Forms for reporting and documenting all quality control activities
6. Responsibilities and procedures for reporting and documenting all quality control activities, to include deficiencies noted, correction of deficiencies, and retesting or re-inspection of deficiencies.
7. The procedures to be followed for the receipt, storage, and handling of construction items to include the method of verifying that the items used are approved.
8. Copies of all letters of direction to the Contractor’s representatives responsible for
31 Mar 2015 quality control, outlining their duties and responsibilities, signed by a responsible officer of the firm.
9. The Quality Control Officer shall certify that each submittal has been reviewed in detail and that it is correct and in strict conformance with the contract drawings and specifications, except for where a specific request for deviation is being requested.
10. Any other specific requirements unique to the type of work as determined from a review of the contract documents.
1.2.2 When the Contractor proposes to make any changes in the CQC plan or implementation during construction, he must notify the Contracting Officer in writing. Such changes shall not be implemented prior to acceptance in writing by the Contracting Officer. A legible copy of the accepted plan and any changes to the plan, directed or accepted by the Government, shall be kept available at the work site.
1.2.3 No work site construction shall be started until the Contractor’s quality control plan is accepted.
1.2.4 The Contracting Officer requires 10 calendar days to review the Quality Control Plan.
1.3 MEETINGS
1.3.1 After the Government’s approval of the QC Plan and before construction operations are started, the Contractor shall meet with the Contracting Officer to discuss quality control requirements.
1.4 REPORTS
1.4.1 Reports shall be furnished to the Contracting Officer’s Representative daily no later than 1200 hours the first working day following the date which the report covers.
1.5 CAPABILITIES
1.5.1 In addition to the qualifications contained in paragraph 1.3, the Contractor’s quality control organization will perform the following required functions:
1. Check materials at the work site to ensure consistency with contract specifications and the “approved” material submittals.
2. Prepare and maintain current records of all quality control operations to include those of subcontractors.
3. Review and submit all required warranties.
4. Ensure record drawings are kept current.

5. Monitor safety program for effectiveness and conformance with the contract requirements and furnish documentation of deficiencies noted and corrected.
6. Report to the Government any discrepancies in plans and specifications.
1.6 ACCEPTANCE / INSPECTION
1.6.1 The Contractor shall submit a listing of all current construction deficiencies to the Contracting Officer at the time he makes a request for an acceptance inspection. The Contracting Officer will determine whether to schedule an acceptance inspection based in part upon his review of this listing for accuracy and completeness.
1.7 DEFICIENCIES
1.7.1 Should recurring deficiencies as determined by the Government, indicate that the quality control system is not being enforced, corrective action shall be taken as directed by the Contracting Officer and progress payment may be withheld until corrective action is taken.
1.8 QUALITY CONTROL ORGANIZATION
1.8.1 CQC System Manager: The Contractor shall identify a single individual within his organization, who shall be responsible for overall management of CQC and have the authority to act in all CQC matters for the Contractor. The manager shall have a minimum of two years experience in managing a quality control program and shall be given the authority by the contractor to control the project work to meet quality control requirements. The CQC System Manager shall be on site or shall be able to be reached by a communication device at all times during the actual construction work. This individual may also fulfill duties of personnel as described in paragraph 1.8.2.
1.8.2 Personnel: A staff shall be maintained under the direction of the system manager to perform all QC activities. The actual strength of the staff during any specific work period may vary to cover work phase needs, shifts, and rates of placement. The personnel of this staff shall be fully qualified by experience and technical training to perform their assigned responsibilities and shall be directly hired by, and work for, the Contractor. This staff shall consist of, at the minimum, one full time person who will be at the work site at all times construction is in progress, and whose sole responsibility shall be quality control compliance with the contract plans and specifications. This person shall have at least a high school education with a minimum of five years of experience in construction and/or inspection of the type of work required by this contract. Other qualifications, i.e., degrees, general supervision, and experience in place of three of the required years of experience will be considered for approval by the Contracting Officer based on qualification information submitted in writing by the Contractor. The staff shall also include additional quality control personnel as required by the Technical Specifications and as otherwise necessary in order to fully implement the contract quality control system as specified.
These additional personnel shall function primarily in the capacity of quality control and will report to the CQC System Manager who, in turn, will report directly to a level no lower than the Contractor’s top field management.

SECTION 0108
RECORD DRAWINGS
1.1 SCOPE OF WORK
1.1.1 Record Drawings: Record Drawings shall fully describe the work included in the contract. The contractor shall maintain copies of all drawings and details utilized on each delivery order. The contractor shall mark up the drawings as construction progresses to show any changes in the conditions and construction. Marked up drawings shall accurately represent final construction and conditions at the completion of the job. Final Record Drawings shall be on new drawing copies with all field markups transferred neatly to the final mark up sheets. The final Record Drawings shall include all the information shown on the contract drawings. The Contractor shall also include on the Record Drawing all deviations, modifications, or changes from the contract drawing (however minor) which were incorporated in the work. Record Drawing shall also include all additional work not appearing on the original contract drawings, all changes made because of final inspection of the contract work. All mark up drawings shall be in Autocad.
1.1.2 Survey Records: During progress of the work, the Contractor shall keep a record of as built survey data as specified in the various sections of the Technical Provisions. Survey data shall be submitted to the Contracting Officer as specified.
2.1 MEASUREMENT AND PAYMENT
2.1.1 Separate measurement or payment will not be made for the Contractor’s work required under this Section. The Contractor’s cost for preparation and furnishing of the Record Drawings and Survey Records shall be included in the various unit prices bid for the Work.
2.1.2 When Contractor fails to maintain and provide Record Drawings and Survey Records, the Contracting Officer will consider that satisfactory progress has not been achieved for the period in question, thereby requiring the Contracting Office to retain 10% of payments to be made until Record Drawings and Survey Records are provided.
SECTION 0109
MEASUREMENT AND PAYMENT
PART 1- GENERAL
1.1 CONTRACT PRICES
1.1.1 The contract price for each item shall constitute full compensation for furnishing all plant, labor, equipment, materials and incidentals, and performing operations necessary to construct and complete the item in accordance with requirements of the Specifications, the Drawings and the best practice of the trade. Payment for each item shall be considered the full compensation, notwithstanding that minor features of work required to complete the item may not be mentioned.
1.2 UNITS OF MEASUREMENT
1.2.1 Lump Sum: Lump sum items will be measured for payment as a complete item. Lump sum measurement unit is indicated as LS in the Bid Schedule.
1.2.2 Each: Measurement of items as each unit shall be the actual in place count of whole unit provided after the item of work is completed. The measurement unit is indicated as EA on the Bid Schedule.
1.2.3 Linear Foot: Measurement of items by length shall be linear measurements of the item to the nearest foot after the item of work is completed. Linear foot measurement unit is indicated as LF in the Bid Schedule.
1.2.4 Area Units: Measurement of items by area shall be completed for the completed surface area to the nearest whole unit. Measurement units for area are indicated in the Bid Schedule as SF for square foot and SY for square yard.
1.2.5 Volume Units: Volume measurements shall be computed from measurements taken of the in-place volumes of the work to be measured. After the entire volume for each volume item is computed, it shall be rounded off to the nearest whole unit. Measurement unit for volume is indicated in the Bid Schedule as CF for cubic foot and CY for cubic yard.
1.3 PAYMENT
1.3.1 Payment will be made at the contract unit or lump sum prices in accordance with the provisions of the Contract and will constitute full compensation for providing complete and usable items. Payment will not be made until the Contracting Officer has inspected and accepted the work. Costs for work to be performed shall be covered by the prices stated on the Bid Schedule.

1.4 FIELD MEASUREMENT OF QUANTITIES
1.4.1 In place quantities shall be measured jointly by the Contractor and the Contracting Officer’s Representative (COR). It shall be the Contractor’s responsibility to schedule his operations to provide sufficient time for the purpose of measurement of quantities. Payment for items shall not be made until items and quantities are accepted and confirmed by the COR.
1.4.2 Completed work shall not be covered by subsequent work that would prevent quantity measurement of the completed work.
1.5 REQUESTS FOR PAYMENT
1.5.1 The Contractor’s request for payment shall be in a form approved by the Contracting Officer. The proposed form shall include, as a minimum the following:
a. Date of Request
b. Contract Number
c. Task Order Number / Tasking number if appropriate
d. Dates covered by request
e. Items with quantities, bid price and extension
1.5.2. Separate requests for payment shall be submitted for each Task Order / Tasking executed.

1.2 SUBMITTALS
1.2.1 Work Plan: The procedures proposed for the accomplishment of the work shall be submitted for approval by the Contracting Officer. The procedures shall provide for safe conduct of work. Careful removal and disposition of materials specified to be salvaged. Protection of property which is to remain undisturbed, replacement of property disturbed by the construction operations, coordination with other work in progress, and timely disconnection of utility services. The procedures shall include a detailed description of the methods and equipment to be used for each operation, and the sequence of operations.
1.3 DUST CONTROL
1.3.1 The amount of dust resulting from demolition shall be controlled to prevent the spread of dust to occupied portions of the construction site and to avoid creation of a nuisance in the surrounding area. Use of water is allowed, but it will not be permitted when it will result in, or create, hazardous or objectionable conditions such as ice, flooding and pollution. Other dust control information and limitations are found in section 0641 ENVIRONMENTAL
PROTECTION.
1.4 PROTECTION
1.4.1 Protection or Replacement of Existing Property: Before beginning any demolition work, the Contractor shall carefully survey the site and examine the drawings and specifications to determine the extent of the work. The Contractor shall take all necessary precautions to avoid damage to existing items to remain in place, to be reused, or to remain the property of the Government. Any damaged items or items required to be removed to accomplish the work shall be repaired or replaced as a part of this contract. The Contractor shall carefully coordinate the
31 Mar 2015 work of this section with all other work and shall construct and maintain shoring, bracing and supports, as required. The Contractor shall ensure that structural elements are not overloaded and shall be responsible for increasing structural supports or adding new supports as may be required as a result of any cuffing, removal, or demolition work performed under this contract.
1.4.2 Protection from the Weather: Protection of stockpiles and materials from environmental conditions shall be the responsibility of the Contractor to maintain these materials in good usable condition. Specific requirements for protection of materials are found in the applicable sections of these specifications.
1.4.3 Environmental Protection shall comply with the requirements of Section 0641.
1.5 BURNING
1.5.1 The use of burning at the project site for the disposal of refuse and debris will not be permitted.
1.6 USE OF EXPLOSIVES
1.6.1 Use of explosives will not be permitted.
1.7 AVAILABILITY OF WORK AREAS
1.7.1 Areas in which the work is to be accomplished will be available in accordance with the approved Work Schedule as specified in Section 0101
1.8 SAFETY REQUIREMENTS:
1.8.1 Barricades: Furnish and install any necessary barricades to protect the public or workmen during demolition. Barricades to keep the public out of demolition areas shall be left in place until removed by the contractor after they are no longer required for protection.

3.1 EXISTING STRUCTURES
3.1.1 All existing structures shall be protected from damage and selective demolition required to perform the project work shall be repaired to existing or better condition. Utility poles, signs, trees and landscaping shall be supported and protected, if possible, or removed and replaced after construction. Temporary measures shall be provided for support, safety and control until such items are permanently replaced. Sidewalks, curbs, gutters and roadways shall be cut, removed and replaced as specified and as necessary for construction.
3.2 UTILITIES
3.2.1 Interruption of utility services shall be accomplished only as specified in Section 0101.
3.3 DISPOSITION OF MATERIAL
3.3.1 The Contractor is responsible for the disposal of all materials not identified to be Government salvage items. Disposal shall be made in accordance with local and federal disposal regulations. Rubbish and debris shall be removed from Government property daily, unless otherwise directed, to avoid accumulation at the work site. Materials that cannot be removed daily shall be stored in areas specified by the Contracting Officer. Salvaged items and materials shall be disposed of as specified. See Section 0641, Environmental Protection paragraph 3.2 Control and Disposal of Waste for additional disposal requirements.
3.3.2 Material Salvaged for the Contractor: This contract does not include contractor-salvaged materials.
3.3.3 Items Salvaged for the Government: Any items identified by the government for salvage or reuse shall be carefully removed and protected until they can be reused in locations indicated in plans or specifications or removed by the Government. The Contractor shall notify the Government Inspector when these items have been removed.

1.2 DEFINITIONS
1.2.1 Satisfactory Materials: Satisfactory materials shall comprise any materials classified by, ASTM D 2487 as GW, GP or SW. Satisfactory materials for grading shall be free from roots and other organic matter, trash, debris, and frozen materials and stones larger than 6 inches in any dimension.
1.2.2 Unsatisfactory Materials: Materials which do not comply with the requirements for satisfactory materials are unsatisfactory. Materials classified in ASTM D 2487 as Pt, OH, and OL are unsatisfactory. Unsatisfactory materials also include refuse, or frozen materials.
1.2.3 Cohesionless and Cohesive Materials: Cohesionless materials include materials classified in ASTM D 2487 as OW, GP, SW, and SP. Cohesive materials include materials classified as GC, SC, ML, CL, MH, and CH. Materials classified as GM and SM will be identified as cohesionless only when the fines are nonplastic. Testing required for classifying materials shall be in accordance with AASHTO T90, AASHTO T27, AASHTO T88, and ASTM D 1140.
1.2.4 Degree of Compaction: Degree of compaction required is expressed as a percentage of the maximum density obtained by the test procedure presented in AASHTO T180, Method D, abbreviated as a percent of laboratory maximum density.
1.3 SUBMITTALS:
The following shall be submitted in accordance with Section 0106 SUBMITTALS:
1.3.1 Statements indicating the procedure and location for disposal of unused satisfactory material and proposed source of borrow material. For each backfill material used, submit laboratory test results for moisture density relationships, per AASHTO T180, Method D
1.3.2 Test reports shall be submitted within 24 hours of conclusion of physical tests, 3 copies of test results, including calibration curves and results of calibration tests.
1.3.3 Submit for approval the name of the independent testing laboratory
1.3.4 Notification shall be given of encountering rock in the project and advance notice on the opening of excavation or borrow areas.
1.4 DIG PERMIT
1.4.1 The Contractor shall be responsible for obtaining a dig permit for each Task Order/Tasking prior to commencing excavation.

1.5 CLASSIFICATION OF EXCAVATION
1.5.1 Excavation specified shall be done on a classified basis, in accordance with the following designations and classifications.
1.5.2 Rock Excavation: Material classified as rock and shall include boulders 1/2 cubic yard or more in volume; solid rock; rock material that is in ledges, bedded deposits, and unstratified masses, which cannot be removed without systematic drilling and blasting; and firmly cemented conglomerate deposits possessing the characteristics of solid rock impossible to remove without systematic drilling and blasting. If at any time during excavation, including excavation from borrow areas, the Contractor encounters material that may be classified as rock excavation, such material shall be uncovered and the Contracting Officer notified by the Contractor. The Contractor shall not proceed with the excavation of this material until the Contracting Officer has classified the materials as common excavation or rock excavation and has informed the Contractor of the action to be taken. Failure on the part of the Contractor to uncover such material, notify the Contracting Officer, and allow ample time for classification and notification will cause the forfeiture of the Contractor’s right of claim to be paid for other than that allowed by the Contracting Officer for the items of work in which such deposits occur.
1.5.3 Common Excavation: Common excavation shall include the satisfactory removal and disposal of all materials not classified as rock excavation.
1.6 BLASTING
1.6.1 Blasting will not be permitted.
1.7 UTILIZATION OF EXCAVATED MATERIALS
1.7.1 Unsatisfactory materials removed from excavations shall be disposed of in designated waste disposal or spoil areas. Satisfactory material removed from excavations shall be used, insofar as practicable, in the construction of fills, embankments, subgrades, shoulders, bedding (as backfill), and for similar purposes. No satisfactory excavated material shall be wasted without specific written authorization. Satisfactory material authorized to be wasted shall be disposed of in designated areas approved for surplus material storage or designated waste areas as directed.
