Past Performance Questionnaire

EXCEPTIONAL VERY GOOD SATISFACTORY MARGINAL UNSATISFACTORY NEUTRAL
Contractor met and exceeded many of the requirements of the contract and consistently performed at a superior level.
Perfomance was accomplished with very few minor problems, and the contractor took immediate and effective corrective actions for those problems.
Contractor met and exceeded some of the requirements of the contract and consistently performed very well. Performance was accomplished with some minor problems, and the contractor took timely and effective corrective action for those problems that did occur.
Contractor met the requirements of the contract and consistently performed at an acceptable level.
Performance was accomplished with some problems, and the contractor took effective corrective action for those problems that did occur.
Contractor did not always meet some of the requirements of the contract and intervention was required to continue performance. There were instances where performance was at a less than acceptable level; performance was accomplished with some problems and some corrective actions appear only marginally effective or were not fully implemented.
Contractor did not meet the requirements of the contract and performance was at an unacceptable level. There were a number of serious problems that required extensive oversight and involvement, and corrective actions were either ineffective or non-existent.
Unable to provide a rating. Contract did not include performance for this aspect, or information was not available. Do not know.

Quality of Workmanship: Contractor completed all work with good workmanship and in conformance with the specifications, resulting in minor, if any, punchlist items.

Adequacy & Implementation of the CQC Plan: Contractor provided effective quality control and inspection procedures that resulted in a quality-finished project.

Quality of QC Documentation: Contractor thoroughly reviewed correspondence for accuracy, completeness, and compliance with contract requirements.
4. Adequacy of Materials: Contractor provided well researched and clearly identified submittals that matched contract requirements.
5.
Adequacy of Submittals: Contractor thoroughly reviewed correspondence for accuracy, completeness, and compliance with contract requirements.
6.
Adequacy of As-Builts: Contractor provided a well documented As- Builts that are accurate, complete, timely, and comply with contract requirements.

II. Management Effectiveness / Business Relations E VG S M U N

Cooperation and Responsiveness: Provided timely/effective contract problem resolution without extensive customer guidance. Contractor provided timely and satisfactory response to warranty issues after project completion.
2.
Management of Resources / Personnel: Contractor hired, maintained, and replaced as necessary, qualified personnel and subcontractors/suppliers. Contractor proposed alternative methods/processes that reduced cost, improved maintainability or other factors that benefited the customer.
Coordination and Control of Subcontractors: Contractor hired quality subcontractors and effectively managed and coordinated their work.
4. Adequacy of Site Cleanup: Contractor cleaned the site as required.
5.
Effectiveness of Job Site Supervision: Contractor ensured the site manager had sufficient authority to make decisions and take actions during project performance to keep the project on schedule.
6.
Compliance with Laws and Regulations: Contractor understood/complied with customer objectives, technical requirements, laws, and regulations.
7.
Professional Conduct: Contractor presented and addressed government concerns in a professional matter. Contractor provided experienced qualified managers and supervisors with the technical and administrative abilities needed to meet contract requirements.
8. Review/Resolution of Subcontractor’s Issues: Contractor paid employees/subcontractors/suppliers as required.
9. Implementation of Subcontracting Plan: Contractor adhered to submitted and approved subcontracting plan.

1. Adequacy of Initial Progress Schedule: Contractor developed and submitted realistic progress schedules.
2. Adherance to Approved Schedule: Contractor met established project schedules to complete the project on time.
3. Resolution of Delays: Contractor resolved delay issues and took necessary actions keep the project on schedule where appropriate.
4.
Submission of Required Documentations: Contractor provided submittals on time as required. Contractor provided payrolls for both their firm and their subcontractor’s employees as required and scheduled.
5. Completion of Punchlist Items: Contractor provided timely resolution of all punchlist items.
6.
Submission of Updated and Revised Progress Schedules: Contractor submitted the progress schedule and interim progress reports as required in a timely manner and the schedule was approved prior to actual construction.

7. Warranty Response: Contractor resolved warranty issues in a timely manner.

1. Correction of Noted Deficiencies: Contractor corrected deficiencies associated with labor standards requirements.

Payrolls Properly Completed and Submitted: Contractor provided payrolls for both their firm and their subcontractor’s employees as required.
Compliance with Labor Laws and Regulations: Contractor facilitated labor inspections for determination of compliance with wage determinations and labor standards. Labor posters and wage determinations were displayed as required. Subcontractor acceptance of labor standard clauses submitted as required. Statement of Compliance for payroll reports was submitted as required.
