Offeror: Complete Section 1 and send a questionnaire to the primary customer point of contact (POC) who is identified as the most familiar with the performance on this contract.
1. The contractor identified in Section 1 of this questionnaire is being considered in a Source Selection by the 325th Contracting Squadron at Tyndall Air Force Base, FL. You have received this questionnaire because you are a source of Past Performance Information for that offeror.

2. Please complete the attached questionnaire for the contractor and send the completed questionnaire via email to bobbie.davis@us.af.mil or britney.petrina.1@us.af.mil. Begin the subject line of the email with “SOURCE SELECTION SENSITIVE INFORMATION (SEE FAR 2.101 & 3.104).”

3. Based upon your knowledge and experience with the contractor’s performance, assign a rating consistent with the rating/description criteria below. Limit your assessment to the particular program referenced in Section 1. Place an “X” in the appropriate column using the definition matrix below.

Section 3: Rating Definition
Exceptional (E): Performance meets/met contractual requirements with many exceeded to the government’s/customer’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with no more than a few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good (VG): Performance meets/met contractual requirements with some exceeded to the Government’s/customer’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with no more than some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory (S): Performance meets/met contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor were satisfactory.
Marginal (M): Performance does not meet/did not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory (U): Performance does not meet/did not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
Not Applicable (N/A): Unable to provide a rating. Contract did not include performance for this aspect, performance was not observed, or information was not available.
