STATEMENT OF WORK
FOR
RPR HVAC & RENO COMM FAC
AT
MACDILL AIR FORCE BASE, FLORIDA
PROJECT NO.: NVZR 16-0123

I. PERFORMANCE PERIOD
A. The performance period is broken down into three phases. A separate Notice to Proceed (NTP) will be issued for phase I and II. The performance period consists of 330 calendar days.
B. Phase I completion requires the following:
1. The Period of Performance for Phase I is 180 calendar days.
2. Approval of required submittals as identified on AF Form 66.
3. Receipt and approval of all required permits.
C. Phase II is the Construction Phase of the project.
1. The Period of Performance for Phase II is 150 calendar days.
2. The contractor shall request a Pre-Final Inspection not less than 14 calendar days prior to the intended date of the Pre-Final. The date of the Pre-Final must allow adequate time to correct all deficiencies and conduct the Final Inspection prior to the construction completion date. A contractor-provided punch list, including the status and projected completion date for all open items, shall accompany this Pre-Final request.
D. Phase III is the closeout phase of the project.
1. Phase III is 30 calendar days.
2. Phase III is not part of the performance period and will be considered complete once all closeout documents and submittals have been received and approved by the Government.
3. As-Built drawings are required and shall be submitted in Micro-Station or AutoCAD (2017 or earlier), as well as in PDF.
E. Upon award of the task order, the contractor will be expected to:
1. Submit Payment and Performance Bonds and the Quality Control Plan specific to the task order within 10 days. After acceptance of the Payment and Performance Bonds and the Quality Control Plan, the Notice to Proceed (NTP) shall be issued by 6 CONS.
2. Request a design kick-off or a Construction kick-off meeting through 6 CONS.
F. Phase requirements are as follows:
1. Phase I (design phase)
a. Contractor shall provide the development of the intermediate design submittals to 100% Final Design documents.
b. Contractor shall provide formal 95%, and 100% design submittals to include the material submittals process.
c. In accordance with the basic MACC Contract: Contractor must coordinate with the Government in arranging a 65% and 95% design review meeting. The Contractor’s design firm with representation from each discipline shall be in attendance at all design meetings. The design team shall be responsible for presenting all aspects of the design. The contractor shall submit minutes of the review meeting within five
(5) working days that details the discussion points and the contractor’s action plan for these discussion points.
d. Contractor shall submit a Contract Progress Schedule, AF Form 3064, within 10 days of the issuance of Phase I NTP. The schedule shall include line item breakdowns for design submittals and construction work elements as well as environmental permitting milestones, if applicable, pre-final inspection / punch list and close-out documents. This includes the 5% retention for closeout submittals.
e. All submittals shall be submitted electronically. A hard copy of the signed and sealed final drawings and permits shall be submitted as well.
2. Phase II (construction phase)
a. NTP to Phase II shall be issued upon:
i. Approval of all Phase I submittals, required permits, and 100% Final Design submittal.
ii. Contractor shall request a construction kick-off meeting through 6 CONS. This meeting shall take place prior to issuance of the Phase II NTP.
3. Phase III (closeout phase)
a. Contractor shall provide all necessary approved close-out documents not limited to payrolls, progress reports, submittals, release of claims, and final AF Form 1354.
Refer to AF Form 66 and SOW for additional close-out requirements.
II. SCOPE
This Statement of Work (SOW) defines the scope for this task order to include: Replacement of the existing HVAC system with a new system, renovation of the office & meeting spaces, and replacement of the existing restrooms with new expanded & accessible restrooms with a separate shower/locker room; all offerors are cautioned that the construction team is expected to spend an adequate amount of time at the facility prior to proposal submittal in order to fully assess the existing conditions to ensure that appropriate construction methods can be used to replicate the existing features in the proposed work. The Government shall reference the basic MACC contract proposals for information such as quality control, teaming, and personnel qualifications. The place of performance shall be at MacDill Air Force Base (MAFB), Tampa, Florida (FL).
Upon completion of Work, the Contractor must ensure all systems affected by the Work must be fully functional and operational for intended purpose.
The following is a brief summary of the project, and is provided solely for the purpose of revealing the general nature of the work involved:
A. Provide all labor, materials, and equipment as required to: Renovate the existing offices & meeting spaces and replace the existing restrooms as generally outlined below and as specifically described and reasonably inferred in accompanying documents:
1. CIVIL: NEW WORK
a. The Contractor shall undertake all work environments and methods in accordance with USACE EM 385-1-1 ‘Safety and Health Requirements’.
b. The project work site is remote and/or lacking personnel life support. The Contractor shall be responsible for providing all required personnel life support, shelter and comfort by importing, relocating and maintaining necessary temporary facilities.
c. The Contractor shall provide materials testing by a third party laboratory in accordance with the Government provided instruction drawing set, project USAF Form AF66, and as provided for elsewhere in the project documentation. Contractor identified testing laboratory shall be listed by USACE Materials Testing Center in accordance with ER 1110-1-261, or possess a current national authority issued certification of accreditation under ASTM D3666, or possess a current CMEC issued certification of accreditation. Contractor identified testing laboratory shall possess listing and/or accreditation for each testing standard they are to undertake.
Copies of all relevant accreditation certificates shall be provided to the Government.
d. Materials identified as FDOT compliant shall be listed by FDOT on an approved material and/or source site list, or certified by an FDOT approved laboratory.
Materials certified by an FDOT approved laboratory do not require additional testing by the Contractor's third party laboratory.
e. The Contractor shall sod unpaved areas disturbed as part of this project effort with Argentine Bahia sod. Disturbed area shall include all areas impacted by the construction equipment where it is indicated that the sod has been destroyed. The contractor shall provide to the Government a certification from the Florida Dept. of Agriculture and Consumer Services stating that sod/hay/straw and mulch materials are free of noxious weeds. The new sod shall be watered with a contractor provided and installed temporary irrigation system with a remote timer. The irrigation system shall operate for a minimum period of 8-weeks. At the end of the 8-weeks the system shall be removed and disposed of by the contractor.
f. The Contractor shall clear and clean limit of work before vacating project.
g. Existing Conditions Investigation:
i. Obtain site details, topographic and utility survey data to the extent required to complete the project design.
ii. Any executed surveys shall be made a part of the plan set.
iii. Contractor shall field verify all Government provided distances, dimensions, and elevations.
h. Permit Activity and Negotiation:
i. The Contractor shall prepare, submit, negotiate and pay for all identified project permits.
ii. The Contractor shall submit copy of all project permit applications to the Government for approval prior to application with governing authority.
iii. The Contractor shall notify the Government in advance of all governing authority meetings, hearings, inspections and actions in regards to project permit applications.
iv. The Contractor shall provide copy to the Government of all final permit and/or action related to project permitting.
v. The Contractor shall be responsible for the full work intent and requirement of all issued project permits.
i. Erosion and Sediment Control Barriers and Barricades:
i. The Contractor shall provide and install all erosion protection and storm water pollution prevention devices prior to construction as required by the MacDill AFB General Design Guidelines for Facility Construction and Renovation and project jurisdictional permit approvals.
ii. The Contractor shall provide and install existing tree barricades in accordance with the Hillsborough County, Tree and Landscape Code Technical Manual.
iii. The Contractor shall provide and install a stabilized construction entrance in accordance with FDEP/NPDES recommendations wherever and whenever construction access to unstabilized areas could track earth onto connecting roadways.
iv. The Contractor shall inspect daily, and repair or replacement as required, all project erosion and sediment control.
v. The Contractor shall provide daily sweeping of connecting vehicle roadways and surfaces whenever tracking of material off site occurs.
vi. The Contractor shall provide daily water spraying for control of dust production from the work site.
vii. In the event that the Contracting Office orders a suspension of work, the Contractor shall install temporary stabilization of all open earth, excavations, earth works, and stockpiles. The effort shall provide sufficient stabilization for the anticipated period of suspension including the closure of open excavations and provision of erosion matting in storm-water conveyances and temporary seeding of disturbed areas if suspension will exceed one month duration.
viii. After completion of work and permanent site stabilization, and as approved by the Government, the Contractor shall remove and dispose of off base all temporary erosion protection and storm water pollution prevention devices.
j. Demolition:
i. Remove and save all existing signage, lighting, fencing, site furniture, etc. for re-installation.
ii. Clear and grub site as directed on plans.
iii. Disposal areas for rubble, unsuitable or excess soil materials or other construction debris are not available on the base for the Contractor’s use.
Disposal must be provided by the Contractor, off Government controlled lands, at lawfully permitted and operated sites.
iv. For utilities to be removed or relocated (water, sanitary, communications, etc.) disconnect and/or plug the utility at the connection to main lines. All abandoned conduits and pipe larger that 2-inches shall be removed (irrigation, sanitary and water mains). The Contractor shall GPS all locations of utility disconnection points and show them on the as-built drawings.
v. Demolish and dispose off-base elements identified on the RFP drawings including but not limited to pavement, sidewalk, hardscape, posts, supports, utilities, trees, bushes and furnishings. All permanent hardscape demolition shall include removal of the base material down to natural sub-grade.
vi. Demolish and dispose off-base site structures and foundations identified on the RFP drawings in their entirety including but not limited to connecting services and accessory pads.
vii. Damage to project site by Contractor not required by this SOW will be repaired at Contractor expense
k. Utility Construction:
Furnish and install all site utility construction repairing any disturbed existing condition to its original condition as indicated on the project drawings and in accordance with minimum standards and specifications, as well as the materials and workmanship Minimum Performance Standards (Appendix F), but not less than the following minimum requirements.
i. No open trenching shall be accessible to the public. All trench work shall be barricaded from pedestrian and vehicular activity in accordance with Base Standards.
ii. All new belowground structures, frames, and covers shall have AASHTO H20 (min) bearing capacity.
iii. All new belowground access ways shall have 24” (min) diameter clear passage with cast iron frame and cover.
iv. Wherever utility run requires the cutting of existing vehicular pavement, the last 18 inches of trench backfill below pavement are to be completed using FDOT Specification 121 Flowable-Fill with a maximum compressive strength of 100 psi. Repair existing pavement in like kind.
v. All new water mains shall be AWWA C900 DR18 with mechanically restrained joints
vi. All new fire hydrant assemblies shall be designed to provide 1,200 GPM minimum.
vii. Prior to any portion of a potable water system being placed into or restored to service, it shall be cleaned, disinfected, and tested for bacteriological contaminants in accordance with the latest applicable provisions of the American Water Works Association (AWWA), "Standard for Disinfecting Water Mains" (AWWA C651); and the Florida Department of Environmental Protection (FDEP), "Permitting, Construction, Operation and Maintenance of Public Water Systems", FDEP Rules 62-555.315(6), 62-555.340 and 62- 555.345, Florida Administrative Code (FAC).
viii. All new sanitary sewer pipe shall be ASTM D1785 SCH 40 PVC (min) for gravity drains and ASTM D2241 SDR 21 (min) for force mains.
ix. All new sanitary sewer manholes shall be lined or epoxy coated.
2. ARCHITECTURAL: Contractor must be responsible for the professional quality and technical accuracy of the services furnished by the Contractor under this contract.
Contractor shall reference the architectural drawings for supplementary information.
Contractor must verify dimensions and design, construct, and install items in adherence to the design intent presented herein and in the drawings, UFC 1-200-01, Design:
General Building Requirements, UFC 3-100-10, Architecture with Change 3, UFC 3- 120-01, Design: Sign Standards with Change 2, UFC 3-120-10, Interior Design, with Change 1, ADAAG, Americans with Disabilities Act Accessibility Guidelines, MacDill Design Guide (latest edition), current edition, and compliance of all applicable codes.
The word "color", as used herein, includes surface color and pattern. When color is not designated for items, Contractor shall propose a color for approval in the Color Binder.
Where color is shown as being specific to one manufacturer, an equivalent color by another manufacturer may be submitted for approval. Manufacturers and materials specified are not intended to limit the selection of equal colors from other manufacturers.
Contractor shall refer to Appendix F for minimum performance standards of items stated below as performance specifications.
a. DEMOLITION: The Contractor shall take the reasonable steps to ascertain the nature and location of work, and that it has been investigated and satisfied itself as to the general and local conditions which can affect the work or its cost.
i. Remove & discard restroom walls, plumbing fixtures & piping (see plumbing), HVAC diffusers/grilles & ductwork (see Mechanical), flooring, restroom accessories, toilet partitions, lavatory counters, ceilings, and lighting (see electrical).
ii. Remove & discard ceiling grid & tiles, light fixtures & other electrical items (see electrical), flooring (in most locations except as noted on drawings), walls & doors/frames (as shown on drawings), and HVAC diffusers/grilles & ductwork (see mechanical).
iii. Return air openings (metal w./fusible link damper) and supply air ductwork (see mechanical drawings) in/through fire walls above ceiling.
iv. Before demolition begins contractor will move and store all furniture; then move all of it back into the building after construction completion. The user will move all personal belongings and IT equipment to their temporary facilities.
b. NEW WORK: Contractor shall construct/install, the following for the renovation of Building 262 for COMM office:
i. Provide & install restroom walls, plumbing fixtures & piping (see plumbing), HVAC diffusers/grilles & ductwork (see Mechanical), flooring, restroom accessories, toilet partitions, lavatory counters, ceilings, and lighting (see electrical).
ii. Provide & install ceiling grid & tiles, light fixtures & other electrical items (see electrical), flooring (in most locations except as noted on drawings), walls & doors/frames (as shown on drawings), and HVAC diffusers/grilles & ductwork (see mechanical).
iii. Seal existing corridor fire walls above ceilings in locations where the return air openings and supply air ductwork (see mechanical drawings) have been removed.
iv. Where electrical & communication outlets and other electrical items are being abandoned (see electrical drawings and/or electrical section of this SOW) fill/patch and prep for paint the existing walls.
3. PLUMBING: The contractor is responsible to construct and install all required equipment and materials in accordance with the following codes and standards:
Unified Facilities Criteria (UFC) 1-200-01, General Building Requirements UFC 1-200-02, High Performance and Sustainable Building Requirements, with
Change 01, 1 October 2017 UFC 3-420-01 Plumbing Systems, including Change 10, October 26, 2015 UFC 3-600-01, Design: Fire Protection Engineering for Facilities, Change 2, 25
March 2018 UFC 4-010-01, DoD Minimum Antiterrorism Standards for Buildings, with
Change 1, dated 1 October 2013 AFI91-203 Air Force Instruction
All specifications stated below are in addition to all of the above mentioned design criteria. When ambiguities or inconsistencies arise between Department of Defense (DOD), U.S. Air Force Regulations, and Civilian Codes/Standards, the U.S. Air Force Regulations shall govern. In the event that certain areas of design are not specifically addressed by the U.S. Air Force Regulations, the most conservative design requirements as defined by applicable codes or association standards and as determined by government representative shall be used. Visit the existing facility and carefully examine those portions of the building affected by this work before submitting proposals, so as to become familiar with the existing conditions and difficulties that will affect the execution of the work. Submission of a proposal will be construed as evidence that such examination has been made. Contractor shall reference the plumbing drawings for supplementary information.
a. GENERAL NOTES:
i. These drawings are considered to be diagrammatic, intended to show general locations of piping, and not necessarily showing in detail or to scale all of the equipment, sizes, or exact locations.
ii. DEMOLITION: The contractor shall remove and dispose of all of, the following in order to accommodate new construction:
a) Demolish and remove or abandon in place the sanitary drain piping where indicated on sheet P-101. Protect the trench for new sanitary piping and infill the remaining trench where not reused. Demolish and remove the vent stack piping up to one foot below the roof penetration and protect the roof vent for reuse.
b) Demolish and remove plumbing fixtures indicated on sheet P-102.
c) Demolish and remove all hose bibs, the electric water heater, and domestic water piping back to the backflow preventer. Protect exterior wall penetrations for reuse.
iii. NEW WORK:
a) Provide and install new schedule 40 PVC piping for the sanitary sewer line.
Provide and install new vent stack piping and connect it to the existing roof vent. Provide and install a new grinder pump connected to the kitchen sink and vent stack.
b) Provide and install new schedule 40 CPVC piping and insulation for the domestic cold and hot water piping. Connect this new piping to the existing backflow preventer.
c) Provide and install a new electric water heater with an expansion tank. Pipe the drain pan to the mechanical room floor drain.
d) Provide and install plumbing fixtures.
4. MECHANICAL: The contractor is responsible for constructing and installing all required equipment and materials in accordance with the following codes and standards:

MacDill AFB General Design Guidelines for Facility Construction and Renovation (Latest Edition)

All specifications stated below are in addition to all of the above mentioned design criteria. When ambiguities or inconsistencies arise between Department of Defense (DOD), U.S. Air Force Regulations, and Civilian Codes/Standards, IBC, ASHRAE, etc.

Visit the existing facility and carefully examine those portions of the building affected by this work before submitting proposals, so as to become familiar with the existing conditions and difficulties that will affect the execution of the work. Submission of a proposal will be construed as evidence that such examination has been made. Contractor shall reference the mechanical drawings for supplementary information.
The Contractor shall be responsible for the professional quality and technical accuracy of all HVAC design documents and shall ensure construction meets all requirements of the approved design. Drawings, specifications, and other design documents upon which construction is based shall be coordinated with other disciplines to ensure compatibility of all building systems.
It shall be the contractors’ responsibility to verify occupancy and heat loads for all equipment prior to commencing work. The contractor shall submit, for review and approval, building HVAC load calculations and shall demonstrate sustainable design and energy use reduction. Heat gain and loss calculations shall be, as a minimum, in accordance with the ASHRAE Handbook of Fundamentals. Computer load calculations shall be provided, and include complete input and output summaries. Cooling equipment shall be selected to meet the calculated load operating at 95 deg F ambient.
The design of the facility systems shall make use of the latest technology to provide equipment with the highest efficiency possible to achieve compliance with EPACT 2005 without compromising maintainability. Avoid placement of HVAC equipment on top of roofs.
a. General Design Parameters

vi. Exhaust Rates:
Restrooms Minimum 75 cfm per fixture.
Janitor's Closets Minimum 1 CFM/Sq. Ft.
Maintain positive pressurization of building to overcome infiltration. Rate of 8% greater than exhaust volume.
vii. Mechanical rooms where air handling equipment is located and electrical rooms shall be part of the conditioned spaces.
viii. Each telecommunication room shall be designed for a sensible heat load of approximately 20 watts per square foot. Verify exact loading prior to design.
Communication rooms shall be provided with computer room air conditioning-type units sized for the equipment load and provided with humidifier, reheat and condensate pump (if required to pump condensate to drain location).
Provide sufficient reheat to offset the cooling sensible capacity of the unit for humidity control at low loads. Provide building air as backup.
ix. All workstations within the facility will be provided with thin-client interfaces.
x. Miscellaneous Loads:
Equipment Heat Load for Office Spaces and Other Areas: Include 1.0 watts per square foot (approx.) for miscellaneous office equipment loads and 1.0 watt per square foot (approx.) lighting load.
xi. Personnel:
As indicated by architectural room descriptions and furniture layouts.
xii. Outside Air Criteria:
Design facilities to comply with ASHRAE Standard 62.1 2010. The Contractor shall provide documentation describing the calculations and methodology for ventilation in accordance with ASHRAE 62.1 2010.
b. GENERAL NOTES:
i. These drawings are considered to be diagrammatic, intended to show general locations of piping, and not necessarily showing in detail or to scale all of the equipment, sizes, or exact locations.
c.
i. DEMOLITION: The contractor shall remove and dispose of all of, the following in order to accommodate new construction:
a) Demolish and remove all ductwork, dampers, thermostats, and air devices.
Protect the mechanical room penetrations for reuse.
b) Demolish and remove the exhaust fan and exhaust ductwork. Protect the roof penetration for reuse.
c) Demolish and remove the air handler, chiller, pump, chilled water piping, and all mechanical equipment in the mechanical room. Recover all R-22 refrigerant and return it to the MacDill AFB HVAC shop.
ii. NEW WORK: Provide and install a complete mechanical system as seen on the construction documents.
a) Provide a complete test and balancing of the air distribution systems and final NEBB or AABC Certified Report shall be submitted for review and approval. The test and balance work shall take place when the daytime high temperature is below 70 degrees F (winter season) and when the daytime high temperature is above 92 degrees F (summer season). The report shall include both seasons.
b) Provide and install a new 15 ton air handler and condensing unit with a unit controller and all devices shown in the controls diagram. Provide and install the air handler condensate drains to the new mechanical room floor drain.
Seal around all ductwork, refrigerant, and suction piping.
c) Provide and install a new 4 ton air handler and condensing unit with a unit controller and all devices shown in the controls diagram. Provide and install the air handler condensate drains to the new mechanical room floor drain.
Seal around all ductwork, refrigerant, and suction piping.
d) Provide and install a 1 ton air handler and condensing unit with a unit controller. Seal around all refrigerant and suction piping.
e) Provide and install new ductwork, insulation, variable air volume boxes, thermostats, and air devices shown in sheet M-102. Provide and install volume dampers on supply ductwork runoffs.
f) Provide and install a new HVAC emergency shutdown button (shelter in place) in accordance with UFC 4-010-01. The button should shut off all air handlers and exhaust fans and close all motorized dampers within 30 seconds.
g) Provide and install a new building control unit located in secure communications/storage room. The controls laptop will be procured from the government. This laptop and the controls software shall have DOD authorization to operate (ATO).
5. ELECTRICAL: The contractor is responsible to construct and install all required equipment and materials in accordance and listed within the latest version of:
a. UFC 3-501-01.
b. UFC 3-520-01.
c. UFC 3-530-01.
d. UFC 3-550-01.
e. UFC 3-580-01
f. UFC 3-600-01
g. Current MacDill Design Guidelines.
h. In addition to above, contractor shall cite industrial or commercial codes observed during design. (NFPA, NESC, et cetera.)
i. When ambiguities or inconsistencies arise between DOD, U.S. Air Force Regulations, and Civilian Codes/Standards, the U.S. Air Force Regulations shall govern.
j. In the event that certain areas of design are not specifically addressed by the U.S.
Air Force Regulations, the most conservative design requirements as defined by applicable industrial codes or association standards shall be used.
k. Code interpretations during construction shall be at the signed off on by a government representative.
l. Visit the existing facility and carefully examine those portions of the building affected by this work before submitting proposals, so as to become familiar with the existing conditions and difficulties that will affect the execution of the work.
m. Submission of a proposal will be construed as evidence that such examination has been made. During the times that the fire alarm system will be rendered non-functional in any manner, an approved/certified fire watch will be provided coordinated with the user and the inspector prior to shut down in accordance with any requirements of the Fire Marshal. The fire watch will be maintained during the times any part of the building is occupied.
i. DEMOLITION:
a) Remove all Electrical Receptacles, Safety Switches, and associated conductors back to panel board as indicated on drawings.
b) Remove all above grade raceway serving Electrical Receptacles, Safety Switches as indicated on drawings.
c) Remove all Light Fixtures, switches, and associated conductors and raceway back to panel board as indicated on drawings.
d) Remove Power Transfer Switch as indicated on drawings.
ii. NEW WORK:
a) Install New Light Fixtures, Switches, Receptacles, and Power Distribution Equipment as indicated on drawings and IAW User needs.
b) Electrical Contactor shall coordinate all drywall patching as required prior to final painting of walls. Walls may need to be opened up to run circuitry for Receptacles and Data Drops on the interior side of exterior walls.
Contractor should take special note for work required to meet new electrical construction of this area.
6. FIRE ALARM:
i. DEMOLITION:
a) Remove existing Fire Alarm Control Panel, Fire Alarm Notification Appliances, Fire Alarm Detection Devices, wiring, and Monoco Panel as indicated on drawings.
ii. NEW WORK:
a) Install new Fire Alarm Mass Notification Control Panel.
b) Install new Fire Alarm Local Indication Panel.
c) Install new Fire Alarm Mass Notification dual colored Speaker Strobes. All wiring in Conduit per MDGL.
d) Install new Pull Stations and Detection devices. All wiring in Conduit per
MDGL.
e) Submit Fire Alarm Drawings signed and sealed by a Fire Alarm Engineer with a Bill of Material for Fire Mass Notification Equipment selected including Control Panel, LOC Panel, Devices, Wiring, Floorplan, Battery Calculations, and all other info required by NFPA 72.
7. COMMUNICATIONS SYSTEMS
i. DEMOLITION:
a) Remove existing Comm Jacks as indicated on drawings.
b) Remove Comm. Wiring indicated on drawings.
ii. NEW WORK:
a) Install new Comm Jacks and Wiring as indicated on drawings and IAW User needs.
b) Install new Cat 6 Patch Panel in Comm Room to accommodate new Data lines and IAW User needs.
III. PROJECT COORDINATION
A. GENERAL COORDINATION:
1. All construction shall be properly coordinated to minimize disruptions.
2. No work shall take place on site until the AF Form 103 Work Clearance Permit and other applicable submittals are approved.
3. For construction requiring permits, work shall not commence until all permits are secured and provided to the Environmental Flight. For project involving the installation of drinking water and sanitary sewer utilities and their associated facility lines/fixtures, use of the newly constructed system shall be prohibited until approved by the regulatory agency. The only exception is for testing purposes.
4. The Contractor shall coordinate the schedule, movement and inspection of all truck convoys through Base gates with MacDill Security Forces (SFS) and Civil Engineering Squadron (CES). The Contractor is forewarned that convoy access during peak hours of gate operations may be limited. Peak hours typically occur between 0630 and 0830 hours, and between 1530 and 1730 hours.
B. PROJECT PHASING:
User will evacuate the building completely during construction so there will not be a need for phasing.
C. SURVEY MONUMENTS:
Any survey monuments damaged or removed as a part of this construction shall be replaced by the Contractor at no additional expense to the Government. A map showing the locations of the documented monuments is available from CES.
D. MATERIALS:
All materials and equipment shall be new and free of defects and/or damage from handling, storage or installation.
E. HAZARDOUS MATERIALS:
The contractor shall complete and submit the “Contractor Hazardous Material (HAZMAT) List” form (Appendix E). All forms shall be submitted electronically in PDF format. The use of any Class I Ozone Depleting Substances (ODSs) is prohibited. No HAZMAT items are to be left on site at the completion of the project.
F. GREEN PROCUREMENT:
Federal/ Environmental Protection Agency (EPA) Green Procurement guidelines must be followed. Products shall comply with requirements set by the EPA Comprehensive
Procurement Guidelines list, the United States Department of Agriculture (USDA) Bio-preferred Program, Department of Energy Federal Energy Management Program (EnergyStar, standby power, water efficiency), EPA Significant New Alternatives Program (SNAP), and EPA Priority Chemicals (PROACT). Contractor shall complete and submit a Green Procurement Determination Form (Appendix G) to identify products that are subject to these purchasing requirements and/or claim an exemption from the requirement if appropriate.
G. ENVIRONMENTAL REQUIREMENTS:
1. Environmental Management System (EMS):
a. MacDill AFB has implemented an Environmental Management System (EMS), which is based on ISO-14001, as their primary management approach in conducting operations and managing their environmental programs. Work performed by all contractors and subcontractors, shall be consistent with the MacDill Environmental Management System (EMS) policy; MacDill’s Environmental Commitment Statement is to: a) Execute our mission; b) Prevent pollution; c) Improve continuously in mission and environment; and d) Comply with all environmental regulations and Air Force requirements.
b. All on-site contractor personnel, and their subcontractor personnel, performing tasks that have the potential to impact the environmental shall have sufficient awareness and experience to realize potential environmental concerns and act accordingly to reduce potential for environmental impacts. In addition, the Contractor shall provide monitoring and measurement information as identified in the Environmental tab of the AF Form 66, "Schedule of Material Submittals" that address performance relative to environmental, energy and transportation management goals. The Contractor may contact the 6 CES/CEIE Environmental Flight for additional information regarding environmental management and reporting requirements.
2. Non-Hazardous Waste Disposal Reporting:
a. Contractor shall report non-hazardous solid waste at 50% and 100% construction completion phases using the MacDill AFB Construction & Demolition Debris Disposal Form (Appendix H). No weight tickets are to be submitted with this form however they shall be kept on file with the contractor for auditing purposes.
3. Asbestos Containing Material (ACM) and Lead Based Paint (LBP) Information:
Visually Inspected and Assumed ACM and/or LBP for removal.
a. Based on the historical asbestos and lead-based paint records specific to this facility, a visual inspection was performed and suspect materials were identified.
There is a high potential that ACM and/or LBP will be encountered during renovation and demolition activity. Suspect items specific to this scope include but are not limited to:
i. Thermal system insulation used to inhibit heat gain or loss on pipes, boilers, tanks, ducts, and various other building components
ii. HVAC mastic and /or tapes
iii. Resilient floor coverings, mastics, and adhesives.
b. Suspect building components and materials commonly known to contain asbestos and lead-based paint (as defined by the EPA) shall be removed and disposed of in accordance with Environmental Protection Agency (EPA), Florida Department of Environmental Protection (FDEP), Occupational Safety and Health Administration (OSHA) and Hillsborough County Environmental Protection Commission (HCEPC) regulations by appropriately licensed and certified workers.
c. In the event that additional suspect asbestos and or lead-based paint that was not visually accessible prior to start of work is encountered during renovation or demolition activity the contractor will cease all work that may disturb the suspect materials and contact the project contracting officer. If the suspect material is confirmed to contain asbestos or lead-based paint, the following steps shall occur.
d. Asbestos Abatement Work Plan – The plan shall be completed by a qualified State of Florida licensed asbestos consultant firm.
e. The contractor shall submit an abatement work plan to MacDill AFB Civil Engineer Squadron (6 CES) for review and approval 10 days prior to removal activity.
f. Pursuant to 40 CFR 61.145(b) the contractor will notify the Hillsborough county EPC 10 working days prior to the beginning of any demolition activities involving the disturbance of asbestos containing materials using DEP form 62-257.900(1) and will be responsible for any fees required therein. A copy of the completed form will also be submitted to 6 CES within the same timeframe for review and record keeping.
g. Asbestos Abatement – The Contractor shall hire a Florida State licensed and certified asbestos consultant to remove and dispose of the asbestos-containing building material (ACBM) in accordance with federal, state and local regulations..
h. Asbestos Abatement Monitoring – The Contractor shall hire a third party, independent consulting firm to perform environmental monitoring services during the abatement work.
i. Contractor shall provide 6CES copies of all surveys and abatement records
4. Environmental Restoration Program (ERP):
a. Ground Water Monitoring Wells
i. Groundwater monitoring wells may be located in the project area. Approximate well locations can be obtained from the Environmental Restoration (ERP) Office (6 CES/CZOE); however, more wells may exist in the project area than are shown. The Contractor shall survey the site prior to start of work for exact locations of all wells. Great care must be taken to protect all the wells found in the project area; as such wells must be identified and clearly marked;
a) If any of these wells are damaged during this project, the Contractor shall either repair or abandon and reinstall the well at the Contractor's expense, in accordance with the MacDill AFB Basewide Environmental Restoration Work Plan, Appendix A, Standard Operating Procedure (SOP) Number 6, Well Installation, Development, and Abandonment Procedures, located in Appendix 7 of the MacDill AFB Design Guide. The determination as to whether the well can be repaired or must be properly abandoned and a new well installed will be made by MacDill AFB ERP Personnel.
b) If the work is such that damage to a well is unavoidable, the well must be properly abandoned prior to construction activities and a new well installed at the Contractor's expense upon completion of construction activities. The Contractor shall coordinate the well abandonment and reinstallation activities with MacDill AFB ERP personnel to ensure that well locations are acceptable to regulators before construction activities take place.
c) Wells must be abandoned/reinstalled by a Florida licensed driller and surveyed by a Registered Land Surveyor in the State of Florida. Well locations are to be surveyed to within 1 foot accuracy using Florida State plane, West Zone, North American Datum, 1983 (NAD 83). Ground surface elevations and top of concrete pad elevations will be surveyed to within 0.1 ft. accuracy; and top of casing elevations will be surveyed to within 0.01 ft. accuracy. Elevations will be referenced to the National Geodetic Vertical Datum of 1929 (NGVD-29).
d) All field logs, permits and survey forms must be provided to 6 CES/CZOE at the completion of well abandonment/installation. The Contractor must coordinate with 6 CES/CZOE to obtain well tag specifications and ordering information.
H. PERMIT REQUIREMENTS:
1. All permitting actions shall be coordinated through the 6 CES/CEIE Environmental Flight. The Contractor shall prepare all required permit applications, including supporting documentation and permit fees, and submit application packages to the Environmental Flight for review and/or staffing for signature by the government.
Permits or regulatory coordination required for this project are understood to be the following:
a. AF 103 – Work Clearance Request Form
b. HCEPC (asbestos notification for demo)
c. Note the project exterior domestic water and sanitary sewer work is exempt from permitting requirements; however, contracting with local authority (FGUA) for inspection and service coordination as provided for below will be necessary.
I. UTILITY COORDINATION:
1. Water:
a. The Contractor shall note the Utility Operator for water and wastewater systems is Florida Governmental Utility Authority (FGUA).
b. The Contractor shall be responsible for coordination of design and construction activities with the utility operator. The coordination shall include, but not be limited to, obtaining design/construction approval, and paying for construction oversight services.
c. The Contractor shall be responsible for coordinating water system connections, terminations and inspections with the Utility Operator in accordance with the forms and estimate provided in Appendix E.
d. The Contractor is advised the proposed fee is based on a finite level of connection, termination and inspection services which are identified in the fee letter.
e. The Contractor is responsible for managing their construction activities to ensure they do not incur additional fees.
J. SITE AVAILABILITY AND ACCESS
1. Project area(s) will be available for contract work between 7:30 A.M. (0730) and 4:30 P.M. (1630) on Monday through Friday (excluding Federal holidays).
2. After hours and weekend/holiday work may be required. All work outside of the normal hours must be coordinated at least a week in advance and approved by the Contracting Officer.
IV. SUBMITTALS
A. The remaining design phases to be scheduled with the following considerations:
1. Contractor shall provide a 65% design in accordance with Design Requirements (Appendix K). Allow 20 calendar days for Government review of the 65% design submittal. Upon completion of Government review of the 65% design, the Government will provide copies of their preliminary comments to the contractor.
2. Upon receipt of the 65% design, Contractor shall coordinate with CE to schedule a 65% design review conference at MacDill AFB. Contractor shall be responsible for providing meeting minutes within five calendar days.
3. Upon Government approval of the 65% Design, Contractor shall provide the 95% Design Submittal in accordance with Design Requirements (Appendix K). Allow 20 calendar days for Government review of the 95% design submittal. Upon completion of Government review of the 95% design, the Government will provide copies of their preliminary comments to the contractor.
4. Upon receipt of the 95% design, Contractor shall coordinate with CE to schedule a 95% design review conference at MacDill AFB. Contractor shall be responsible for providing meeting minutes.
5. Upon Government approval of the 95% Design, Contractor shall provide the 100% Design Submittal in accordance with Design Requirements (Appendix K).
6. The 100% final design must be signed and sealed by a Florida registered Professional Engineer/Architect.
7. If a design is incomplete or found to be deficient the submittal will be disapproved and shall be re-submitted by the contractor without additional time.
B. Minimum Requirements for Drawing Submittals:
1. All drawings shall be in accordance with current Tri-Service Standards (Architect/Engineer/Construction CADD Standards, ERDC/ITL TR-09-2).
2. A PDF format shall be submitted in addition to CAD format (Micro-Station or AutoCAD 2017 or earlier). Most image formats will be accepted (e.g., TIFF, JPG).
3. The minimum page size shall be the page size of the documents (i.e., 11” x 17” page sizes shall be scanned at 11” x 17”, 22” x 34” page sizes should be scanned at 22” x 34”).
4. The minimum resolution for the deliverables (i.e., PDFs, TIFs, JPGs) shall be 300 dpi.
C. COLOR BINDER SUBMITTAL
1. Color Binder is defined as the project’s interior and exterior finishes presented in a binder, or set of binders, and as outlined below.
2. The creation of the Color Binder will require a Color Binder Meeting scheduled by the Contractor within 10 days of the 65% design meeting.
3. Any color selection changes after submission of the Color Binder must be resubmitted.
Individual pages may be submitted for substitution in the binder. Substituted pages must be clearly marked with the revision date, phase %, and project title and number.
4. In addition to the physical Color Binder submission, the Contractor shall include in the AF Form 3000 submission a scanned copy of the Color Binder in its entirety with the material’s manufacturer, color name, color style/number, and identifier which correlates to the Finish Schedule. All information must be legible and in color. Monochrome scans will not be accepted.
5. Color Binder
a. Submit SID information and samples in separate three ring binders with pockets on the inside of the covers. When samples are numerous or thick, use more than one binder. Large D-ring binders are preferred to O-ring binders. Fold out items must have a maximum spread of 25 1/2”. Finish binders must be in 8 1/2” x 11” format and sturdy enough to support samples. Use page protectors that are strong enough to keep pages from tearing out. Anchor large or heavy samples with mechanical fasteners, Velcro, or double-faced foam tape rather than rubber cement or glue.
Label finish samples with the material codes used in the contract documents.
b. Samples must be labeled with the finish code so they can be identified independently if removed from the binder.
c. Material and finish samples must indicate true pattern, color and texture.
Photographs or colored photocopies of materials or fabrics to show large overall patterns are required in conjunction with actual samples to show the actual colors.
Finish samples must be large enough to show a complete pattern or design where practical. For example, if the specified carpet has a large pattern, provide a color photograph showing the overall pattern in addition to the carpet sample with representative colors.
d. Each binder must be labeled on the outside spine and front cover with the following information: Phase %, Date, A-E firm, Project Title and Number, Location and Volume number. Include the Color Schedule or the Room Finish Schedule and Finish Color Schedule from the drawings. The contractor must coordinate the binder format with the installation design guides where applicable. .Five Percent (5%) of the Contract Value will be withheld as a final payment until all project deliverables such as As-Built Drawings, GIS Data, Final 1354, O&M Manuals, and Training Material are submitted to and accepted by the Government.
D. All material submittals shall be submitted and approved as part of the Phase I, Design Phase in accordance with the AF Form 66 (Appendix C).
E. Upon receipt of each material submittal the Government has 20 calendar days to review and respond. The Contractor owns burden of managing project time and of soliciting material submittal approval in accordance with project schedule needs.
F. All submittals shall be in both PDF format and hard copies.
G. All submittals with an asterisks (*) next to the line number on the AF Form 66 require the contractor’s Design Engineer (Designer of Record or Engineer of Record) to review and approve the identified submittal by printing and signing their name in the “DESCRIPTION OF MATERIAL” section of the AF Form 3000.
H. Each AF Form 3000 shall be numbered sequentially in the “SUBMISSION” block (e.g. 1 for first submittal, 2 for second, etc.). Ensure each item listed on the AF Form 3000 uses the same alpha-numeric identifier defined on the AF Form 66 for that particular item.
When providing digital files, use the following naming convention: Project Number -submittal number-AF 66 Item Number”. For example, the second submittal for project 11- 0001 AF 66 Item Number G3 Progress Schedule submittal would be named: “11-0001- Sub2-G3”. If a re-submittal is required, the new file name would add an R1 (for the first re-submittal), R2 (for the second), etc. For example, the first re-submittal for an item disapproved from the above example would be named: “11-0001-Sub2R1-G3”.
I. DD Form 1354 Submittal
1. Completion of DD Forms 1354 shall be in accordance with UFC 1-300-08, Criteria for Transfer and Acceptance of DoD Real Property. The 2016 AFCEC/CIT Real Property Category Codes book will identify all new capital assets (Category Codes) to be used in the DD Form 1354. The most current SAF-MIL(A)7115 for MacDill AFB identifies current capital assets (Category Codes) being renovated or repaired to be used in the DD Form 1354. Instructions for completing the DD Form 1354 are included on the DD Form 1354 and UFC 1-300-08.
2. Three DD Form 1354s are required:
a. The Draft DD Form 1354 is required at the 95% Design submittal.
b. The Interim DD Form 1354 is due upon request for Pre-Final Inspection.
