GENERAL PROVISIONS

FOR

MULTIPLE AWARD CONSTRUCTION CONTRACT (MACC)

1. DEFINITIONS:

a. 35% Design – In general, documents which show the essential components of the proposed design and convey that the contractor fully understands the Task Order (TO) requirements and that his / her proposed price includes all required elements of work to complete the project. The documents will include the following minimum information, as applicable to the project; drawings, a list showing sections to be incorporated in the specifications, and any other design components relating to the 35% design as identified in the Task Order. Design review periods will be stated on individual task orders. Major mechanical and electrical equipment items shall be shown on the drawings with single line diagrams. All utilities and structures shall be shown. Drawings shall also include the following, as applicable:

(1) All required controlling dimensions. Site plan showing proposed location of work including benchmarks, access from main roads, and surface or sub-surface drainage concepts.

(2) Floor plans with dimensions and functional arrangement of all areas; including corridors, exits, stairs, and utility spaces properly related to exterior access, roads, parking, and service areas, etc. Individual treatment shall be given items involving special design and/or deviations from accepted standards and of complex design. Gross floor areas shall be shown for each floor and for the entire building. Floor and roof framing, and loadings shall be indicated.

Built-in, installed, and specialized portable equipment shall be indicated to scale. Pertinent information regarding fire prevention and safety requirements shall be shown. Design live and wind loads shall be shown.

(3) Elevations shall include story heights, fenestration, and adaptation to finished grades at the site.

(4) Sections of structures, including framing, partitions, suspended ceilings, (if any), ducts, etc., shall be shown.

(5) Typical wall sections shall be shown at a scale of no less than 3/4 inch = 1 foot. Sections shall include material and thicknesses, methods of attachment, and type of windows with relation thereof to supporting structural columns or walls.

(6) For heating, air conditioning, mechanical ventilation and plumbing, and special features such as elevators, hoists, kitchen equipment, etc., indicate the location, capacity, and space requirements for all major items of mechanical equipment. Single line indication and riser diagrams of ducts, pipes, and equipment and their approximate location shall also be shown. Indication shall be made as to whether piping and duct work are exposed or concealed.

Functional areas where pipes are exposed shall be delineated. Approximate operating range or capacity for heating, ventilation, air conditioning, and refrigeration equipment shall be clearly stated.

(7) For electrical systems, indicate lighting arrangements, type of fixtures proposed, general light intensities, special electrical requirements of the user, including communications and electronic facilities where applicable, light and power service entrance and distribution arrangement. Conduits, including those to be wired by others, shall be shown together with indication as to whether conduits are exposed. Functional areas having exposed conduits shall be delineated. Riser diagrams, showing service equipment, feeders, and panels, other than branch circuits, shall be shown. Cable sizes, current demand factors and the switch and panel board descriptions are not required at this time. Location capacity and space requirements of all major items of equipment shall be shown including interior substations with service equipment and panels. Power, communication, and electronic features will be indicated in sufficient detail to identify requirements clearly and to indicate the means of satisfying the requirements.

(8) Provide separate lists of all Government-furnished equipment to be installed by the contractor and equipment furnished and installed by the Government, if applicable.

b. 65% Design – Documents building upon the 35% design, including drawings and the complete text of the specifications.

The drawings will show an additional 30 % progress in all disciplines which may include site plans, floor plans, elevations, wall sections, details, and all other elements as identified in the 35% design. All cut sections and details shall also be shown on the drawings at this time. Any updated or corrected design analyses of architectural, structural, civil, electric, mechanical, and utility systems shall be included. The contractor shall stop work until the Contracting Officer issues a Notice to Proceed to the next design milestone.

c. 95% Design – Documents building upon the prior design submittal. This will include, for review, final and complete drawings, and specifications. All project requirements identified in the most current TO revision, site visit minutes, modifications, or review comments must be incorporated.

d. 100% Design / As-built Drawings – Documents building upon the prior design submittal which include final contractor accomplished corrections based on the Government's review of the prior design documents. As-built drawings must be in accordance with the latest version of the Shaw Design and Construction Standards.

e. Best Value – A process used to evaluate proposals based on the quality of the overall design in addition to the other criteria included in the Fair Opportunity Proposal Request (FOPR). Task Orders issued under the MACC IDIQ will follow the Fair Opportunity Proposal Procedures in FAR 16.505. Evaluation methods will be identified on each Fair Opportunity Proposal Request (FOPR) issued. Evaluation methods could include any of those identified in FAR Part 13, 14 and 15 to include best value. When used, the “best value” evaluation factors will be included in the Task Order and may vary between projects. Award may be made to other than the low offeror.

f. Beneficial Occupancy Date (BOD) – The mutually established date by the Contractor, Inspector, Contracting Officer, and the user in which the government takes ownership of all or part of the construction area. Typically punch list items and minor construction activities remain.

g. Color Board – A visual presentation consisting of samples of the various finishes, grouped together based on location within the project. Samples shall be of adequate size to show the full pattern, texture, and color of each item.

h. Conventional Design-Build – The process by which the same contractor provides both the design and construction services for a project, completing the design before construction activity begins.

i. Design Costs – Those costs identified by the contractor, on the Bid Schedule and / or AF Form 3052, as part of the RFP process which represent the portion of the overall project cost attributed to preparation of the 100% design and construction documents.

j. Designer of Record (DOR) - The individual accountable for specific portions of the design documents. Where professional certification is required for the design, the DOR would be the individual providing certification for that portion of the design. Where professional certification is not required, the DOR would typically be the individual who provided the design or specifications for a particular aspect of the project (i.e.: architectural, civil, structural, mechanical or electrical).

k. Fast Track Design-Build – The process by which the same contractor provides both the design and construction services for a project, beginning construction at a mutually agreed point prior to completion of the design and construction drawings.

l. Government Furnished Equipment (GFE) / Government Furnished Material (GFM) – Items of equipment or material which the government will supply to the contractor for use in the construction of a project.

m. Investigative Costs – Those costs identified by the contractor, on the Bid Schedule and / or AF Form 3052, as part of the RFP process which represents the portion of the overall project cost attributed to all investigative services necessary to prepare the 100% design and construction documents.

n. Investigative Services – For exterior work / new facilities, the investigative services will typically include performing a survey of the project site including topography, cross sections, drainage survey, and bench level circuit. In addition, it may include verification and documentation of the location and size of all existing utilities and pertinent surface features for inclusion on the topographic survey map. For interior work, the investigative services will typically include verification of all existing conditions affecting the design and execution of the work as described in the TO.

o. Invitation for Bid (IFB) – A package of documents issued to a contractor requesting a firm price for construction and the required design documents for a project based on the Task Order. Once submitted, no revisions to the bid are permitted. Award is made to the low bidder.

p. Professional Certification – The process of affixing a seal or statement to a drawing or design document to indicate that the work to which it is affixed has been performed by a person licensed as a professional, in that area of expertise, in the state of South Carolina.

q. Request for Proposal (RFP) – A package of documents issued to the contractor requesting an itemized cost proposal for construction and design documents for a project based on the Task Order. Following submittal of the proposal and at the government’s option, discussions may be held with the contractor to clarify and / or negotiate elements of the proposal.

r. Statement of Work (SOW) – A document prepared by the government and issued to the contractor as part of the task order. Included in the SOW are the specific technical requirements for the project.

s. Task order (TO) – A document prepared by the government and issued to the contractor which contains the general requirements and parameters of the proposed project. Included in the TO is the Statement of Work defining specific technical requirements for the project. Also included will be the design certifications and drawings required to be prepared by the contractor.

2. DESCRIPTION OF WORK:

a. Contract Scope:

The MACC will be utilized to execute a broad range of maintenance, repair and minor construction projects on real property at Shaw AFB, South Carolina to include Poinsett Electronic Warfare Range which is located approximately 15 Miles from Shaw AFB, and Wateree Recreational Area located approximately 45 miles from Shaw AFB. Sites will be identified at the time of any task order bid. The projects will include tasks in a variety of trades such as demolition, site work, excavation, cast-in-place concrete, masonry, structural steel, electrical, mechanical (including HVAC), painting, millwork, and other specialty and general contracting work. (NOTE: The military family housing (MFH) contractor maintains all military family housing units. Military family housing maintenance applies to all housing units including all interior/exterior components and accessories, fences, garages, carports, storage sheds, playgrounds, tennis courts, basketball courts, and bus shelters. These components are not included under the MACC contract)

b. Proposal Preparation

Task Orders issued as a result of the FOPR may be awarded to more than one contractor or to a single contractor. Each FOPR issued will include a Requirements Summary, definition of the project requirements and other documents, as necessary, to give the contractor a complete scope of what is needed. The project may ultimately require the contractor to produce a full design, partial design, or no design depending on the level of design already available and provided to the contractor in the FOPR. For example, a new project with no, or partial, previous design accomplished may include requirements to provide a 35% (minimum) design as part of the contractor’s proposal. Conversely, a project for which a 100% design is already available will only require a bid in the prescribed format. If awarded a TO, as a result of the FOPR, for the project, the successful contractor will, as necessary, complete the design to the 100% level. This will include participating in design reviews, conducted at Shaw AFB, at each design milestone (35%, 65%, etc.) described in the TO, if required. Proposals will be submitted on the bid schedule provided by the Government. When required by the FOPR, a construction cost estimate breakdown may also be required. Cost estimate shall be submitted using the government provided construction cost estimate worksheet. In addition, the TO may also include, but is not limited to, the following, as applicable, based on the level of design required:

(1) Government prepared Statement of Work, sketches, and any available as-builds, specifications, or design information

(2) Design calculations requirements

(3) Construction drawings requirements including professional certifications

(4) Direction regarding fast-track vs. design-build

(5) Timeframes for preparing the proposal, completing design work (if awarded), and completing construction.

(6) Contractor prepared specification sections and version required

(7) Submittal requirements

(8) Government Furnished Materials (GFM) or Government Furnished Equipment (GFE)

c. RESPONSE REQUIREMENT

The primary goal of MACC is speedy acquisition of Civil Engineering requirements, contractor responsiveness is paramount.

Once the Contracting Officer notifies the contractor of a requirement, the contractor is expected to respond expeditiously and prepare the proposal. The Contracting Officer will identify the requirement as either routine or emergency.

1. Upon notification of a routine requirement, the contractor shall respond to the needs of the Government within seven to ten working days by

(i) Visiting the proposed work site accompanied by the Contracting Officer or

(ii) Establishing verbal contact with the Contracting Officer to further define the scope of the requirement

2. Questions arising from specific FOPRs will need to be addressed in writing to the Contracting Officer no later than the time and date specified in each individual FOPR. If questions are not received by the deadline, they will not be answered.(i) In the event of a short lead time between the scheduled site visit and the questions due date, the Contracting Officer may extend the question response due date.

3. In the event of an emergency requirement, the contractor’s response time will be as soon as directed by the Contracting Officer. The contractor or his/her designated representative, who is able to make decisions and obligate the company, must be readily available to communicate with the Contracting Officer.

d. Proposal Evaluation Criteria:

Proposals received will be evaluated based on compliance with the FOPR requirements and evaluation procedures outlined in each FOPR issued. FOPRs will be issued IAW FAR 16.505 which affords the government the latitude to use proposal evaluation methods from across the FAR. Price or cost will always be one of the factors evaluated in the selection decision. The government reserves the right to enter into discussions with responsive contractors (those submitting technically acceptable proposals) for the purpose of requesting minor revisions to the proposals.

e. Access to Work Areas:

The contractor may be required to work within controlled or restricted areas. Refer to paragraphs 19 and 20 for additional information concerning security.

3. CONTRACTOR OPERATIONS: The Contractor shall conduct all operations in accordance with Shaw AFB Design Guide (Attachment 03) and the provisions listed below:

a. AF Form 103 – The contractor is to complete the AF Form 103 in accordance with the Shaw Design Standards.

b. Construction Signage – The contractor shall furnish, install and maintain traffic control signage for street and highway construction operations per the requirements of the Manual on Uniform Traffic Control Devices for Street and Highways.

Prior to the contractor beginning construction, a detailed traffic control plan showing all construction signage shall be submitted to the contracting officer for review and comment.

c. Contact List – The contractor shall provide a list of employees and contact info to be displayed in the contract management office in the event of an emergency or inclement weather. The list should be updated at a minimum every 6 months.

d. Contractor Damages – Areas not identified to be removed and/or worked under this contract that are disturbed or damaged during construction shall be removed and replaced at no additional cost to the Government.

e. Contractor’s Haul Routes – Access to the project will be by the SAFB Commercial Vehicle Gate during the week (Monday through Friday). The SAFB Commercial Vehicle Gate will open when prearranged during weekends (Saturday and Sunday) Federal Holidays, and family days thence over the haul route indicated on the Title Page of the drawings.

The designated routes shall be used by both loaded and unloaded vehicles. The haul route shall be followed by contractor vehicles when entering the base, when exiting the base the vehicle should use the nearest exit.

f. Existing Conditions – After award, the contractor shall video all work areas prior to starting work and run all systems that are to remain prior to starting the project. For example if a fire alarm panel is being tied to the contractor shall coordinate with the responsible shop and the user to test the existing fire alarm panel to ensure it is in good working order prior to starting the project. If existing equipment is found to not be working correctly, the contractor shall notify the contracting officer and the Government inspector.

g. Mobile Phone – The Contractor shall provide an emergency site representative (superintendent or foreman), throughout the execution of the project, shall be accessible via a mobile phone.

h. Monthly Meetings – Monthly status meetings will be held with the MACC contractors on a rotating basis. The contractor shall provide meeting minutes a maximum of 3 working days after the status meeting for approval. If corrections are requested, the contractor shall make necessary corrections and provide for approval. The approved meeting minutes will become official record for the project.

i. Notice of Inspection – The contractor shall provide 48 hour notice for specialized inspections and witnessing of testing, e.g. a close-up inspection on walls or DALT testing for ductwork.

j. Notification Requirements (USED FOR HOUSING AREAS ONLY):

(1) Following award of the contract for the project, the Contractor will notify the housing manager of the upcoming work and the residents' responsibilities. For exterior work, the contractor will be responsible for providing each notification at least (14) calendar days prior to (1) when the road in front of their unit will be closed to traffic and / or (2) when work on their yard will begin. Further, the contractor shall provide notification at least (48) hours prior to shutting off utility service and what units it will affect. The notice shall clearly state the dates of road closure, utility service interruption, or when yard work will begin. For interior work, the contractor will be responsible for providing notification 14 calendar days in advance of the work and a follow-up notification five calendar days before work starts. In all cases, a brief description of the work, how long the particular item of work is expected to last, and points of contact for the Air Force and Contractor shall also be stated (the Air Force point of contact will be provided at the pre-construction conference for each project).

k. Progress Schedule and Progress Report – The contractor shall submit an AF Form 3064 (progress schedule) and schedule of values within 7 calendar days of the issued NTP or as specified by the Contracting Officer. The contractor shall submit and AF Form 3065 (progress report) and schedule of values every two weeks up the receipt of the approved progress schedule.

l. Site Maintenance - The contractor shall at all-time keep the premises free from accumulations of waste materials or rubbish caused by his employees or work. At completion of each work day, the contractor shall remove all rubbish from the construction site. All of the contractor’s tools, equipment, and surplus materials, etc., shall be stored in the storage area or temporary staging area. Grass and weed growth in excavated materials that are stockpiled on site shall be controlled and/or cut weekly. Weeds and grass within the construction site shall also be controlled and/or cut weekly.

For the duration of each TO and on a daily basis, Contractor shall remove from all base roads and/or streets mud, soil, rocks, trash and debris that result from his/her construction operation on base. All roads and/or streets affected shall be cleaned within 1 hour. Contractor or Contractor’s employees, including subcontractors, shall not park any personally owned vehicles (POVs) or company and/or delivery vehicles on grassy areas near or in the construction site without prior approval from the CO, or his/her designated representative. When allowed, parking on grassy areas will only be for short-term delivery purposes (to include heavy tools, equipment, construction materials, etc.). Use designated hard surfaces or existing parking lots near or within the project site for parking of POVs or company and/or delivery vehicles. Permission to park on grassy areas does not relieve the contractor of the responsibility to restore affected areas to original condition.

Tire and track impressions (ruts) created on wet or soft soil by vehicles and/or equipment used in the Contractor’s operation shall be repaired. Contractor shall restore the affected area(s) to its original condition within a timely manner.

Final payment shall not be made until all repairs have been completed to satisfaction of the Contracting Officer. When areas of the site are raked prior to seeding (or sodding) or for leveling of topsoil to rid the site of mounds, clods and/or ruts, the Contractor shall ensure that no clods larger than 1” are left on the site. Contractor shall restore the site to a condition and appearance similar or equal to existing before the damages occurred. Such restoration work will not be considered complete until approved by the CO or his/her designated representative. The Government will not accept sites that have not been raked and restored to this standard. At any given construction site, the Contractor shall grade, rake, and seed (or sod), or prepare for seeding (or sodding), the job site as soon as possible following work at that particular site. Grass areas disturbed by construction and/or those areas required to be “re-seeded” shall be re-established by hydro seeding only, unless otherwise specified.

m. Storage Units – The contractor shall request the Contracting Officer’s approval for the siting, size and color of storage units, CONEX boxes, office/storage trailers and dumpsters. The Contractor shall not bring any such items to the job site without the Contracting Officer’s approval of all particulars noted above. A nameplate shall be visually displayed on the exterior of all storage units, with the following information: Contractor’s Company, name for Contractor’s Point(s) of Contact (POC) and telephone number(s) where he/she can be reached, (during and after normal working hours).

n. Utility Outages - The Contractor shall perform the work under this contract with a minimum of outage time for all utilities. If possible, the contractor shall schedule any work that requires utility outages (electrical, water, gas, etc.) which will close down or limit normal activities in the building, construction area, or other affected areas at a time other than regular work periods of the organization occupying the facility including nights, weekends and holidays. The Contractor shall be responsible for coordination of all utility outages required to execute the contract. This will include execution of a government furnished utility outage coordination form, which must be reviewed and signed by representatives of all appropriate base organization prior to approval by the Contracting Officer’s representative. The Contractor will be provided a list of building managers and other appropriate points of contact, and will be given assistance by the government on an as needed basis. It shall be the Contractor’s responsibility to initiate and coordinate utility outages forms. Coordination of the outage form must be completed a minimum of fourteen (10) working days prior to the requested outage date, and may require coordination with as many as twenty (20) individuals located on or near base.

Any unusual delays in obtaining clearance from any particular organization shall be reported immediately to the Contracting Officer or Contracting Officer’s representative for assistance. Electronically submit one (1) copy of the completed utility outage form to Civil Engineering construction management. Following completion of the outage coordination, the contractor shall request shutdown from the utility company or Civil Engineer shop responsible for the service. All base owned utilities shall be turned off by the responsible shop. The Contractor shall pay any fees associated with the required shutdown and shall comply with all instructions pertaining to the shutdown. Once work begins on an approved outage, work must continue without interruption until the contractor restores utility services to the affected lines and/or facilities.

o. Work Schedule – Working hours for the contractor will normally be between the hours of 7:30 a.m. and 4:30 p.m.

excluding Saturdays, Sundays, federal holidays, and family days. If the contractor desires to work during periods other than above, additional government inspection forces may be required. The Contractor shall make a request via email to the Contracting Officer three work days in advance of their intention to work outside the standard work schedule to allow assignment of additional inspection personnel. The request shall state each project name and number and the work that is being accomplished. The Contracting Officer shall determine, at his discretion, whether such inspection forces are available and whether work will be allowed on a non-scheduled day. The Contracting Officer may authorize the contractor to perform work during periods other than normal duty hours and/or days; however, if inspectors are required to perform in excess of their normal duty work schedule solely for the benefit of the contractor, the actual cost of inspection at overtime rates will be charged to the Contractor. These adjustments to the Contract price may be made periodically as directed by the Contracting Officer.

4. COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK: The contractor shall commence work and complete the project in accordance with the time frames established in the Task Orders (TO) and Notice to Proceed (NTP).

5. DESIGN DOCUMENTS, GENERAL: All design submittal shall be sealed and signed by the engineer of record. The design documents developed by the contractor shall also comply with the following requirements:

a. Drawings:

(1) All drawings shall be sealed, signed and dated by a Professional Engineer (PE) and/or Registered Architect (RA). The sealer shall be the design professional in the area covered by the sheet(s) of the drawings they seal. The PE for fire suppression systems must be registered in South Carolina.

(2) The contractor shall receive one copy of the appropriate record drawings if available, with the TO. The Government does not guarantee the accuracy or adequacy of the existing as-built drawings. It is the Contractor’s responsibility to field-verify all as-built drawings during preliminary site visits, planning of the project, and prior to issuance of each TO.

Contractor shall be responsible for the reproduction of these drawings if multiple copies are needed.

(3) The contractor shall submit one (1) hardcopy Standard D (24” x 36”), one (1) hardcopy Standard B (11” x 17”), bound

PDF, and Auto CAD files with each design submittal. Contractor developed drawings shall be prepared and submitted utilizing Auto CAD 2017 (or earlier version) computer aided drafting software and all drawings shall be in the same electronic format. The Shaw AFB cover sheet and title block sheets shall be used, templates for which will be provided to the Contractor in Auto CAD 2017 (or earlier version). All drawings shall be prepared in Standard English scale.

Once the contractor is approved for demolition or construction the Contractor shall submit two (2) sets of Standard D and B size drawings (or as otherwise required by the TO) to the Contracting Officer. Throughout the design process, each submittal shall be clearly identified as to the level of completion (i.e.: 35% etc.). The “revisions” blocks on the plan sheets shall not be utilized to document changes to the drawings until after the 100% design is completed and accepted by the government.

(4) The Contractor shall prepare drawings, as specified in the TO, for Government approval prior to the start of construction.

The drawings developed by the contractor beyond the 35% point shall conform to industry standards relating to good design and drafting practices. All completed drawings shall clearly identify the work being performed under the TO.

Unless otherwise identified in the TO, use of existing drawings provided by the government will be allowed provided they are modified by the contractor to accurately reflect the project requirements. It is not necessary for each drawing to be on an individual sheet, more than one drawing can be included on a sheet. Each sheet shall have an approved title block and border with professional certification when required by the TO. The title block shall contain project information including but not limited to Project #, TO #, Project Name, and submitted date. Basic design criteria utilized for each discipline will be included on the appropriate drawings as required by the TO. Examples would include wind and live/dead loads assumed for structural elements, function specific conditions such as lighting levels or future use considerations for electrical systems, site specific climatic design conditions for mechanical systems, and International Building Code analysis (performed to the same level as if being applied to a building permit process). The following is a list of drawings which might typically be required under this contract:

b. Specifications:

(1) Specifications for the project shall be identified on the drawings unless otherwise requested in the TO. For projects requesting specifications, the technical specifications to be utilized by the contractor shall be Masterspec, Short Form or Long Form versions. In the TO, the government will identify the specification sections, which represent major elements of work and, as a minimum, are to be included in the project. The contractor shall submit (if required) the specifications electronically with the first design submittal, as well as one (1) hard copy.

(2) In addition to the technical specifications described above, the latest version of the following references is to be utilized in the preparation of project designs;

Shaw AFB Design Standards Shaw AFB Architectural Design Standards American Society of Testing and Materials (ASTM) Standards American National Standards Institute (ANSI) Standards National Fire Protection Code (NFPA), with particulate attention paid to:

NFPA 101, Life Safety Code NFPA 70, National Electrical Code (NEC) Unified Facilities Criteria (UFC), with particulate attention paid to:

UFC 3-600-01, Fire Protection Engineering For Facilities UFC 4-010-01, DoD Minimum Antiterrorism Standards for Buildings UFC 4-021-01, Design and O&M: Mass Notification Systems International Building Code ANSI C-2, National Electrical Safety Code (NESC) ANSI/ASCE 7, Minimum Design Loads for Buildings and Other Structures

As well as any applicable manufacturer or industry specifications or recommendations associated with material, supplies, or systems. If a discrepancy exists between these documents, the contractor shall bring this to the attention of the Contracting Officer and shall utilize the most stringent requirement(s) in the preparation of the project design.

(3) A copy of Unified Facilities Criteria (UFC 3-600-01) Fire Protection Engineering for Facilities and the Design

Compatibility Standards – Shaw Air Force Base are included in the Appendix. (Note that due to the size of the electronic files, the photos have been deleted from the Design Compatibility Standards – hard copies of the document, including the photos, will be provided to the contractor following contract award.) The contractor shall have a copy of each of the other documents identified in paragraph (2) above available for use during the term of this contract.

(4) The bottom of each page shall have the following relative information indicated in the format noted:

(6) The use or specification of materials not made in the United States is prohibited. Only American made products shall be used.

(7) Submittal Schedule: An AF Form 66 (Attachment 11) is required, submittals shall be listed on this form and the form included in the design submittal, ensuring all items requiring submittals are annotated. The contractor shall provide electronically. Reference each submittal item on the form to the specification section and paragraph. Consecutively number each page of the submittal, e.g. “Page 1 of 1”. Use all line number rows on one page before starting another.

Number all submittals consecutively beginning with “001” for each project. A copy of the AF Form 66 is included in the Appendix.

c. Design Analysis:

(1) As specified in the TO, design calculations may be required, to include code references, as applicable for each discipline, to show compliance. Calculations shall be provided by the contractor electronically and included with each required design submittal. The following is a list of calculations which might typically be required under this contract

d. Perspective Drawing:

(1) When required by the TO, the contractor shall prepare a black line concept perspective and/or a rendered (color) perspective of the project. (May be computer or hand generated.)

(2) The perspectives shall be approximately 14” x 28” in size and shall be mounted on a standard presentation board, complete with a small site plan and supporting data (i.e. size, cost of landscaping, site work, building, etc.) affixed to the presentation board.

(3) The black-line concept perspective shall be provided with the contractor’s bid/proposal as part of the 35% submittal.

The rendered perspective shall be prepared by the successful contractor and delivered with the final design submittal when requested.

e. Minimum Design Submittal Requirements:

Design Minimum submittal requirements

35% Submittals submitted shall include, but not be limited to:

1. Draft DD Form 1354, Transfer and Acceptance of Military Real Property.

2. Asbestos Survey

3. Any site survey information and details with existing and final grades for specific areas.

4. Demolition plans.

5. Existing and proposed interior room layouts.

6. Location, size and route for all exterior equipment and utilities (gas, water, sanitary sewer, electrical, telephone). Show exact location where the utilities will enter the facility.

7. All interior finish proposed schedules to include specifications on the finish schedule.

8. Any design, calculations or equipment submittals.

9. Initial phasing plan

10. Any items deemed necessary by the Contractor to allow for the continuation of the design.

65% Submitted shall include, but not be limited to:

1. Completed phasing plan

2. Updated Draft DD Form 1354.

3. Complete asbestos report

4. Demolition plans

5. All interior room layouts.

6. Location, size and route for all exterior equipment and utilities (gas, water, sanitary sewer, electrical, telephone). Show exact location where the utilities will enter the facility.

7. All interior finish proposed schedules to include specifications on the finish schedule.

8. Major interior construction/finish details.

9. All building HVAC equipment layout, location and some details. Sizing calculations for unit(s) shall also be provided at this time.

10. Any special systems, such as ventilation and/or DDC, shall be submitted complete.

11. The complete fire alarm and mass notification system shall be provided.

12. All building electrical equipment layout location and details. Service sizing and major electrical equipment shall be shown at this time. Layouts for any new receptacles or updated receptacles.

13. Any design, calculations or equipment submittals deemed necessary by the Contractor to allow construction to begin.

14. Any items deemed necessary by the Contractor to allow for the continuation of the design.

15. Color Boards: Color boards shall be provided for all finishes. Finishes to be included, but not limited to, paint, laminate, tile, wood finish, etc.

95% Submittals shall include, but not be limited to:

1. All interior, non-load bearing construction including details.

2. Complete HVAC design.

3. Complete DDC system.

4. Complete fire alarm system (panels, smoke detectors, carbon monoxide detectors, etc.).

5. All interior finishes and covers. This shall include a color board showing all previously selected and proposed colors for the facility. Color board shall consist of the arranged, mounted and labeled color samples on a ¼” foam board.

6. Any and all remaining design, calculations or submittals that are necessary to provide a complete and constructible design.

100% Provide after construction completion:

1. All O&M manuals

2. Completed DD Form 1354, including all costs and quantity breakouts with warranty spread sheet

3. All design & construction documents including all as-built drawings, field modifications and design corrections made during the process of construction. All drawings shall meet the Shaw AFB design standards and be in AutoCAD V2017 or earlier format. If specifications were provided as part of the deliverable, they shall be in Microsoft Word doc file format. Submittal shall be provided on a 5¼”

CD-R.

4. All utilities shall be surveyed and point files provided with the points for GIS input.

6. MATERIAL APPROVAL SUBMITTALS

a. Technical Data, Samples, and Color Boards:

All equipment, products, material, and articles requiring approval, as indicated on the Submittal Schedule, shall be submitted by the contractor using AF Form 3000. All wall covering, carpet and other fabric type submittals will show the full pattern of material as a minimum. When required by the TO, all applicable product samples shall be submitted on a color board grouped to correspond with the anticipated installation. i.e.: floor covering, paint, paneling, chair-rail, wall covering and ceiling tile. The contractor shall include technical data with the products presented on the color board so that the items may be approved for technical compliance as well as architectural compatibility. Contractor is responsible for identifying and locating required materials for submittals to include matching any existing materials.

b. Designer of Record (DOR) Approval:

When required by the TO, the DOR shall review and approve equipment, product, or material submittals and indicate compliance with the design documents prior to the submittal being forwarded to the government for review. Those submittals requiring DOR approval shall be included on the Submittal Schedule. (Re: Paragraph 5.b. (7) above)

7. AS-BUILT DOCUMENTS:

a. Drawings:

During the progress of the work, the contractor shall keep a careful record at the job site of all changes and deviations to the layouts shown on the drawings. The contractor shall manually enter such changes and corrections on record drawings promptly. If the project was designed by the government, submittal and approval of the “red-line” as-builds and GPS data referenced below, at the project’s conclusion, completes the contractor’s responsibility for this item. If the project was designed by the contractor, he/she shall provide the Government with a draft of the as-built drawings in Standard D size, on paper for review at the project’s conclusion. Following government approval of the draft, the contractor shall submit the GPS data referenced below and the final as-built drawings; and a CD containing the Auto CAD 2017 (or earlier version) as-built drawings as well as a bound set in PDF format. The drawings shall indicate, in addition to all changes and corrections, the actual location of all subsurface utility lines.

b. GPS Data:

The contractor shall furnish survey data showing the horizontal location of all site improvements constructed under the TO to include, but not limited to, buildings, pavements, utilities, and all similar construction. Site improvements such as buried utility lines require one survey shot every 50’ of straight line, at each turn or bend in the line, hydrants, fittings, valves, valve boxes, curb stops, relocations and crossings of existing utilities, structures, or other appurtenances relocated or affected by the construction. Also provide elevations for the topmost point of visible appurtenances such as fire hydrants and valve boxes. The contractor shall use conventional surveying methods such as total station, Global Positioning System (GPS) for field data collection at an accuracy of +/-2cm. Data delivered in a format other than ESRI geodatabase must have an external spatial reference (.prj) file attached that specifies the parameters of the coordinate system in standard ERSI format. All electronic data deliverables shall be a CD-ROM in format that conforms to the latest version of both the CADD/GIS Technology Center’s Spatial Data Standards and A/E/C CAD Standards. These standards can be found at http;//tsc.wes.army.mil/products/. A copy of all survey data deliverables shall be a submittal item. The survey data shall be tied to the 1998 Shaw AFB Control system (NAD83 Datum); location of the control points can be obtained from SAFB Civil Engineering.

8. OPERATION AND MAINTENANCE MANUALS:

For those projects which include mechanical / electrical / specialty systems requiring periodic or routine maintenance, the contractor shall prepare and submit operation and maintenance manuals in two (2), bound, hard copies as well as two (2) electronic copy on CD ROM by discipline (mechanical, electrical, structures, etc.). As a minimum, the manuals shall contain technical data, diagrams, parts listings, maintenance instructions, and the name, address, and phone number of the manufacturer and servicing / replacement parts source. These manuals shall be submitted prior to the final inspection.

9. WARRANTY OF CONSTRUCTION:

a. Scope of Coverage:

In addition to any other warranties in this contract, the contractor warrants that work performed under this contract conforms to the contract requirements and is free of any defect in equipment, material or design furnished, or workmanship performed by the contractor or any subcontractor or supplier at any tier. This warranty shall not limit the Government’s rights under the Inspection and Acceptance clause of this contract with respect to latent defects, gross mistakes or fraud.

b. Period of Coverage:

This warranty shall continue for a period of one (1) year from the Beneficial Occupancy Date (BOD), unless otherwise specified in the individual TO. If the Government takes possession of any portion of the work before final acceptance, this warranty shall continue for a period of one year from the date of beneficial occupancy.

c. Remedies of Defects:

The contractor shall remedy, at the contractor’s expense, any failure to conform or any defect. In addition, the contractor shall remedy, at the contractor’s expense, any damage to Government-owned or controlled real or personal property when that damage is the result of:

(1) The contractor’s failure to conform to contract requirements or

(2) Any defect of equipment, material, workmanship, or design furnished.

The contractor shall restore any work damaged in fulfilling the terms and conditions of this clause. The contractor’s warranty with respect to work repaired or replaced will run for one year from the date of repair or replacement.

d. Notification to Contractor:

The Contracting Officer shall notify the contractor, in writing, within a reasonable time after the discovery of any failure, defect, or damage. If the contractor fails to remedy any failure, defect, or damage within a reasonable time after receipt of notice, the Government shall have the right to replace, repair, or otherwise remedy the failure, defect, or damage at the contractor’s expense.

e. Subcontractor and Supplier Warranties:

With respect to all warranties, express or implied, from subcontractors, manufacturers or suppliers for work performed and materials furnished under this contract, the contractor shall:

(1) Obtain all warranties that would be given in normal commercial practice

(2) Require all warranties to be executed, in writing, for the benefit of the Government if directed by the Contracting

Officer

(3) Following notification by the CO in writing, the Contractor shall respond to a warranty service requirement as identified in accordance with the “Warranty Service Priority List”. This list prioritizes warranty work into the following categories:

a. Emergency: deemed by the CO in agreement with the Base Civil Engineer such as a life-threatening situation caused by a utility outage to Shaw Hospital/Clinic or any other items deemed hazardous to the safety of personnel.

b. Urgent: all utilities, including HVAC, plumbing and electrical. Also, intrusion alarm systems, all roof leaks, kitchen equipment, and fire detection/protection and sprinkler systems.

c. Routine: all other areas.

(4) Warranty of Construction FAR 52.246-21, Subparagraph e and f. Definition of “within a reasonable time” applies as follows:

a. Emergency: perform on-site inspection to evaluate situation, determine course of action, and verbally notify CO or Government Inspector of the course of action. Initiate work within twenty-four (24) hours and work continuously to completion. Upon completion follow up in writing what action was taken.

b. Urgent: perform on-site inspection to evaluate situation, determine course of action, and verbally notify CO or Government Inspector of the course of action. Initiate work within five (5) days and work continuously to completion. Upon completion follow up in writing, what action was taken.

c. Routine: all other work to be initiated within ten (10) days and work continuously to completion.
