About this file
This is an Exterior Pressure Vessel Inspection Report for the LOSC ISI Program, completed on 1/24/2024 by Inspector Brett Strickland (Level II) for client Call Henry Inc. The report documents inspection findings for vessel PV-230, a horizontal pressure vessel manufactured by Taylor Forge with serial number 750400-1V8, rated for 6,000 PSI maximum allowable working pressure and located at the High Pressure Gas Storage Area (HPGSA) on SLC-4 at Space X facility. The vessel stores gaseous nitrogen at temperatures ranging from -25 to 140°F.
The inspection identified multiple deficiencies requiring attention. Significant code violations include: dissimilar metals on bolt/flange assemblies (galvanized steel washers in contact with stainless steel studs and nuts), violating AFSPCMAN 91-710 11.3.1.5.6; relief valves missing system reference designators, violating AFSPCMAN 91-710 LOSC Tailored 11.2.1.7.6.7; and a missing U-bolt on the pipe support. Additional findings include moderate corrosion on the isolation valve, a 7/8-inch offset and 1.25-1.5 inch gap in concrete foundations, replacement gauges missing "No Calibration Required" stickers, relief valves outside the one-year calibration period, and vegetation growth around foundation gaps. Most vessel components including heads, shell, welds, and attachments (except bolts/flanges rated as rejectable) received acceptable or acceptable-with-reservation ratings. The report recommends corrective action for corrosion, gauge registration, vegetation removal, and continued monitoring of the foundation offset.

Additional Notes/Recommendations
The vessel isolation valve has areas of moderate corrosion. This item should be addressed before the corrosion impacts the function of the valve. The gauges on the panel were replaces and no longer have "No Calibration Required" (NCR) stickers on them. As these gauges are identical and serving an identical function as the previous gauges that had NCR this item has been listed as "Other". It is still recommended that the site owner register these new gauges as NCR and affix the NCR stickers to them. There was a fair amount of vegetation growing around and in the gaps of the vessel foundations. It is recommended that this be removed and controled as plant life/growth can cause damage to foundations.
Other
There was a slight offset of 7/8" between the concrete foundations and a gap of 1.25"-1.5". It is recommended that this continue to be monitored. There was not notable change in the offset severity from the previous years inspection. The relief valves were missing system reference designators. This is a violation of AFSPCMAN 91-710 LOSC Tailored
11.2.1.7.6.7. There were dissimilar metals on the bolt\flange assemblies that were not mutually isolated from each other.
It appeared that galvanized steel washers were in contact with stainless steel studs and nuts. This is a violation of
AFSPCMAN 91-710 11.3.1.5.6. The pipe support was missing the U-bolt that supports it. The relief valves are outside the traditional one year calibration period.
