Construction Services Task Order

Statement of Work

To

RENOVATE

RESTROOMS

AT

MAINTENANCE HANGER

BLDG. 711

At

DOVER AFB, DELAWARE

For Project:

FJXT111186

1.0 SCOPE

The Contractor shall design and construct renovations to Restroom Areas of Maintenance Hanger Bldg. 711, at Dover Air Force Base, Delaware. The work shall consist of, but not limited to, providing all labor, materials, tools, expertise, testing, surveying, equipment, sampling, warranties, and transportation necessary to design and construct the identified renovations to Maintenance Hanger Bldg. 711. All necessary design and renovation work required to modify existing facility systems affected by this renovation shall be included in this scope of work.

The facility is an approx. 100,000 SF aircraft maintenance hangar which will be occupied 24 hours a day during both the design and construction of this project. The specific area to be renovated within this facility is only the Restrooms, Locker Room, Break Room, & two associated Corridors of the building. The majority of this project will involve finish upgrades to these areas while reusing (where possible) existing restroom fixtures, wall coverings, ceiling, etc. All design and construction activities shall be performed in a manner that minimizes impact to the facility users and equipment down time.

2.0 APPLICABLE DOCUMENTS

The Contractor shall identify and comply with all applicable Federal, State, and local statutes; DoD/Air Force instructions, manuals, handbooks, regulations, guidance, and policy letters including but not limited to;

Occupational Safety and Health Administration (OSHA); International Building Code (IBC); Unified Facility Criteria (UFC); National Fire Protection Association (NFPA); National Electrical Code (NEC) and including all changes and amendments in effect on the date of issuance of this TO. It is the Contractor’s responsibility to identify and comply with all applicable requirements. In addition, the Contractor shall refer to the latest version of The USAF Project Manager’s Guide to Design and Construction.

3.0 GOVERNMENT-FURNISHED INFORMATION, EQUIPMENT, MATERIAL, AND PROPERTY

(GFI, GFE, GFM, GFP)

The Government will provide information in the form of available plans, reports, maps, and drawings as necessary to facilitate accomplishment of the work required in the contract Statement of Work at the pre-construction conference.

The Government will furnish blueprints of available as-built drawings that are pertinent to the facility involved.

However, these drawings are not warranted to show conditions as they actually exist. The designer shall verify (by field investigation) information shown on the as-built drawings (which is relevant to the project) for accuracy to ensure a quality-designed project.

The following is a list of Government supplied documents which should be used for project execution:

4.0 MANAGEMENT, PLANNING, AND REPORTING REQUIREMENTS

The Contractor shall implement a full range of construction and engineering activities as specified in each TO and in accordance with all applicable compliance documents. The Contractor shall supply all labor, equipment, and materials necessary to accomplish the work assigned unless otherwise specified. The Contractor shall perform management and planning functions, including performance measurement and funds status reporting, through the course of the effort.

4.1 Work Breakdown Structure (WBS)

The Contractor shall prepare and submit, for approval, by the Contracting Officer (CO), a WBS consistent with the most recent Construction Specifications Institute (CSI) format for construction activities. The WBS shall be used to report the cost and schedule status for this project. All tasks required under this type of TO shall be included in the WBS.

4.2 Schedule and Planning Requirements

The Contractor shall provide schedules for tracking work progress as specified in this TO.

Major features of work on this project shall be completed in accordance with the following schedule.

Final Design Completion: 90 days from NTP

Major Construction/Renovation: 90 days from Design Completion

A final detailed project schedule shall be submitted for approval by the contractor at the start of the project. Upon government approval the contractor may be allowed to overlap design and construction efforts. This must be determined and approved during the design phase of the project.

4.2.1 Project Planning Chart (PPC)/Construction Planning Chart (CPC).

The Contractor shall prepare and submit a PPC/CPC for approval. The PPC/CPC shall detail the project schedule and status through the use of Gantt charts, which shall depict percent complete for each task. The project schedule shall be reported using the approved WBS.

4.3 Cost and Status Reporting Requirements for Firm Fixed Price TO

Cost reports are not required under fixed-price TOs. The Contractor shall prepare and submit a CPSMR. The CPSMR shall be used to review and evaluate the overall progress of the project, along with any existing or potential problem areas. The report shall be prepared in a Contracting Officer’s Representative (COR) approved format. The CPSMR shall include a summary of the events that occurred during the reporting period, discussion of performance, identification of problems, proposed solutions, corrective actions taken, and outstanding issues. The CPSMR shall also include project schedule status. Project schedules, status reports, and invoices are required monthly.

4.4 Meeting and Conference Requirements

The Contractor attend a site visit and/or attend a pre-proposal site visit and conference, preconstruction conference, and regular progress meetings necessary to support construction activities. The Contractor also shall attend and support meetings and/or teleconferences to discuss technical or regulatory issues and project progress and status.

The Contractor shall prepare, and submit for review, presentation materials for meetings and an agenda. The

Contractor shall prepare minutes for these meetings attended, including teleconferences, and distribute minutes to attendees within 3 working days.

4.5 Contractor Documentation

The Contractor shall create and maintain a Master Document List (MDL) and/or Submittal Register, i.e. AF Form 66, that includes all documents, whether the documents are a deliverable or not, which are prepared during the course of the TO. The MDL and its documents shall be maintained in libraries readily available for submittal to the Government. All Material Submittals shall be accomplished in accordance with the instructions pertaining to AF Form 3000, Material Approval Submittal.

4.6 Notification Requirements

The Contractor is required to notify the CO of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to, health risks, spills, changes in critical personnel, and finding unexploded ordnance (UXO). As an example, if UXO were discovered during field activities, the Contractor would be required to immediately stop work, report the discovery to the facility Point of Contact (POC), CO, and implement the appropriate safety precautions. Field activities could not continue until clearance was received from the CO. On critical issues, verbal notification should be made immediately, followed by written notification as soon as practical.

4.7 Permits

The Contractor shall develop, coordinate, and assist the installation in applying for and obtaining all Federal, State and local, and other applicable permits, licenses, and certificates required to perform and execute this TO. The Contractor shall maintain a library of these documents at the Contractor’s site office as well as the corporate facility. The Contractor shall comply with all applicable permit conditions.

4.8 Photo Documentation

The Contractor shall prepare digital photo documentation, including site(s) and building(s) under construction, field activities, and sample locations if applicable. Digital photos will be taken and submitted using a minimum 5 mega pixel camera in JPEG format. Preconstruction photos must be taken of all impacted areas and shall be submitted to the Contracting Officer prior to the start of work (permission must be given by DAFB prior to taking all photos). The Contractor shall provide an index for each set of photographs submitted, identifying the base or facility, project number, contractor, and a brief description. Photography of any kind must be coordinated through the installation, customer, or facility POC. Photo documentation shall be loaded to a Compact Disk and forwarded to the Project Manager.

4.9 Site Access Badges

The Contractor shall obtain and monitor assigned security badges (used by both the Contractor and subcontractor staff) used during the duration of this contract. All security badges or passes shall be returned to the base or facility POC upon expiration of the badge, upon completion of the project, or when possession of the badge is no longer necessary (e.g., upon removal of contracted personnel from specific projects).

4.10 Worksite Activities and Coordination

4.10.1 Coordination of Activities/Safety Requirements

The Contractor shall coordinate worksite activities with all applicable personnel to ensure the protection of human health and the environment; the prevention of damage to property, utilities, materials, supplies, and equipment; and the avoidance of work interruptions. The Contractor shall provide physical security to work areas with security equipment and personnel.

The Contractor shall be responsible for protecting the lives and health of employees and other persons on the work site; preventing damage to property, materials, supplies, and equipment, avoiding work interruptions and complying with Occupational Safety and Health Administration (OSHA) safety and local safety office requirements. The Contractor shall perform all operations in a prudent, conscientious, safe and professional manner and conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work. The Contractor shall maintain documentation supporting training records on site and have written Health and Safety Plans on site and available for workers and/or regulatory review. The Contractor shall provide the CO copies of any OSHA report(s), regarding a project site, submitted during the duration of the TO. For areas not covered by OSHA, the Contractor shall comply with regulations regarding safety and health and the current edition of the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1.

The Contractor shall record and report promptly (within 1 hour) by telephone, facsimile or other direct means to the CO, COR, the Base POC, and to the military installation’s Safety Office, all available facts relating to each instance of damage to Government property or injury to either contractor or Government personnel. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, preserve evidence until released by the accident/mishap investigative authority through the CO, and immediately report the incident to the Fire Department at 911. If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in conducting of the investigation until it is completed.

The Contractor agrees that his personnel and equipment are subject to safety inspections by Government personnel while on federal property.

4.10.2 Hazardous Material and Hazardous Waste Activities Local Requirements.

The Contractor shall sample and handle the abatement, removal, and disposal of all hazardous materials and waste in accordance with applicable Federal, State, local and TO requirements. The Contractor shall provide all hazardous materials use and hazardous waste disposal documentation to the installation or facility POC, and shall register with the Hazardous Materials Pharmacy program (if available) at the installation to ensure appropriate and efficient tracking of the Contractor’s hazardous material purchases, inventories, use, and releases such as required by the Emergency Planning and Community Right-to-Know Act (EPCRA), EOs, or any installation reporting requirements. The Contractor shall also comply with Federal, State, and local requirements for any task involving the transportation of hazardous wastes and/or contaminated materials to off-site treatment, storage, and/or disposal facilities. This includes 40 CFR 260, 49 CFR 172, 173, 178, and 179, and all other applicable Federal, State, and local transportation regulations.

5.0 PLANS, REPORTS, AND DELIVERABLES

The Contractor shall prepare and submit a Work Plan describing the Contractors plan to implement the work required in this task order. The Contractor shall prepare and submit to the CO for approval, any health & safety, sampling and analysis, waste management plan, construction plan, quality control plan, or other required plans as required prior to receiving a notice to proceed and the commencement of work.

5.1 Health and Safety Plan (HSP)

The Contractor shall provide a Health and Safety Plan (HSP) as specified in this TO.

5.2 Quality System Plan (QSP)

The Contractor shall prepare, for review and approval, a site-specific QSP. The Contractor must implement, maintain, and comply with the approved site-specific QSP. The Contractor shall organize, document, prepare procedures for, and implement a management-driven system to provide maximum assurance that products and services conform to specified requirements. Products and services submitted in the performance of this contract shall be according to the American National Standard Institute (ANSI)/American Society for Quality Control (ASQC) standards if applicable, or Best Commercial Practice, and contract requirements. All work accomplished by the Contractor shall be of sufficient quality and level of acceptance by Federal, State, and local regulators or code inspectors. The Contractor shall submit a Quality Control Program Plan detailing the execution of this program to the CO for approval which is required prior to receiving a notice to proceed.

5.3 Technical Plans and Reports

The Contractor shall provide technical plans, specifications, and reports as defined in this TO. Some of these may include, but are not limited to, the following:

5.3.1 Construction Work Plan

The Contractor shall prepare a Construction Work Plan that may include, but is not limited to, any or all of the following subsections as identified in this TO.

5.3.2 Technical Reports

The Contractor shall prepare and submit the following technical reports as identified in this TO.

a. Daily Field Reports (Status Reports)

b. Hazardous Materials Survey Report

c. Hazardous Material and/or Hazardous Waste Disposal Report

d. Design Drawings

e. Shop Drawings and/or As-built Drawings

f. Design Specifications

g. Computer Aided Design Drawings

6.0 SITE WORK

6.1 Conservation

Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, monitoring and other types of wells, and the general site environment. This includes the protection of trees, shrubs, and other vegetation not in the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. If appropriate, the Contractor shall conserve uncontaminated topsoil by removal, storage, or redistribution. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. All fill materials shall be uncontaminated. The Contractor shall conduct all operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone-depleting chemicals, generation of hazardous waste, and use of energy and water. The Contractor shall plan, construct, operate, maintain, optimize, and decommission systems necessary to control storm water run-on and runoff; and transport surface water drainage to a treatment plant, discharge location, or any other destination.

6.2 Site Preparation

The Contractor shall perform site work as necessary to prepare sites for construction activities. Security and access controls shall be implemented to prevent unauthorized entry to sites and to protect wildlife from site exposure. The Contractor shall survey existing utilities to determine adequacy and need for modifications to support site activities. The Contractor shall obtain appropriate approvals and shall construct connections or new systems for electrical power, water, sewer, gas distribution, telephone, and other utilities, as required, to accomplish the activities.

6.3 Demobilization

The Contractor shall demobilize facilities as necessary, and restore the site. The Contractor shall remove any temporary facilities and implement erosion control measures such as seeding, mulching, sodding, and erosion control fabrics; restore roads, structures, and utilities; and plant trees, shrubbery, grasses, and other vegetation. The Contractor shall document and report on activities demobilization activities.

7.0 FACILITY CONSTRUCTION REQUIREMENTS

The Contractor shall perform a full range of activities to meet all customers’ construction requirements as designed and specified. Requirements include design, construction, demolition, repair, and emergency response tasks.

7.1 Turnkey Not applicable to this TO

7.2 Design-Build (D-B)

The Contractor shall manage and perform D-B construction work as defined in this TO. The following are typical D-B construction TO requirements: completion of working drawings, permitting, construction, construction management, scheduling, inspection and testing, maintaining construction facilities and temporary controls, environmental protection, quality control, construction quality control, and project closeout. Specific requirements will be provided in this TO.

The Contractor will conduct a Design Charrette, a 50% design review submittal, 100% design submittal, and Final design submittal. The Contractor will be required to take meeting minutes and publicize the minutes in accordance with this Task Order.

Base Bid Scope Items:

The overall purpose of this project is to complete a finishes upgrade to the Bldg. 711 Rest., Locker, & Break Rooms. This section of the facility was last renovated in 2005 therefore it is anticipated that many of the fixtures and finishes can be reused. The limits of construction for this project shall include the Men’s Restroom, Women’s Restrooms, Locker Room, Break Room and the Access Corridors. These limits of construction shall not apply to utilities (e.g. electric, HVAC, plumbing, etc.) that must be revised to accommodate the renovation work. All work and products used in the renovation must be commercial grade with durability being a top priority. This work shall include (but not be limited to) the following:

(Doors & Wall Coverings)

The contractor shall clean the existing wall tile and repaint any areas currently covered by paint within the limits of construction. This painting shall include the repainting of doors within these limits. Any damaged wall tiles or wall section shall be repaired prior to being finished (new wall tiles shall match existing). Any damage door hardware shall be replaced in kind or repaired to proper working order.

(Restroom Fixtures)

The existing restroom showers, lavatories, faucets, and counters shall be replaced with new water efficient fixtures conforming current air force and Dover AFB requirements. Showers shall be replaced with larger shower stalls (48” wide minimum) with one in each restroom designed meet ABA requirements. All shower drains shall be provided with integral trap primers. The existing toilets, urinals, flush valves, and toilet partitions shall remain in place and will not to be removed as part of this renovation however all shall be fully cleaned and repaired to full working order. All restroom accessories (paper towel dispensers, soap dispensers, shelves, mirrors, etc.) shall be replaced as part of this project.

(Floor Tile)

All existing floor tiles shall be stripped and refinished (e.g. saniglaze). Any damaged tiles shall be replaced prior to being refinished.

(Ceiling)

The existing ceiling shall be removed and replaced with a new drop ceiling.

(HVAC)

There is currently a continuous odor in the Men’s Locker Room and Restroom. The contractor shall evaluate the existing HVAC system to determine if it meets current ventilation requirements. This evaluation shall include but not be limited to system air flow measurements performed by a certified TABB agent. The contractor shall also replace the existing exhaust fans that serve the Men’s/Women’s Locker Room and Restroom areas. These exhaust fans and accessories shall be replaced with new fans capable of exhausting the current code required flow rates while ensuring that an excessive negative pressure is not created. The contractor shall evaluated and ensure that there are no closed dampers preventing supply air from entering the renovation areas. The replacement of the existing Air Handling Unit is shall not be included in this project.

(Electrical)

All lighting shall be replaced with new lights that meet current Dover AFB design standards. Providing power to the new lighting fixtures from the existing branch circuits is acceptable is the existing branch circuits are sufficient.

It is expected that all existing fire alarm and mass notification devices will remain.

(Furniture)

All lockers, benches, seats, etc. shall remain as part of this project. The contractor is responsible for protecting and/or storing these fixtures during construction. Existing fixed benches shall be re-secured to the floor to eliminate and movement. All Toilet partitions shall be cleaned and remain as part of this project.

(Temporary Restrooms)

The contractor shall provide a minimum of five (5) temporary restroom facilities throughout the duration of construction. These facilities shall include toilets and sinks with running water. Two (2) of the five (5) temporary restrooms shall be ADA compliant.

(Break Room)

The contractor shall work with the user to develop the optimal layout for the break room. This shall be done using the existing break room space, furniture, and equipment (or furniture/equipment to be purchase by the government). The existing break room cabinets, counters, and island shall be replaced with new durable fixtures.

The layout of new cabinets and counters shall be developed with the user based on their operational requirements.

The Contractor shall function as an integral team member on this Design Build project in support of the mission, to include the sharing of information as required with other contractors working on DAFB and cooperation with communities, and other government entities.

Requirements include efficient management of TOs including accurate, on-time submittals of contract deliverables and timely identification and solution of impediments to successful project execution. Technical requirements include early involvement in the process to allow for the development of the most cost-effective and technically sound solution. The government will rely on the Contractor’s expertise in recognizing and addressing problematic issues and successful execution of this TO. The Contractor shall perform all work in accordance with Federal, State, and local statutes and regulations.

7.3 Design-Build Plus (D-B +) Not applicable to this TO

7.4 Design-Bid-Build (D-B-B) Not applicable to this TO

7.5 Other Construction Services

Upon approval by the CO, the Contractor may commence on-site construction activities. The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to review, plan, develop and implement quality control and oversight services during the construction phase of this TO. Details regarding construction management requirements, inspection and testing, construction facilities and temporary controls, environmental protection, quality control system, construction quality control, and project closeout are provided in this TO.

7.5.1 Inspections

7.5.1.1 Pre-Final Inspection

The Contractor shall conduct a pre-final walk through inspection and publish the pre-final inspection findings in a pre-final inspection report. Closeout starts at 95% project completion. Commissioning needs to be completed prior to the pre-final inspection taking place.

7.5.1.2. Final Inspection

The Contractor shall conduct a final inspection and publish the findings in a final inspection report in accordance with the format specified at the pre-construction conference. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection report. The final inspection report shall: (1) certify that all items of the design have been implemented and that the construction is complete, and (2) include a record of “signed and sealed” as-built drawings and specifications verifying that all development standards have been met. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the Base Civil Engineer (BCE) or other appropriate organization for signature and acceptance.

7.5.2 Delivery and Warranty

The Contractor shall complete all inspection and commissioning requirements prior to pre-final inspection. The warranty shall be issued in accordance with FAR 52.246.21 (Warrantee of Construction). Extended warranties offered by the Contractor and its subcontractors or suppliers may be accepted at the Government’s discretion.

7.6 Demolition

The Contractor shall demolish facilities, systems, and other improvements as required and specified by this TO.

The Contractor shall ensure all storm water catch basins and attached lines within and adjacent to the project areas are cleaned out and/or free of any debris at the end of the project.

7.7 Maintenance and Repair

The Contractor shall perform all normal maintenance and repair of facilities, systems, and utilities that are under his control during construction. Maintenance and repair can be performed incidental to other requirements or as a stand-alone project.

8.0 ENVIRONMENTAL

The Contractor shall consult on environmental engineering problems incidental to construction efforts. The Contractor must be able to recognize when specialized environmental expertise is necessary and have the capability of providing that expertise.

9.0 POINTS OF CONTACT (POCs)

Points of contact will be provided at the preconstruction meeting.

Appendix 2 Delivery and Performance Information

Pertaining to the Seed Project

52.211-10 COMMENCEMENT, PROSECUTION, AND COMPLETION OF WORK (APR 1984)

The Contractor shall be required to (a) commence work under this contract within 10 calendar days after the date the Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than 180 calendar days. The time stated for completion shall include final cleanup of the premises.

52.211-12 LIQUIDATED DAMAGES--CONSTRUCTION (SEP 2000)

(a) If the Contractor fails to complete the work within the time specified in the contract, the Contractor shall pay liquidated damages to the Government in the amount of $385.36 for each calendar day of delay until the work is completed or accepted.

(b) If the Government terminates the Contractor's right to proceed, liquidated damages will continue to accrue until the work is completed. These liquidated damages are in addition to excess costs of repurchase under the Termination clause.
