About this file

This is a Work Order Request and Response Form Template document for the Texas Health and Human Services Commission (HHS) under RFQ No. HHS0016852, designed for state and local contract opportunities. The template establishes a structured three-section process for work order solicitations: Section I contains work order request requirements completed by the System Agency Contract Manager, Section II comprises the contractor's response, and Section III documents any negotiated modifications. The form requires contractors to respond to specific work order solicitations with detailed information regarding their capability to deliver identified services and deliverables. The template includes placeholder fields for key project parameters including work order identification numbers, project titles, estimated start and end dates, contractor questions deadlines, response submission deadlines, and estimated award dates. All communications between the System Agency and contractors must be conducted through designated email points of contact, with failure to comply potentially resulting in contractor disqualification.

The compensation structure for work orders may utilize either lump sum/fixed fee arrangements or time and materials compensation with not-to-exceed amounts, with all proposed pricing tied to specific deliverables and subject to rates established in the underlying Master Contract. Contractors must provide detailed projected expenditures by deliverable, including hourly rates, hours of labor, and total costs for each assigned staff member, along with itemized travel expenses if permitted under the Master Contract. The form requires contractors to submit updated Historically Underutilized Business (HUB) Subcontracting Plans upon execution of any resulting work order agreement and obtain approval from the HHS HUB Program Office before commencing work. Contractor responses are limited to a specified number of pages and must adhere strictly to formatting requirements without modifications; responses may be submitted via email, USB drive, or paper with USB drive as specified by the System Agency.
