About this file

This is Exhibit K, Goal and Performance Measures, for RFP No. HHS0016376 issued by the Texas Department of Family and Protective Services through the Health and Human Services Commission for Child Protective Post Adoption Services. The exhibit establishes measurable performance standards for contractors providing post-adoption services across eleven DFPS administrative regions, including case management, counseling, mental health services, parenting intervention, support groups, and residential treatment. The contract term runs from September 2026 through August 2030 with potential one-year extensions. Performance evaluation occurs during two performance periods annually: September 1 through February 28/29 (PP1) and March 1 through August 31 (PP2), with quarterly data reporting and annual determination of results.

The performance measures establish four key outcomes and outputs tracked through the IMPACT data system. Outcome #1 measures the percentage of adopted children returned to DFPS care within one year of receiving services, with a target set at the FY24 statewide baseline. Outcome #2 targets 95% of referred clients receiving services during the performance period. Output #3 establishes a target for the average monthly number of clients receiving paid services at 95%, and Output #4 targets the average monthly number of families receiving paid services at 95%. All metrics are calculated using numerators and denominators derived from IMPACT system data, with removal reasons specified as Refusal to Accept Parental Responsibility, Abandonment, or Solely to Obtain Mental Health Services. The performance period for annual evaluation runs September 1 through August 31, divided into four quarters for reporting purposes.
