About this file
The document is a Past and Present Performance Questionnaire for a Department of Veterans Affairs (VA) contract opportunity. The form is designed to collect detailed performance evaluations from previous project references, with ratings on 22 specific performance criteria ranging from project management and quality control to schedule adherence and problem-solving capabilities. References are asked to rate the contractor's performance on a scale of 1-6, with the option to provide narrative explanations for low ratings.
The related federal contract opportunity is a resolicitation (Solicitation Number 36C24825R0127) for correcting patient privacy deficiencies at the 2N/2S Lake City, FL VA facility, previously solicited as 36C24825R0067. This new solicitation is specifically opened to small businesses, replacing the prior Service-Disabled Veteran-Owned Small Business (SDVOSB) solicitation. The questionnaire is part of the VA Network Contracting Office 8's procurement process, with the contract specialist Amber Milton overseeing the solicitation and requesting references to complete and submit the performance evaluation form directly to her via email.
