About this file

This Quality Assurance Plan (QAP) is developed by the County of Volusia for the MIL-VOL-002 / B-19 Canal Site project in Volusia, Florida. The project involves pillow form removal, repairing eroded areas, installing rock riprap, and implementing vegetative measures. The inspection will be conducted by a designated inspector who will ensure continuous and periodic monitoring of various construction activities, including geotextile installation, rock riprap placement, and vegetation establishment. The inspection will focus on verifying compliance with project plans and specifications, with the intensity of oversight varying based on the complexity and potential impact of each work item.

The document outlines specific inspection requirements, including checking material quality, proper installation techniques, and measuring quantities of materials used. Louis Trivett is designated as the primary inspector, with Jason Goodnoe serving as a backup. The project will require maintaining daily job diaries, taking digital photos, recording quantity computations, and preparing comprehensive documentation. At project completion, the engineer must provide NRCS with a certification of specification compliance, sealed as-built drawings, and a list of installed items with their quantities. The documentation must be submitted within three working days of project completion, with all computational records to be checked and initialed by a second person to ensure accuracy.

The Quality Assurance Plan (QAP) outlines the responsibilities of the County of Volusia to ensure that the works of improvement are installed in accordance with the plans and specifications. The County of Volusia will designate a person to fullfill the inspection responsibilities.

I. ITEMS OF WORK TO BE INSPECTED

The work to be accomplished under this contract consists of pillow form removal, repair of eroded areas and the installation of rock riprap, and vegetative measures.

The intensity of the inspection of these items will vary from periodic to continuous. The intensity of the inspection will depend on the complexity of the work item and what the damaging results would be should that portion of the installation fail.

The timing and intensity of the required inspection is shown in Table 1. Support personnel should be provided as needed to assist the assigned inspector.

Other type work items may be encountered during construction. The intensity of the inspection will be determined by the Inspector or Engineer.

A. Pillow Form Removal

Inspection effort required will be to verify the concrete pillow forms are removed from the designated areas. The inspector will check the site to verify that the item was completed in accordance with the plan and specifications.

B. Repairing Eroded Areas

Periodic inspection will be required on all eroded areas to be paired for armoring.

The inspector will need to verify that all eroded areas are prepared in accordance with plans and specifications to accept rock riprap armoring. The inspector shall be on-site to ensure earthfill used to repair eroded areas is of suitable quality and has sufficient moisture content for compaction. Inspector shall check compaction to ensure it is in accordance with the plans and specifications.

Materials used to repair eroded areas will not be measured and is incidental to the installation of rock riprap.

C. Geotextile

The inspection effort for this item will be to ensure proper material and installation of the geotextile. The inspector will need to check the geotextile to ensure that the specified type is installed. The inspector will record the type, weight, and manufacturer of the geotextile.

The inspector will determine and stake the area to be covered by the geotextile.

The inspector shall check that the material is properly imbedded at the edges and proper overlap is provided during layout.

The quantity of geotextile will be measured.

D. Rock Riprap and Bedding Stone Installation

The inspection effort for this item will be to ensure proper material and installation of the rock riprap and bedding stone. The rock riprap and bedding stone must be inspected to ensure that the gradation is the size specified in the plans and specifications.

The quantity of rock riprap and bedding stone will be measured

E. Vegetative Measures

Periodic inspection will be required on all areas to be vegetated. The inspector will need to stake all areas to be vegetated and the type of vegetative requirement at each area. Performance sod will be installed at areas called out by the plans Inspection will require that the sod be checked as to the quality and specified type. All other disturbed areas shall be vegetated in accordance with the plans and specifications. The quantity and type of seed, fertilizer and lime will be verified by the inspector. Vegetated areas will be measured and the inspector will need to compute quantities.
