About this file

This document is a set of General Notes and Special Provisions for an Open Channel Debris and Sediment Removal and Bank Stabilization project by Volusia County's Public Works Department, Road & Bridge Division, funded through a cooperative agreement with the U.S. Department of Agriculture Natural Resources Conservation Services (NRCS). The project involves implementing recovery measures for hurricane damage in specific open channels and a box culvert bridge in Ormond Beach, Port Orange, and New Smyrna Beach, Florida. The work includes waterway removal of fallen trees and debris blockages, and bank stabilization to prevent flooding.

The project will be funded through Emergency Watershed Protection Project #NR254209XXXXC008, with the contractor responsible for providing all labor, materials, equipment, and services necessary to complete the work. The specifications are based on the Florida Department of Transportation (FDOT) Standard Specifications for Road and Bridge Construction, with specific modifications outlined in the document. The contractor will be required to follow extensive environmental, safety, and technical provisions, including developing a Stormwater Pollution Prevention Plan, maintaining traffic flow, implementing erosion control measures, and providing detailed record drawings and post-construction documentation.

6. Storm inlets, catch basins, swales, and storm drain pipes shall be protected from sedimentation resulting from surface runoff until completion of all construction operation that may cause sediment runoff. Filter fabric shall be placed and maintained under the grate and filter socks placed in front of the throat of curb inlets, during construction.

7. Sediment (turbidity) barriers must be installed at all locations where the possibility of transferring sediments and suspended solids into the receiving water body exists due to construction. All proposed paving surfaces in intersections and adjacent existing sections shall be graded to drain positively and to provide a smoothly transitioned driving surface for vehicles with no sharp breaks in grade, and no unusually steep or reverse cross slopes. Approaches to intersections, entrance and exit grades to intersections may require minor local field adjustments should actual conditions vary from the surveyed information the design was based upon in order to accomplish the intent of the plans. It is the Contractor's responsibility to accomplish the above and consult the County Representative as needed to make any and all required interpretations of the plans or give supplementary instructions should the intent of the plans be unclear.

3. The Contractor is responsible for removing any excess cuts or supplying additional fill as necessary to grade the site to the proposed elevations as designed within the construction documents at no additional cost to the County.

3.4.1. Section and ¼ Section Corners: All Section Corners and ¼ Section Corners disturbed shall be reset with monuments and monument boxes. Monument boxes of a type approved by the Road & Bridge Director will be supplied by the Contractor for installation by the Surveyor employed by the Contractor. The Surveyor employed by the Contractor shall re-reference Section Corners and ¼ Corners within the project limits, preferably with “X” cuts in curbs, sidewalks or traffic signal foundations, and submit a Florida Department of Natural Resources (DNR) Certified Corner Record Sheet in accordance with Chapter 177, Part II, Florida Statutes, to DNR via the County Surveyor. In the case where the corner marker was only paved over and is still in place, an approved monument box shall be installed to provide access to the corner.

Right-of-Way Monumentation: All right-of-way owned by the County, including right-of-way and easements for lateral ditches, environmental impact mitigation sites, storm water management pond sites and other drainage facilities shall be monumented in accordance with the information shown on the right-of-way maps. The rights-of-way and easements will be monumented using markers as approved by the Road & Bridge Director.

Those markers shall be set on the right-of-way at each deflection and right and left opposite each PC, each PCC, and each PT. The maximum distance between the right-of-way markers shall be 1,320'. These right-of-way markers will be placed on all projects. The final field location of the right-of-way markers installed will be shown on a “Specific Purpose Survey” map indicating that the corners were set in accordance with the Volusia County Right of Way map prepared for the project as applicable and shall be certified to Volusia County by the Contractor’s surveyor. The “Specific Purpose Survey” map shall be submitted in AutoCAD and Adobe .PDF format, together with a hardcopy, plotted on ledger size paper, unless otherwise specified.

3.4.4. P.I. Monumentation: The center lines of the right-of-way of all intersecting roads, canals and railroads, unless a Section or ¼ Section Corner, shall be monumented with a nail and disc with four straddlers. The location of these PIs shall be established by using the off project references previously established in the field by the design consultant or the County Surveyor.

3.5.4. As-Built Wetland Mitigation Area Grading Plans: As-Built Mitigation area grading plans include a certified as-built survey of all wetland mitigation areas. A separate survey for each mitigation area shall be made. Each survey shall include, as a minimum, the surface area (total square footage or acreage), the location and elevation of a permanent bench mark set, and the required number of cross-sections needed to verify that grading of the area conforms to the approved plans. The as-built mitigation grading plan must be submitted to permitting agencies for review.

3.5.5. During Construction: In addition to Record Drawings as specified above, during construction, the Contractor shall maintain records of all plans and specification deviations and prepare “red-lined” record drawings showing correctly and accurately all changes and deviations from the work made during construction reflecting the work as it was actually constructed. These drawings shall conform to recognized standards of drafting, shall be neat and legible, shall be labeled “As-Built” and shall be provided to the Road & Bridge Director at times as may be required elsewhere in these Contract Documents.
