About this file
This document is an addendum to environmental, health, and safety requirements for Fermi Forward Discovery Group (FFDG) construction subcontracts. The addendum mandates compliance with 10 CFR 851 and the Fermilab Environmental, Health & Safety Manual (FESHM), Chapter 7010, and establishes immediate updates to these requirements for all subcontractors. Key requirements include task-specific Job Hazard Analyses (HAs) that must detail scope, steps, hazards, and mitigations, with HAs for work at heights requiring submission to the Construction Coordinator at least three business days prior to work commencement and acceptance before work begins. All personnel must maintain current OSHA-required specialized training records (crane operation, powered industrial trucks, fall protection, silica exposure, LOTO, power tools, and other permit-required work) available onsite, with subcontractors responsible for obtaining and maintaining records for all sub-tier subcontractors. Personal protective equipment and equipment inspections must be completed by a Competent Person prior to use, and daily toolbox meetings are required to discuss work plans and high-hazard tasks with review of applicable procedures, PPE, and critical mitigations.
Additional requirements include briefing new site or task personnel on HAs through written copy and verbal presentation, discussing worker ability to pause work and ask questions, obtaining DOE and FFDG approvals for high-hazard activities outside normal work hours (Monday-Friday, 6 a.m. to 7 p.m.), and obtaining site access approval with valid REAL ID-compliant government identification and Fermilab badge worn 100 percent of the time on-site. Sub-tier subcontractors must submit safety training records, complete the Fermilab Subcontractor Safety Information Questionnaire, provide Experience Modification Rating (EMR) and OSHA 300 logs for the past three years with expectation of EMR below 1.0, and complete Construction ES&H Certification. FFDG reserves the right to reject sub-tier subcontractors with EMR of 1.0 or above or incomplete safety records, requiring replacement at no additional cost. Emergency procedures must be well-developed with identified local care centers and clear communication procedures, including preservation of incident scenes as required.

ADDENDUM TO ENVIRONMENTAL, HEALTH & SAFETY REQUIREMENTS
Fermi Froward Discovery Group’s (FFDG) construction Subcontracts require all Subcontractors to comply with 10 CFR 851 and incorporate the Fermilab Environmental, Health & Safety Manual (FESHM), Chapter 7010. FFDG is in the process of updating FESHM, Chapter 7010. For purposes of this Subcontract, the following updates are effective immediately and applicable to this Subcontract:
• A Job Hazard Analysis (HA) is required for each task. The HA must be task- specific and include the scope, steps of the task, hazards associated with each step, and the mitigations that will be implemented to address the hazards.
• HAs for work at heights must be submitted to the Construction Coordinator who will transmit to the DOE Fermi Site Office Team for review.
o Documents shall be submitted no less than three business days prior to the scheduled work.
o A HA must be accepted by the FFDG Construction Coordinator or Task Manager (FFDG CC/TM) prior to commencing work on the task.
o Acceptance will be communicated by a stamp on the HA submittal indicating acceptance. No work may be performed unless FFDG indicates an unqualified acceptance of the HA.
• OSHA required specialized training records (crane operator, powered industrial trucks, mobile elevated working platforms, fall protection, silica exposure, work that requires a permit, LOTO, power tools, etc.) records shall be available and onsite for all personnel performing the work.
o Subcontractor is responsible for obtaining and maintaining training records for all its sub-tier subcontractors.
• Inspections of all personal protective equipment (PPE) and equipment required per FESHM, must be completed by a Competent Person prior to use. All documented inspections shall be available for FFDG verification.
• Subcontractors are required to conduct daily “Toolbox” meetings to discuss the work plans of the day/week and highlight any high hazard tasks. As part of these meetings, a review shall be provided for applicable procedures, necessary PPE, and any critical mitigations necessary for safe completion of the task.
• Personnel new to the site or task shall be briefed on the HA by receiving a written copy of the HA and verbal presentation of the mitigation measures (such as emergency procedures, PPE locations, PPE inspection expectations, and critical measures like approved tie-off points for fall protection).
• Subcontractors shall discuss with all employees and sub-tier subcontractors the ability of each worker to pause work and ask questions about the procedure or any mitigating measures. At a minimum, this discussion must occur during site orientation.
• All high-hazard and high-risk activities conducted before or after normal work hours (Monday through Friday, 6 a.m. to 7 p.m., on weekends, or on holidays) require additional reviews and approvals from FFDG and DOE.
• Access approval is required prior to accessing the site. Subcontractors are required to follow the appropriate processes to obtain site access. Subcontractors must provide a valid REAL ID compliant form of government-issued identification and complete site access forms.
o Fermilab badges are to be worn 100% of the time while on Fermilab property.
o Badges must be worn on the front of the body, below the neck, and above the waist. The Fermilab Point of Contact will guide personnel through the badging process. Badges are not to be worn off-site.
o Personnel who escort non-badged personnel must follow current escorting policies and escorts must complete the Escort Responsibilities at Fermilab Sites training.
• Sub-tier subcontractors are required to submit the following submittals after Notice to Proceed:
o Sub-tier subcontractor employee safety training records o Sub-tier subcontractors shall be evaluated utilizing the Fermilab Subcontractor Safety Information Questionnaire. The questionnaire will be submitted to the Subcontractor and FFDG CC/TM for review upon selection of the sub-tier subcontractor.
Experience Modification Rating (EMR) Rates and Occupational Safety and Health Administration (OSHA) 300 logs for the last three years for sub-tier subcontractors. The expectation is that the EMR be below 1.0. If an EMR is 1.0 or above, or the sub-tier subcontractor has less than 3 years of ratings, the sub-tier subcontractor shall provide explanation.
o Sub-tier Subcontractor Construction ES&H Certification To ensure acceptance to 10 CFR 851, the sub-tier subcontractor shall complete and submit the Construction ES&H Certification to the subcontractor, who will submit to the FFDG CC/TM for review.
• FFDG reserves the right to reject a sub-tier subcontractor whose EMR is 1.0 or above or safety records and training are not complete. In this circumstance, the subcontractor shall replace the sub-tier at no additional cost to FRA.
• Emergency procedures shall be well developed with local care centers identified and clear procedures of communication to those centers. The emergency procedures shall detail communications with the Fermilab Construction Coordinator. In accordance with the 13100, procedures to preserve the scene of any incident must be clearly defined in the Site-Specific Plan.
